                          Student Participation Journal Format

Student participation in classes is an essential ingredient to maximizing learning. Student
participation consists of many activities, some of which include:

1. Class discussion including questions or comments
2. Visiting the instructor following class, using email, or scheduled appointments to
   clarify and convey ideas
3. Engaging in electronic learning via the internet relative to each of the following
   disorder areas
       a. Articulation
       b. Voice and Resonance
       c. Fluency
       d. Swallowing/Dysphagia
       e. Language
       f. Cognition
       g. Social aspects
       h. Communication modalities
       I. Hearing/Aural Rehabilitation
       j. Accent Modification

4. Demonstrating initiative and enthusiasm for learning through creative thought and
   ideas about the subject/topics

Each student will briefly record the activity or level of participation (25 words or less per
entry/double spaced between dates of entry) using the prescribed electronic format,
keeping the journal entries in sections according to their type. For example, all #1 type
entries must be kept together, #2 etc, then #3, etc…….

The student will maintain the information and be prepared to present it to the instructor
for review and assignment of cumulative points. (Due November 23, 2011)—No
exceptions.

Total Required Points: 65 points

Note: Failure to complete the assignment as represented or present the information
by the prescribed due date will result in a grade of “ZERO” for this journal
assignment…..It’s all or nothing…..not debatable.
