This talent release form gives Streetlight Productions unlimited rights to videotape, copy, publish, distribute, and use footage of the signing talent filmed on a specified date. The talent agrees to abandon all future claims against the production company regarding use of the filmed material within the scope outlined by the agreement. Both the talent and producers must sign and date the form to finalize the rights transfer.

I understand that there is a videotape being recorded that will be featuring me on the date of

I assign and authorise the producers of Streetlight Productions all the rights to such a
videotape. I also authorise the producers, with no limitations, the right to reproduce, copy
and publish or distribute the videotape and abandon all the rights or claims I may have in the
future against your organisation or any of its additions or other assignees other than in
anything stated in this agreement.
