2. Ordering of ophthalmic lens
Use a separate form for each order.
Do not include unnecessary data or information on
any forms if such data are not necessary or
applicable to the order.
3. Lens Information
When written on a blank prescription pad, such a
prescription would be written with the left lens
value directly below that of the right lens.
OD: −3.00 D sph
OS: −2.75 −1.50 × 175
4. Always use at least three figures for the sphere
and cylinder components.
If the dioptric unit is less than 1.00 D, use a
prefatory zero before the decimal point, as for
example, +0.75 D.
Carry figures two places after the decimal point,
as for example, +2.00 D (not +2 D) or −1.50 D (not
−1.5 D).
5. State the axis as x, but do not put a degree sign
after the numbers representing the cylinder axis
because it may be mistakenly read as an extra
zero.
6. Frame Information
Be sure to specify the style of temple desired if
more than one style is available.
Print the name of the frame and include the name
of the manufacturer.
7. Obtaining Lens Information for
Existing Spectacles
Take New Facial Measurements.
Retake the wearer’s PD.
If the lenses are either bifocal or trifocal lenses,
note where the segment line falls in reference to
the lower limbus.

9. (1) Some recommend that the wearer’s last name
be written here in all capital letters.
(2) List only if ordering special base curves.
Otherwise leave blank.
(3) List the MRP height only this. fitting cross
height is listed for progressive addition lenses.
10. (4) Do not give both binocular and monocular PD s.
Use either one or the other, but not both.
(5) You must indicate a lens material.
(6) Normally you do not specify a thickness for
the lens.
(7) Indicate multifocal type and seg size, or
progressive lens brand when appropriate.
11. (8) Indicate as specifically as possible what color
lens you want. Indicate the desired transmission
or shade.
lenses have no tint, write “clear” or
“white.”
(9) If slab-off is being ordered, indicate the prism
amount of the slab-off.
12. (10) The frame name & the manufacturer must be
listed.
(11) Color may be a number.
(12) Temple name : skull, cable, riding bow,
library.
(13) Indicate whether the frame is metal, plastic,
or rimless.
13. (14) Special Instructions” is for calling attention
to anything unusual about the prescription.
e.g.:- SRC,UV,ARC
(15) Signature of the person or the name of the
office that is ordering the prescription.
