12. Write three goals (two core subjects and one behavior); make sure they are measurable, realistic, content base, achievable.

One SST form completely fill in with the selected child’s information (you may need to look at the cum for information, confer with other professionals who have dealt with the child, home visit or phone conversation with parent, consult with past teacher and present teacher, if you are neither of them) Note: please use child’s first name only
16. If possible, student will turned in the SST referral and will document its result
17. Typed reflection of the SST process, include the reason why this particular child attracted your attention, any aha moments, disillusions, or comments you may think will give me a clear vision of your endeavor.
