His heart had stopped beating. It is hard to describe how I felt in that moment as I am still trying to
process my emotions. I remember turning away from the table where the patient lay lifeless, and
staring at the sink in the far corner. A single tear rolled down by flushed face. Suddenly, I felt as if

Introduction Every day in the United States there are 136.3 million people that visit the emergency
department (CDC, 2015). To put this number into perspective that is 44.5 people per 100 persons
(CDC, 2015). 11.9% of these visits result in hospital admission leaving 88.1% of visits to be
discharged home with or without caregiver assistance or to another healthcare facility (CDC, 2015).
While high numbers of patients being discharged is desirable, it is important to consider that "In one
out of every 30 discharges things get missed. [For example] patients [are] sent home who didn't

EMTs have so–called chores they need to perform such as "[cleaning] the equipment.[They need to]
check and replace the used and damaged supplies." This is in case if on a call they don't have any
important damaged supplies that could be used to save someone's life. Other duties of the job can

begins with what interventions the ambulance crew provides if one is called. In the prehospital
environment, there are four levels of providers: Emergency Medical Responder (EMR), Emergency
Medical Technician (EMT), Advanced Emergency Medical Technician (AEMT), and Paramedic
(National Scope of Practice Model, 20–21). The EMT is considered the "backbone" of prehospital
emergency medicine (National Scope of Practice Model 23). It is the first level that a provider can
either give a patient medicine or assist with the patient's own medications (National Scope of
Practice Model 24). Medicines that an EMT in Kansas can use are supplemental oxygen,
nitroglycerin, aspirin, epinephrine and glucagon by auto injector, oral glucose, activated charcoal,

The Medical Field Of Medical Emergencies
Growing up in a rural Eastern Oregon community, my earliest memories of medicine were of the
mobile clinic that a doctor set up every six months. The town flocked to him, and for some, this was
the only medical care they received. Yet, the greatest challenge this setting presented was in medical
emergencies.
I remember getting the call. Alycia was in a car wreck. My heart was racing. She was my best friend,
cousin, and roommate for the last three months. After the two–hour ride to the small local hospital,
they airlifted her to the nearest trauma center. A few hours of stabilization and tests later, the results
were grim: traumatic brain injury, broken ribs and arm, collapsed lungs and several lacerated organs.
After maintaining a comatose state for a year, she died.
Despite the circumstances, Alycia's hospital stay provided me with a greater exposure to medicine
than I ever had growing up. I learned to admire her doctors and their gentle, yet honest, ways of
interacting with patients and family. As a lover of science, I would ask to talk to them after family
meetings to get more details on what they described and learn about their procedures. Through these
interactions, I gained a respect and admiration for medicine, and a scientific curiosity was fostered in

Einstein Medical Center, located in North Philadelphia, is a leading Level 1 Regional Resource
Trauma Center and Certified Stroke Center. The facility has 772 beds and maintains one of the
busiest emergency rooms in Philadelphia. Einstein's emergency department treats over 95,000
patients each year, providing cutting edge care to patients with a wide variety of illnesses and

Einstein Medical Center services a largely urban patient population. Located in North Philadelphia,
the medical center caters to various ethnic groups. The majority of North Philadelphia's population
consists of African Americans and Hispanics, mainly Puerto Ricans. Although there is a significant
presence of many other racial groups including Irish, Dominican, White Americans, and Italians,
North Philadelphia has the highest percentage of Puerto Ricans in the nation (United States Census
Bureau). "According to the American Heart Association, the cardiovascular disease death rate among
African Americans is 34 percent higher than for the overall U.S. population" (Seconds Count.org).
The American Heart Association states that heart disease is the number one cause of death for all
Americans and strokes are the fifth highest. Much like African Americans, Hispanics and Latinos
"face even higher risks of cardiovascular diseases" (American Heart Association). Since North
Philadelphia's population is comprised of mainly African Americans and Hispanics, the hospital
patient population will most likely present with more cases of cardiovascular disease.
In addition, more than half of North Philadelphia's population and 28% of the entire city live below
the federal poverty line, about $20,000 of income per year (United States Census Bureau). North

Medical Director For Snohomish County Emergency Medical...
Dr. Eric Cooper is the medical director for Snohomish County Emergency Medical Services
(SCEMS). SCEMS is made up of numerous Fire Departments and Private Ambulance providers to
serve the citizens of the county (E. Cooper, personal communication, May 23, 2016). All of the Fire
Department services are on the same instance of Electronic Health Record (EHR). Implementation of
the EHR began in 2010, with Advanced Life Support entities. In 2013 the EHR vendor added a
feature called Health Data Exchange (HDE) as an option. This feature allows bi–directional flow of
information from the pre–hospital EHR to the hospital EHR. For the first time, pre–hospital

One reason that Providence won this award is the fact that they have a single instance of Epic
installed system wide. It should be noted that Providence and Swedish, although merged, do operate
on two different instances of Epic EHR. In order to transition to a single instance of Epic the cost
would be $30 million, compared to a $500,000 interface that connects the two systems (E. Cooper,
personal communication, May 23, 2016).
Proposal
McDonald (1997) points out that health care data is siloed in multiple areas that are inaccessible to
others. This kind of management of patient data does not serve the patient well. It is for this reason
that SCEMS approached Providence and Swedish hospitals to propose implementation of HDE.
Moreover, as stated in the McDonald article a feasible way to integrate data from disparate sources is
through the use of interfaces such as the HDE. In addition, a problem that exists in the integration of
these two data sources is the fact that the hospital system communicates via the standard HL7
language, while the pre–hospital system communicates via XML. Fortunately, the HDE structure
accounts for this difference by translating back and forth between the two different languages. With

The Techniques Used By Emergency Medical Services
Introduction
The topic proposed for research is to discuss and compare the techniques used by emergency medical
services to fully immobilise a patient with suspected spinal injuries with two highly debatable
methods, Spine Immobilisation and Spinal Motion Restriction (SMR). The research collected will
determine the safest and most practical technique, comparing the advantages and disadvantages of
the two.
Discussion
Spinal Immobilisation involves the use of a number of devices to stabilize the spine after injury,
which will consequently prevent further damage to the spine, spinal cord, surrounding tissue and
vital organs. Spinal motion restriction on the other hand, can be described as being the preferred
practice amongst emergency medical practitioners. Spinal motion restriction aims to maintain the
spine in anatomic alignment and reduce the amount of movement as much as possible. (Physicians,
2015)
Spinal immobilisation is considered for utilization for two specific patient groups: (1) unconscious
injury victims and (2) any patient with a motion injury. These two specific groups lead to the
perception that these patients have a possible spinal injury and immobilisation is required.
(M.J.Sanders, 2010) Applying clear, clinical guidelines for evaluating a possible spinal injury in a
patient can support the accuracy of pre–hospital assessment. The following signs and symptoms may
assist in the decision of full spinal immobilisation:
