HEALTHCARE ENVIRONMENT
High-quality healthcare organizations are driven by attention to
their customers, excellent workmanship, empowered
workforces, and
innovation and change as a response to their environment. The
Patient Protection and Affordable Care Act of 2010 established
the mandate
of increasing access to care while decreasing costs. Ongoing
healthcare reform emphasizes the importance of collaborative
teams in the
redesign of healthcare delivery systems that provide high-
quality, client-centered, mutually accountable care across the
lifespan (Roussel,
Thomas, & Harris, 2016). Consequently, healthcare providers
are challenged to focus on quality, cost-effectiveness, and
responsiveness to
the voice of the consumer in all aspects of care.
The overall goal of transforming the healthcare system is to
develop a seamless, coordinated care approach for clients by: 1)
improving
population health and client outcomes, 2) enhancing the client
experience, and 3) continued improvements in providing high-
quality services
at a lower cost (Stiefel & Nolan, 2012). Traditional measures of
these outcomes are now being combined into value-based
competitive
reimbursement models from third-party payors and regulatory
agencies. It is generally accepted that in order to survive
financially and
competitively, healthcare organizations must break down
traditional organizational hierarchies and embrace a culture of
collaboration and
teamwork among providers and stakeholders (including
consumers) at all levels. High-quality healthcare organizations
recognize the
environmental factors inherent in maintaining competitive
advantage and ensuring financial viability include an
examination of organizational
culture with an emphasis on team-building, group dynamics,
conflict resolution, negotiation, reframing historical sources of
power,
environmental issues, cultural competence, and diversity.
ORGANIZATIONAL CULTURE AND ENVIRONMENT
It is impossible to promote high-performing teams without true
commitment to teamwork at all levels. Shared vision, open
dialogue, trust, and
a sense of belonging are critical elements for overcoming
organizational challenges and ongoing strategic development.
Consequently, it is
important to insure that the culture, alignment of corporate
objectives, and educational/technical supports are congruent
with establishing a
culture of teamwork and collaboration. Longstanding paradigms
of organizational hierarchies, privileged conversations, and
discipline-
specific silos must be replaced by an organizational climate that
fosters equalization of power, shared governance, participative
management, transparency, open communication, and
individual/group contributions to organizational goals. Synergy
of vision and desired
outcome is essential. Team-related activities and organizational
objectives must also be aligned. Stakeholders must understand
how their
contributions align with corporate objectives and feel that their
efforts are valued. It is increasingly recognized that alignment
of the culture
and capabilities of an organization are derived from
philosophies and practices related to the strategic management
of human capital
(including the free flow of information, job security, selective
hiring practices, elimination of status differences between
groups,
decentralization of authority, and team-building) as basic
elements of organizational design (Pfeffer & Veiga, 1999).
TEAMBUILDING
“A team remains the most flexible and most powerful unit of
performance, learning, and change in any organization”
(Katzenbach & Smith,
2003, p. XIX). The abilities to express one’s ideas clearly and
decisively, to listen attentively and respectfully, and to invite a
range of
opinions are among the communication skills that help
managers build team cohesiveness. Teams usually are developed
reflecting the
diversity and culture of the organization, and although the chief
executive may be fundamental in the formation of high-
performance teams,
he or she is not always a team leader.
According to Katzenbach and Smith (2003), developing a team
is the inclusion of “a small number of people with
complementary skills who
are committed to a common purpose, performance goals, and
approach for which they hold themselves mutually accountable”
(p. 275). A
team develops around the task to be accomplished while taking
into account the mix of personalities and competencies; the
desired
performance outcomes; and the processes of communication,
involvement, performance orientation, and enabling leadership
(Higgs, 2006).
Central to a team’s success will be its members’ work behaviors
such as constructive listening and giving the benefit of the
doubt to others
(Katzenbach & Smith, 2003). Team members need to not only
be mature and self-motivated, but also be aware of their social
and cultural
differences, appreciate each other’s cultural diversity, and have
a basic understanding for each other’s value systems. Only
through an active
display of this understanding and open and nonjudgmental
communication can a team develop its highest potential to meet
group goals and
complete its mission.
The team leader can facilitate this process by using good
meeting skills and appropriate meeting behaviors. The task of
reminding team
members of their mission, goal, and tasks falls to the leader as
well.
GROUP DYNAMICS
Teams tend to evolve through a predictable development
process that was first described by Tuckman in 1965 and then
modified by
Tuckman and Jensen in 1977 (Smith, 2005). The five stages
include: Forming, Norming, Storming, Performing and
Adjourning. In the
Forming stage individuals come together and begin to establish
team relationships. While there is often a sense of enthusiasm
for the task at
hand, individuals tend to be cautious in their communication
with one another; as they may be relative strangers and may
need purposeful
teambuilding activities. A skilled team leader or facilitator may
be needed to clarify team purpose, goals, and to assist the team
in
establishing rules of engagement.
As the group proceeds through the maturation process, it arrives
at the second stage, known as Storming. In this stage, members
of the
group compete for position, power, and status; informal leaders
may emerge. When team members come from differing
backgrounds there is
a tendency to approach critical issues from differing standpoints
and opinions that cause uneasiness and conflict among group
members.
These differences need to be openly confronted and addressed
proactively so that effective resolution can be achieved occur in
a timely
fashion (Kelly, 2008). The storming phase of group work is
critical to team success. Katzenbach and Smith (2003) assert
that real teams
don’t emerge unless individuals take risks, address conflict, and
develop a sense of trust and interdependence.
The third stage of group formation, Norming, is where the rules
for working collaboratively are made explicit; structure, roles,
and
relationships are further clarified resulting in a sense of
cohesion. Team members become more adept at resolving
conflict, respect
differences in opinion and work cooperatively to achieve team
goals. The leader’s role during this stage of group development
focuses on
relationship building and assisting the group in overcoming
barriers.
As the group matures, it enters the Performing stage. In this
phase, group cohesion, collaboration, and solidarity are evident.
Personal biases
are overshadowed in an effort to achieve team goals (Kelly,
2008). The leader’s role is to provide feedback on the work that
the group is
accomplishing, redirect group energy when necessary, and
further cultivate interpersonal relationships.
The final stage of team development, Adjourning, addresses the
individual and group tasks associated with completion of the
team’s specific
charge and dissolution of the formal team structure. During this
phase of termination and consolidation, the ream reviews their
activities and
evaluates the outcome of their efforts (Kelly, 2008). The
development of strong interpersonal relationships inherent in
team-building and the
loss of team structure may be difficult for some team members.
Consequently, a focus on closure is paramount. At this stage it
is appropriate
to recognize team accomplishments and to facilitate an ongoing
culture of collaboration and teamwork (Roussel, Thomas, &
Harris, 2016).
Celebration of both successes and failures are essential for
building a culture of inclusion and cooperation by: valuing the
process and the
contributions of individuals involved, supporting an ongoing,
proactive culture of change, celebrating insights gained and
providing a sense of
renewal that will serve as the cornerstone for ongoing
collaboration and teamwork necessary to meet the inherent
challenges of redesigning
healthcare delivery now and in the future.
Team development and function rarely proceed in a completely
linear fashion. Changing circumstances and intervening
variables may have
a profound impact on team function, alignment, and outcomes.
Consequently, management intervention may be needed when
teams regress
or encounter seemingly insurmountable barriers to performance.
In these instances the leader should consider strategies to assist
teams in
overcoming obstacles, evaluate whether modifications to team
membership are needed, and in extreme cases, determine
whether planned
abandonment of the team activity is needed. Experts have also
suggested that the group formation process, and thus the
productivity of the
group, can be accelerated under the guidance of a skillful
facilitator or in the face of actual or fabricated crisis
(McKnight, Kaney, & Brewer,
2010).
CONFLICT AND CONFLICT RESOLUTION
Conflict occurs naturally in and among groups and individuals;
it is inevitable and a condition essential to change. Although
conflict is often is
viewed negatively, if managed constructively it can provide a
sound foundation for the evolution of high-performing teams as
members bring
their unique insights, perspectives, and values to develop a
shared vision and plan of action. New paradigms for
constructive conflict
resolution include replacement of old attitudes of competition
with new approaches that foster collaborative problem-solving,
shared
accountability, respect, open and honest communication, a
willingness to let go of past paradigms in favor of exploring
new possibilities; and
developing a mutually agreed-upon plan of action (Katzenbach
& Smith, 2003).
Conflict may be intrapersonal (within oneself), interpersonal
(between the self and another person), intragroup (among
members of a
particular group), or intergroup (among members of two or more
groups). Other types of conflict include competitive conflict and
disruptive or
destructive conflict. In both instances, the desired outcome is to
overcome one’s opponent (i.e., to “win”).
Conflict management occupies a significant portion of a
leader’s work. Some suggest that at least one-quarter of the
leader’s time is spent in
conflict management activities. The challenge for the leader is,
of course, to help her or his subordinates reach a “win–win”
outcome in which
the parties to the conflict each believe they have come away
from the encounter with a sense of resolution. Strategies include
• Focusing on goals, not personalities;
• Meeting the needs of both parties, equally if at all possible;
and
• Building consensus.
Achieving a win–win outcome is much easier in the abstract
than in the workplace setting. Nonetheless, it is a worthwhile
goal.
Conflict is a state of disharmony or disequilibrium. Managing
conflict requires maintaining the level of conflict neither too
high nor too low,
with the ultimate goal being to stimulate growth and coping
behavior without reaching the point where the conflict seems
overwhelming.
Common conflict resolution strategies include compromise,
competing, cooperating, smoothing, avoiding, and collaborating.
The choice of
the most appropriate strategy depends upon many variables such
as the urgency of the decision, inherent power and status
disparities, and
the maturity of individuals involved in the conflict (Marquise &
Huston, 2009).
NEGOTIATION
Negotiation can be thought of as a formal process; one example
is the negotiations that take place at the time of contract
deliberations
between unions and management. Negotiation also can be
thought of as a political process in that it is a “power play”
among individuals who
compete to “win” but generally compromise in the end.
The keys to successful negotiations include the principles of
• Separating the people from the problem,
• Focusing on interests, not positions,
• Inventing options for mutual gains, and
• Insisting on objective criteria.
Interest-based negotiation is a somewhat newer concept, the
principles of which were outlined by Fisher, Ury, and Patton
(1991) in Getting to
Yes: Negotiating Agreement Without Giving In. Bizony (1999)
has distilled the principles listed below from Fisher and
colleagues’ work on the
Harvard Negotiation Project; their value has been convincingly
promulgated through the years.
• Treat people as equals.
• Resolve issues on their merits.
• Define issues with a definition that is acceptable to all parties.
• Focus on interests, not on conclusions or positions.
• Develop options that may meet the interests of both parties.
• Apply objective standards to resolve conflicting interests.
Twelve points of negotiation can prepare leaders for successful
negotiations.
1. The greatest failure in negotiation is failing to negotiate.
2. The most important person to know in negotiation is yourself.
3. Everyone has power in negotiation.
4. Single-issue bargaining leaves both parties unsatisfied.
5. Urgency drives decisions.
6. Agreement is the end; trade-offs are the means.
7. The best results are obtained by keeping the other party on a
need-to-know basis.
8. The value of something is always in the eye of the beholder.
9. Success in negotiation is directly related to the amount and
kind of preparation preceding it.
10. Being able to walk away or select an alternative to a
negotiated agreement puts a negotiator in a very strong position.
11. Two sides can always agree on something, even when they
are far apart on major issues.
12. Conflict is a part of meaningful negotiation.
Mediation as a method of dispute resolution uses a neutral third
party who attempts to bring parties together to solve a conflict.
The
mediator’s role is to fact-find, make suggestions, and draft a
solution that is agreeable to both parties (Epstein, 2003).
SOURCES OF POWER AND EMPOWERMENT
Power is the capacity to act and the energy to mobilize
resources to create change and achieve goals (Kelly, 2008).
Whether power has a
positive or a negative connotation depends on how it is used.
The most widely accepted power-base classification identifies
five sources of
power inherent in interpersonal interactions. The categories
include: legitimate power, reward power, coercive power,
referent power, and
expert power (Huber, 2010).
• Legitimate power is the right to command within an
organizational structure or setting based upon the position held.
The reciprocal
nature of this power obligates employees to comply with
legitimate orders.
• Reward power is based on the ability to deliver desired
rewards or the ability to compensate others in some way.
Compensation need
not be monetary.
• Coercive power is derived from the ability to threaten
punishment or deliver penalties. It is a source of power used to
apply pressure so
that others will meet what is demanded.
• Referent power means that the individual has characteristics
(charisma) that appeal to others resulting in the ability to
influence without
having to offer rewards or threaten punishment.
• Expert power means that the individual has specialized
expertise or knowledge that can influence the actions of others
toward certain
outcomes or goals.
All sources of power are potentially important, and the
assumption is that the most powerful leaders are those who have
high legitimate,
reward, or coercive power. However, one should not
underestimate the strength of referent and expertise power.
These sources of power are
more closely related to personal motivation and may, in the long
run, make a greater and more lasting difference within a
company or
organization. It is not necessarily the leaders whose names are
well known or who sit atop the organizational chart who exert
the most power
of this type.
In today’s healthcare environment it is important that all staff
are empowered to make decisions, especially those decisions
that positively
influence client outcomes. An understanding of the concepts of
power and empowerment is also essential if nurses are to realize
their full
potential for leadership and ongoing professional advancement.
ADAPTING TO CHANGE
Change is the province of leaders. It is the work of inspiring
people to do things differently, to struggle against uncertain
odds, and to
persevere toward a misty image of a better future (Kouzses &
Posner, 2012). Consequently it is imperative that nurse
executives understand
and use strategies that will support staff members in proactively
adapting to change.
Appreciative Inquiry
Appreciative inquiry is the process in which an organization
asks questions of its members in seeking information that can
be used to
anticipate and identify areas of potential strengths. The four
characteristics of appreciative inquiry include
• Appreciative,
• Applicable,
• Provocative, and
• Collaborative.
Appreciative inquiry is best described through the 4 Ds:
1. Discovering the best of what is,
2. Dreaming what might be,
3. Designing what should be, and
4. Creating a destiny of what will be.
Appreciative inquiry is to organizations what creative
visualization is to individuals; both are positive approaches to
foster change.
Appreciative inquiry is considered to be a “soft business
strategy” that can be used to create organizational visions, build
cultures, and align
groups to achieve organizational goals. Through imagination
and thoughtful analysis, appreciative inquiry can contribute to
measurable
results for organizations.
Crisis Management
A crisis is anything that has the potential to significantly affect
an organization. Organizations with crisis management plans are
better able to
work effectively with local responders, promptly attend to the
needs of those affected, assist investigating bodies without
jeopardizing the
organization’s legal position, provide for accurate and timely
information, and minimize damage to the organization’s
reputation.
The four objectives of crisis management are
1. Reducing tension during the incident;
2. Demonstrating organizational commitment and expertise;
3. Controlling the flow and accuracy of information; and
4. Managing resources effectively.
Managing a crisis begins with the creation of a crisis
management team and assessing potential crises before they
occur. Managing a crisis
also includes
• Developing crisis management team plans;
• Establishing guidelines for gathering information and
beginning an internal investigation;
• Providing periodic crisis training evaluation; and
• Developing guidelines for crisis communication.
The key points for crisis management of any dimension include
• Having a flexible structure capable of responding to any cr isis
quickly, decisively, and in a coordinated manner;
• Preparing operational contingency plans;
• Creating and communicating a document retention policy
(e.g., nothing is thrown away that might enhance an
investigation or document
the event);
• Developing training that includes addressing legal issues
before they occur, developing investigational procedures,
identifying necessary
equipment and systems before a crisis, and developing good
media relations skills;
• Preparing to communicate with a variety of constituents,
including employees, the media, neighbors, investors,
regulators, and
lawmakers; and
• Preparing a business contingency plan to minimize disruption
and damage.
Because crisis management is the manner in which
organizations respond to unexpected events over which they
have little or no control,
and “uncontrollable” situations occur with relative frequency in
health care, developing purposeful intervention strategies seems
prudent. The
“truthful disclosure” approach is one that healthcare
organizations take in order to maintain public trust.
A classic example of a crisis management in healthcare products
can be seen in the Johnson & Johnson’s handling of the
Tylenol®
tampering crisis (Kaplan, 1998). In 1982, seven people in the
Chicago area died after taking Extra-Strength Tylenol capsules
that had been
injected with cyanide. The containers had been tampered with
after they left the manufacturing plant. Rather than claim that
the company
was not to blame, Johnson & Johnson immediately launched a
public relations program to preserve the integrity of the product
and the
company.
Marketing experts believed that Tylenol would disappear from
pharmacy shelves, never to be mentioned again except in
negative terms.
However, Johnson & Johnson’s leaders put public safety first
and worried about financial impact later. They alerted
consumers throughout
the nation to avoid the consumption of any Tylenol product
until the extent of the tampering could be determined. They
stopped production of
all Tylenol products and recalled all Tylenol capsules from the
market at a cost of more than $100 million. They offered to
replace any Tylenol
capsules people had already purchased with Tylenol tablets.
They quickly began working with the Chicago Police
Department, the Federal
Bureau of Investigation, and the Federal Drug Administration.
They put up $100,000 in reward money to help catch the
perpetrator of the
crime.
Not only did the company survive and thrive, Tylenol remains
one of its biggest-selling and most profitable products. The
forthright approach
of the company reassured the community that its safety came
first and that Johnson & Johnson cared enough to be publicly
open and truthful
about the crisis (Kaplan, 1998).
Crisis management by companies such as Qantas Airlines,
Exxon-Mobil, and Boeing demonstrates the effectiveness of the
four objectives of
crisis management in responding to the crisis in an organized
format, protecting an organization’s reputation, communicating
effectively
through company channels to resolve the crisis, and normalizing
operations following catastrophic events.
ENVIRONMENTAL FACTORS
Cultural Competence
Cultural competence can be defined as the ability to interact
effectively with people of different cultures with a focus on
personal awareness
of one’s own culture and attitude toward culture, and
development of knowledge and skill across multiple cultures.
Cultural competence is
essential as a leadership strategy in today’s healthcare
environment. Our “shrinking” planet, globalization, and
population movement bring
people from different cultures throughout the world together in
the workplace, and people may have widely divergent
understandings of
identity and society. Globalization has increased the need for
awareness of cultural ideas and expectations other than our own
in an
environment where intermixing national, religious, and ethnic
identities may lead to conflict.
The world’s population is expected to double by 2050
(Kotlikoff & Burns, 2004). Industrialized nations are “graying,”
while 80% of the world’s
population growth is in developing nations. Challenges for the
workplace include language and cultural differences, increasing
incidence of
chronic illness, generational issues, maximization of resource
use, and ethical differences.
Strategies for success in cultural competence include
• Know your own culture, values, and biases.
• Listen and observe.
• Emphasize corporate values at all times.
• Be a teacher and a learner.
• Hold up your end of the bargain by displaying awareness of
personal cultural practices.
• Give clear directions and provide resources.
• Delegate the responsibility for outcomes.
• Give the big picture.
• Consider the rules and procedures from all perspectives.
A leader’s responsibility in cultural competence includes
• Managing personal expectations,
• Providing straightforward steps for decision-making,
• Being courageous and displaying correct behavior,
• Applying leadership and management skills according to
values and attitudes, and
• Providing employees the opportunity to grow.
Because today’s workforce consists of four generations (i.e.,
mature, baby boomers, generation x, millennials), for the first
time, a leader also
must consider generational diversity as well as cultural
diversity. There are challenges relating to work ethics, duty,
and sacrifice for the job.
A leader’s role in cultural and generational competence in
today’s healthcare organization includes
• Creating a culture that has a diversity-sensitive orientation;
• Building the blocks of culturally and linguistically competent
healthcare delivery;
• Recruiting and retaining a diverse workforce; and
• Ensuring the success of a culturally competent organization.
Effective leaders are aware of and make use of cultural
competence to maximize the benefits of diversity in their staff
and client population
and minimize the costs in delivering healthcare services today.
In nursing practice, nurse leaders should encourage members of
other
cultures to become nurses, discuss the benefits of nursing, and
support efforts of nurses from other cultures to assimilate into
nursing’s
culture.
The time to consider cultural and generational diversity is now.
The nurse leader must consider the change in the population
demographics,
that industrialized nations are “graying” and the workforce is
too. A survey of staff demographics can help the nurse leader
prepare for an
increase in chronic illness among the workforce as well as
generational workforce issues and language and cultural
challenges that might
disrupt optimal effectiveness of operations (Kotlikoff & Burns,
2004).
Leveraging Diversity
Leveraging diversity is the ability of an organization to become
culturally competent and, in doing so, value diversity from both
a personal (the
organization) and business (the customer base) perspective
(Shipp & Davidson, 2001). This business strategy links the
workforce, the
workplace structure, and the marketplace and displays the
ability of organizations and their leaders to recognize and use
every advantage to
ensure success. Leveraging diversity maximizes all talents and
intellectual capital within an organization and ensures that all
persons are
included in every aspect of the organization.
Leveraging diversity is known to boost employee morale,
reduce grievances, and enhance problem-solving and decision-
making abilities.
This approach to management of human capital in an
organization reduces barriers, enabling all employees to fully
use their talents on
behalf of the organization.
Leveraging diversity in the marketplace includes serving
customer groups with a sensitivity to their cultures and allows
an organization to
maintain a multicultural perspective. Cultural diversity can be
leveraged for bottom-line impact and therefore ties directly to
business strategy
and practices.
There are three leadership behavioral elements of leveraging
diversity.
1. The cultural element includes exerting influence within the
organizational culture to set the tone for underlying values,
beliefs, and
principles.
2. The leadership element includes giving employees a sense of
direction, meaning, and purpose to navigate organizational
waters as
well as recognizing and using the full potential of all persons in
the organization.
3. The connectivity element includes building a bridge between
cultural and leadership elements and organizational intellectual
capital,
maintaining vision clarity, sharing power, and identifying and
assessing problems.
The end result of leadership support and participation in
leveraging diversity for an organization is an increase in
productivity, efficiency, and
quality, and ultimately an improved work environment.
The nurse executive is a role model in setting the tone and
direction of an organization’s daily work effort. The nurse
executive acts as a
coach and supporter of diversity among all stakeholder groups
to achieve the end goals and objectives of the organization and
its
constituencies.
Personnel Matters While Leveraging Diversity in the Workforce
Leveraging diversity in the workforce overlaps with human
capital and human resources management functions. Areas to
consider include
writing and using objective documents, including job
descriptions and performance appraisals. Leveraging diversity
in the workplace should
be a consideration during personnel selection efforts and
orientation and training of new employees. Addressing
behavioral issues and
guiding employees to organizational support mechanisms such
as employee assistance for alcohol, drug, or other issues are
also times
when leveraging diversity can increase an organization’s
productivity, efficiency, and quality, and ultimately lead to an
improved work
environment. Providing the assistance or the mechanism for
employee assistance to all employees promotes increased
productivity and
leverages diversity.
Vulnerability audits and exit interviews offer opportunities to
gather data about the work environment and consider whether
this should be the
time of change for job enrichment, engineering, rotation, or
enlargement opportunities. Enhancing the work environment
through job
enrichment makes jobs more interesting and challenging,
maximizes efficiency, and adds more variety to daily tasks.
Goal-setting with
employees provides objectives to structure the job while
considering that job rotation adds variety (Slocum, 1981).
Organizational Transparency
Current management literature defines organizational
transparency intentionally shared information exchanged
between two parties. It is
distinguished by the degree of information disclosed, clarity and
accuracy (Schnackenberg &Tomlinson, 2014). A transparent
organization
encourages behaviors that support access to information,
participation, and decision-making. These behaviors create a
higher level of trust
among all stakeholders.
Organizational transparency implies a trusting environment
wherein transparency and trust are interconnected. Cultural trust
in the
organization requires clarity and consensus about what
constitutes success, open access to information, and confidence
in the competence
of all involved. Leaders should consider creating an
environment that fits the definition of success within the
organization and for the
organization.
Other important leadership behaviors include
• Practicing nonmanipulative leadership,
• Communicating frequently and repeating important
information,
• Opening up access to documents regarding decisions,
• Sharing background information about important decisions,
• Providing clear financial reporting, and
• Hiring and appointing trustworthy people.
Challenges to organizational transparency include
• Risks from potential distortion of the truth through increased
access to information,
• A slower-than-usual decision-making process, and
• Additional organizational vulnerabilities.
Organizational transparency does not guarantee that the right
decisions are being made and may require additional time and
resources at all
organizational levels. Transparency may at some point have
diminishing returns, when communication, information-sharing,
and trust levels
reach a status quo and information flows in all directions. In the
final analysis, the value of organizational transparency lies in
the protection
and promotion of an organization’s reputation, a corporate
culture of communication among all stakeholders, and the
involvement of current
and future members in decision-making processes (Fung,

Transforming healthcare delivery requires robust data
collection, and analysis of a broad range of performance
measures. Transparency is
critical is establishing a culture of safety embraced by all levels
of staff and key stakeholders. Transparency affords healthcare
organizations
with a complete view of operational performance that enables
comparison with other like facilities and standards of practice
(Kerfoot, 2009).
The publication of nursing outcomes and program evaluation
reports is an example of organizational transparency. Nurse
executives serve
as role models and should support transparency, promote
communication in all directions, and participate in shared
decision-making within
the organization.
Lateral Violence (Nurse Bullying)
Lateral violence is aggressive and destructive behavior, in this
case, of nurses against each other (Woelfle & McCaffery, 2007).
The end
result is damage to another’s dignity, confidence, and self-
esteem. Often, those who experience lateral violence in the
workplace then
transfer it to others.
Lateral violence in nursing can consist of a variety of behaviors,
ranging from unintentional, thoughtless acts to purposeful,
intentional,
destructive acts meant to harm, intimidate, or humiliate a group
or individual. Behaviors can range from random instances to a
pattern, and
such behaviors can create a hostile work environment.
Examples of behavioral lateral violence include
• Talking behind others’ backs;
• Scapegoating;
• Criticizing a colleague in front of others;
• Excluding a coworker from group interaction;
• Withholding pertinent information;
• Violating a coworker’s privacy and confidentiality;
• Making inappropriate, condescending remarks; and
• Displaying inappropriate nonverbal language, such as making
faces or raising one’s eyebrows.
Any time there exists an “us vs. them” attitude or when an
imbalance of power occurs, conditions are prime for lateral
violence. Examples of
relationships in which lateral violence can occur include nurse
manager to a staff nurse, nurse executive to a nurse manager or
a staff nurse,
nursing faculty member to a student nurse, and peer to peer.
The consequences of lateral violence can manifest themselves as
physical symptoms, increased absenteeism and, in extreme
cases, suicide
of the targeted employee. Increased turnover of staff is also a
symptom (Beecroft, Kunzman, & Krozek, 2001). Lateral
violence can place
clients at risk for poor care and outcomes; for example, what
might happen to a client when one nurse deliberately withholds
pertinent
information about the client’s care that needs to be shared with
another nurse?
Nurse executives must be aware of and address lateral violence
in the workplace and implement strategies to prevent and stop
such
behavior, such as
• Educating all staff and managers about this behavior;
• Disciplining any manager or staff member who engages in this
behavior;
• Creating a culture that does not tolerate this behavior;
• Implementing organizational transparency, or having an open
culture; and
• Implementing research to study this behavior in their own
organizations.
The Health Work Environment
The health work environment is a topic that addresses nurses’
working conditions, which are linked to the quality of care that
is provided to
clients and clients’ safety. Accrediting organizations and
Magnet Recognition Program® include standards that address
working conditions
because they are associated with health and safety outcomes for
nurses and other healthcare providers (Geiger-Brown &
Lipscomb, 2010).
Employers are charged with the responsibility to assess and
address aspects of the nursing work environment that have been
linked to
hazards and adverse exposures for nurses and the most common
health and safety outcomes of nursing work. It is the
responsibility of the
nurse executive to ensure a healthy work environment while
facilitating a culture that values diversity and recognizes
individual and group
contributions to organizational goals.

How could a sunny February afternoon take such a turn for the
worse? The clock ticked in the MIT
Sloan Student Life Office (SLO), where the three Sloan Women
in Management (SWIM) club co-
presidents and two conference directors were gathered.1
SWIM’s 3rd annual conference, 11 months in
the making, was scheduled to take place the next day. A winter
storm was looming, with weather
predictions varying widely from three to 50 inches of snow in
the next 48 hours. It was almost
2:00pm and a decision had to be made – setup was slated to
begin in minutes, and speakers were
boarding their flights for Boston. Should the conference be
canceled, modified, or go forward as
planned?
SLO Associate Director Marco Esquandolas looked at the five
women seated in front of him and
asked, “Ladies, what is your final call?”
Background on SWIM
The Sloan Women in Management club was founded in the
1980s and had become one of the largest
clubs at MIT Sloan. The organization’s primary goals were to
create an inviting and supportive
community for all Sloan women; advance the careers of current
female Sloan students; work with
faculty, administrators, and the greater business community to
increase opportunities for women in
business; and, attract top female students to MIT Sloan.

In an effort to meet all of these goals and cater to the broadest
set of women, SWIM hosted a variety
of year-long programming including networking events, speaker
series, professional development
workshops, mentorship programs, and community building
events. In addition, the club held three
flagship events, one of which was an annual conference where
students could dialogue with leading
women in business. As of February 2013, SWIM had
approximately 300 members,2 550 Facebook
fans, 640 Twitter followers, 270 LinkedIn group members, and
300 individuals on the alumnae
mailing list.
The annual conference had quickly become the organization’s
largest event, despite its inception just
two years earlier. After the relatively small-scale initial
conference featuring panelists primarily from
the MIT Sloan community, the 2012 conference directors sought
to create the gold standard in
women’s business conferences. The “Innovating through
Adversity” 2012 conference brought several
renowned speakers including Fortune 500 CEO Laura Sen (BJ’s)
and VP of Google
Local/Maps/Localization Marissa Mayer to a sold-out crowd of
over 400 attendees. All who
participated hailed the conference a tremendous success. Male
and female students who attended
agreed that of the roughly 30 annual MIT Sloan conferences,
SWIM’s was the best conference of the
year.
There was strong support and interest from the administration
due to the high profile of 2012’s
conference. As SLO Director Christine Pugh noted, “The 2012
conference knocked it out of the park
– probably based on some learnings from the 2011 conference. I
heard it was just phenomenal,
enough people really raved about it that I’m very confident in
saying that.”
The 2013 Conference Leadership Team
The 2013 SWIM club co-presidents, Rachel, Lea, and Tarini,
were eager to build on the previous
year’s success and, immediately after their election to their
leadership roles in February 2012, they
began planning for the 2013 conference.3 As the conference was
the club’s largest event of the year,
Rachel, Lea, and Tarini agreed that they wanted to maintain the
previous year’s structure of heavy
involvement with the event, taking on the roles of conference
CFO, COO, and CMO, respectively.
They would next need to select two directors who would
oversee the content of the conference,
including selecting the theme and speakers, motivating a large
team to execute the event according to
their vision, and serving as the masters of ceremonies for the
event.
The 2013 conference directors, Janet and Merrill, were selected
in March 2012 by all of the co-
presidents, both past and present, and the previous year’s
conference directors. According to Janet, “I

was thinking through my next year’s goals. Something I wanted
to do was make a substantial impact
on Sloan. There were lots of ways I could do that. I decided that
one of my goals would be to work on
the SWIM conference which has a very tangible output with a
group of people I really respect.”
The theme for the 2013 conference was set as “Dare to Fail:
Taking Risks When it Matters Most.”
Soon after deciding the theme, the directors finalized a shortlist
of keynote speakers, began
brainstorming speakers for panels, and scheduled meetings with
relevant Sloan faculty and
administration.
The past presidents warned Rachel, Lea, and Tarini that they
experienced significant tension the
previous year over who had final authority in the decision-
making process. Despite this warning, the
same dynamic quickly developed with the new team. The co-
presidents wanted be included in all key
decisions, believing they needed to have a say in the selection
of speakers and topics, both for the
purposes of the conference, as well as to ensure integration with
the SWIM brand and overall
organizational strategy. The conference directors felt that they
were being micromanaged for an event
that they were directing.
As the SWIM leadership team headed off for the summer to five
different cities to pursue internships,
expectations were set that Merrill and Janet would focus on
recruiting speakers, and Rachel, Lea, and
Tarini would work on sponsorship, operations, and marketing
(including ticket sales), respectively.
The five loosely agreed to check-in calls, though specific dates
and milestones were not set.
Maintaining contact during the summer proved challenging, as
the group of five were in different
time zones and had different work schedules. Throughout the
summer Merrill and Janet continued to
chip away at speaker recruitment without much success. Despite
sending out multiple keynote
requests, they continued to hit dead-ends due to scheduling
conflicts and non-responses. On August
10 the first keynote was confirmed – Four Star General Janet
Wolfenbarger – and the team breathed a
small sigh of relief. As they all geared up to return to campus in
the fall, however, there was still
significant concern from the entire team about the lack of
progress that had been made over the
summer in finalizing speakers.
Shortly after General Wolfenbarger was confirmed, the group
faced yet another unanticipated
setback. Merrill, whose summer internship had been at a start-
up, had been offered significant equity
to leave MIT Sloan and continue in her role at the start-up. On
August 20, she sent the following
email to the co-presidents:
…I am nothing less than obsessed with this event – and with
working with Janet and each of you
– and I seriously considered returning to school JUST to direct
the conference. However, that
didn’t seem like the most logical decision, and I ultimately
didn’t feel that giving up my stake in

the company was worth the conference, regardless of how
amazing the conference is going to be.
I do, however, have a proposal… I would like to ask to continue
to co-direct the conference with
Janet. I am committed to this event’s tremendous success, am
passionate about the theme, and
will work incredibly hard to develop speakers and panels that
are worthy of SWIM. I’ve spoken
with Janet, and she is in support of this.
While it was not clear how involved Merrill would be moving
forward, the co-presidents agreed to
this arrangement. They were concerned about finding an
alternate director given the late stage in the
planning process and trusted that Janet and Merrill had
discussed managing their time appropriately.
The five-person team committed to weekly meetings to ensure
that communication lines would
remain open and progress would continue.
Conference Planning
Despite these challenges, the team maintained a positive
outlook upon returning to campus for the fall
semester, and worked hard to harness the energy of the first
year MBA students. The annual
conference was historically a crucial way to engage new
students and identify future leaders for the
organization. Committees were created to focus on speaker
recruitment (led by Merrill and Janet),
marketing (Tarini), operations (Lea), and sponsorship (Rachel).
Speaker Recruitment
Over the course of the fall semester, it became increasingly
clear that Janet and Merrill were
struggling to find speakers. The plan had been to secure at least
two keynote speakers by the end of
the summer, and then form teams of first year students to take
the lead in recruiting four to five
speakers for each of the two panels.
The panel teams were moving forward with mixed success.
Speakers were responding well to the
theme of failure, but were unable to commit to the conference
date. This had a significant effect on
the rest of the conference planning, particularly the marketing
team, which struggled to accurately
frame the conference. With Merrill on leave from MIT, a
disproportionate amount of work was
placed on Janet.
Though angry words were never exchanged, tension grew
between the conference directors and the
co-presidents. While the presidents had each other to rely on,
Janet shouldered much of the pressure
on her own, searching for a way to push forward and maintain
an energetic exterior to keep the
conference team engaged.
By the end of November 2012, the panel teams were pushing out
multiple speaker requests and
tapping all of their networks for introductions. The team
scrambled to finalize the conference agenda

before they left for winter break, as everyone would be spread
out traveling the globe until the
beginning of February. By the end of December, nearly all the
speaking engagements were
confirmed.
On January 23, 2013, with two weeks left until the conference,
original keynote General
Wolfenbarger canceled her participation due to government
budget-cuts. The team, exhausted from
the search for speakers, scrambled once again to fill the gaps in
the lineup and manage the fallout
from attendees who had bought tickets intending to hear the
General speak. The co-presidents were
convinced that there was no way to find a keynote at this late
date, but to their surprise a new keynote
speaker who was a growing force in the venture capital world
was quickly confirmed. This was just
what the team needed to sell their last few tickets.
Marketing
In 2012, an aggressive marketing campaign had led to a sold out
conference (400 tickets) nearly two
months prior to the event. For the 2013 conference, the
directors wanted to sell 500 tickets, at a
slightly higher price – $25 for students and $50 for
professionals. The main challenge for the
marketing team, however, was not the price point, but that
marketing needed to begin before speakers
had been confirmed. Falling steadily behind 2012’s numbers,
Tarini commented, “I don’t even know
why the current attendees are buying tickets. Right now, I’m
just selling air.” To compensate, the
marketing plan from the prior year was beefed up to include
social media, outreach to other schools,
and mailing lists targeting professionals. December was a busy
month for ticket sales thanks to the
team individually emailing over 100 professionals in the area,
including every female professor
associated with MIT Sloan, launching an aggressive social
media campaign, and revamping the
promotional material each time a new speaker was added to the
roster. However, ticket sales still
lagged behind the previous year, and Tarini checked daily as the
fear of a half-empty event continued
to grow. The weekend before the conference, the team
confirmed that the event was sold out.
Operations
The conference event space, the premier location on campus and
a highly sought after venue, had
been booked in June 2012. Lea formed a team to help with
operations including vendor management
(catering, facilities, custodial services, security, furniture
rental, audio-visual needs, lighting,
videography), day-of logistics management, and preparation for
the VIP dinner, traditionally held the
night before the conference to welcome speakers and thank
corporate sponsors for their support.
Students who had been active participants in conference
planning were also invited to attend. To cut
costs and improve logistics, the team opted for a new dinner
venue close to campus, which meant
building new relationships with vendors and planning the
budget, menu, and event space from the
ground up. A non-refundable deposit had been put down to hold
the space, and the final payment for
the dinner, estimated to be $3,000, would be due upon
completion of the VIP dinner.

During the planning period, the leadership team identified a
major glitch – a classroom in the same
building as the event venue, traditionally used as an overflow
space for conference attendees, was
booked for a class during the conference. Preparations had to be
adjusted accordingly to
accommodate the reduced seating and inflow of non-conference
attendees that would be coming
through the building entrance.
By the end of January, all of the work orders and purchase
orders had been set up, though no formal
contracts had been signed, and vendors were scheduled to begin
arriving at 2:00pm on Thursday,
February 7 to set up (Exhibit 2). The MIT Sloan administration
had agreed to host an alumnae
breakfast at the conference. In-kind donations had been
solicited from coffee and pastry shops in the
area, who would be delivering items on the day of the
conference. However, on January 24, the MIT
administration notified Lea that they could no longer sponsor
the alumnae breakfast as originally
promised due to budget constraints. With just two weeks until
the conference, the team scrambled yet
again to adjust.
Sponsorship
The sponsorship team, led by Rachel, began planning for the
conference in late June 2012. Initially a
small group worked on drafting promotional materials that
would be shared with potential sponsor
organizations and would provide a summary of the 2012
conference, the vision for 2013, and
information about the benefits of sponsorship. With this
information in hand, the team began reaching
out to the prior year’s conference sponsors to gauge their
interest in sponsoring once again. The team
had a fundraising goal of $40,000. This amount, in addition to
the revenue from ticket sales, would
enable them to cover all conference costs and have a buffer,
which could be applied to the following
year’s conference should the funds not be required. While many
of the previous year’s supporters
were interested in partnering with SWIM again, many
companies did not have the same budget
resources available to them, and as such the sponsorship team
was hard pressed to raise enough
money to fund the conference. Rachel turned to the first year
student committee to help brainstorm a
list of new sponsor organizations and reach out to their network
of contacts. Despite getting off to a
slow start, a driven first year student stepped up taking on
virtually all fundraising tasks and, in late
January, signed the final sponsor, bringing the fundraising total
to $38,000, enough to fund the 2013
conference.
Miraculously the week before the conference, speaker
recruitment, marketing, operations, and
sponsorship were all coming to a successful close.
Final Preparations
Monday, Feb. 4
Five days to go. On Monday, February 4, students arrived back
on campus after their January break,
ready for a new semester. The energy level was at an all -time
high as the full conference team, 20

members strong, assembled to discuss final details on the event
that was now 11 months in the
making.
Much like a dress rehearsal, the operations team walked through
all events for the week, hour-by-
hour, to finalize last minute details. The marketing team
confirmed that the event was officially sold
out with 500 tickets disbursed (Exhibit 3), and the sponsorship
team officially closed out their
sponsorship efforts with $38,000 raised, confirmi ng that the
conference would easily break even on
costs (Exhibit 4). All 15 speakers had recently been confirmed
and information on their travel
logistics was being collected. Outside of the five-member
leadership team, most students had been
focused on their own assignments until now, and this was the
first time they were able to see the
breadth of work that had been completed to date. At the
meeting, final decisions were collectively
made regarding social media strategy for the day of the
conference, speaker biographies were
approved for the programs, and signs for the event space were
designed and sent to the printers.
Tuesday, Feb. 5
On Tuesday, between and after classes, the co-presidents
focused on last minute details for the
conference including printing nametags, determining the room-
flow for the event space,
communicating with the photographer, assigning handlers for
VIP attendees, and setting up
interviews between speakers and members of the media.
Volunteer assignments, that took into
consideration both the schedules and personal interests of 50
student volunteers, were confirmed.
Late that afternoon, murmurs began that a snowstorm was
approaching, but in New England,
predictions of inclement weather were an ordinary occurrence.
Weather reports were vague, varied,
and contradictory across networks (Exhibit 5). The conference
team continued operating according to
the original schedule, knowing that since 1997 MIT had closed
only twice due to snow. As Janet
remarked, “Boston is Boston. There is really no way of knowing
so let’s just keep going. It’s just go
go go. There is a lot to be done.”
However, as night approached, weather predictions became
more distressing.

began to indicate the possibility that a major storm would hit
the Boston area some time on Friday.
Starting at 8:00am, the team began operating on two streams:
(1) continuing to prepare for Friday’s
conference with the finalized set of speakers, and (2)
identifying alternate local speakers should
flights be canceled.
At 9:00am, an email was exchanged among the conference
leadership team:

Weather looks like it’ll be ok in the AM on Friday but get
steadily worse for the evening.
Hopefully this means speakers will be able to arrive and leave
without issue. Here is a status
report on travel plans:
Traveling in:
• Alison Gerlach: Arriving Thursday night
• Julia Hu: Flying in from San Francisco Thursday afternoon
• Julie Sygiel: Train in from NYC Friday morning
• Sandie O’Connor: Switched flight to Thursday night due to
weather concerns
• Maura O’Neill: Flight from DC Thursday evening
In case of contingency for above speakers:
• Christina Chase can move to Panel #2 (I’ll ask her today)
• 2 of 3 speakers on Panel #1 are local, even if Julia can’t make
it
• All Keynotes are flying in…this is a tough one. I know an
MIT professor who could
possibly do a talk last minute (I’ll ask today)
Thirty-six hours before the event, the conference team sent a
message to all attendees notifying them
that the conference would continue as scheduled and reminding
them of key logistics (Exhibit 6).
Since Monday, the team had been receiving emails from people
on the waitlist for tickets, asking if
there was anything they could do to get off the waitlist. Worried
that people might be scared off by a
potential storm, the marketing team began releasing tickets from
the waitlist although the event was at
full capacity. The remainder of the day unfolded rapidly:
• 4:47pm: One of the corporate sponsors of the conference and a
core sponsor of SWIM
emailed to say their three representatives, who were coming
from upstate New York, would
not be attending due to the weather forecasts. There was no
mention of any implications for
sponsorship funds.
• 7:14pm: New weather reports indicated a stronger storm than
earlier anticipated and in
response, Christine Pugh, SLO director, contacted the
leadership team requesting a meeting
for the next day to discuss contingencies should MIT close (a
historically rare occurrence).
• 8:35pm: Attendees began emailing the conference team to ask
about postponement and
cancellation options, and when they would get their money
back. These emails trickled in
throughout the night and into the next day (Exhibit 7). They
were not sent any response.
Thursday, Feb. 7
T-minus 24 hours. Thursday morning began early, though none
of the presidents or conference
directors had been getting a full night’s sleep that week. The
team pushed forward, operating under
the assumption that either the conference would continue as
planned, or face speaker cancelations.

Before, between and during classes, texts and emails were fired
off rapidly between the conference
directors, presidents, and within each conference committee. At
7:33am, Tarini wrote, “If I keep
getting so many emails from ticket attendees asking if the
conference is cancelled, I will have a legit
breakdown. I’ve gotten nearly 100 emails this morning. Let me
know what they say at the 9am
meeting.” Rachel responded with, “According to my latest
weather stalking this thing will go on.
Snow won’t get bad until afternoon.”
At 9:00am, Lea, Rachel, and Merrill attended a meeting at the
SLO, at which time multiple
contingency scenarios were discussed. The following email was
sent to the entire leadership team to
summarize the meeting and next steps:
We talked about a couple of different scenarios with Marco
Esquandolas, Christine Pugh, and
Sharon Knope this morning:
A. Move forward as planned – expect some speakers not to
make it, and low attendance.
Potentially hold a half-day conference.
B. Cancel the conference this afternoon. Marco Esquandolas is
doing some recon for us
today on whether MIT is likely to close, but there is no
indication that this will happen
yet.
C. Wait to receive notification of MIT closing tomorrow (likely
late tonight or early
tomorrow morning; could be either full day or half day closure).
*TO DO*
1) Now – Janet/Merrill reach out to speakers now to get latest
news on travel plans
2) Now – Merrill check in with MIT Sloan Dean to update him
on contingency plans
3) 12:30PM – Lea check in with Marco for update on MIT
closure status
4) Until a decision is made, Tarini should not respond to any
emails from attendees
As of 11:00am, the speakers team had identified local speakers,
should confirmed speakers face travel
delays or cancelations. At 11:30am, conference discussions
were temporarily put on hold while
Rachel and Tarini attended a SWIM event on personal finance
and career decisions for women,
unrelated to the conference. It was a longstanding goal that the
conference not detract from SWIM’s
general programing at any point during the year. As a rule, at
least one, if not all three, of the club’s
presidents attended any of the events the club held. Lea was not
present, as she was initially planning
to print the 500 nametags needed for the next morning. As a last
minute decision, the team decided to
hold off on this task, and she instead checked in with Marco to
see if he had received any more
information regarding the likelihood of an MIT closure.
While waiting for Marco to return to his office, Lea emailed the
operations committee and the
conference leadership team at 11:53am with an updated
operations schedule and the following

message suggesting she had received some good news: “For
now, please notify all vendors that we
are moving forward as planned - I will let you know
immediately if anything changes!” However, at
12:36pm, Lea sent a short, cryptic, and concerning text message
to the co-presidents: “Just spoke with
Marco - Can you come now?”
Decision Time
During the many months of planning, each member of the
leadership team had visualized dozens of
possible scenarios for the conference. In some, there were no
attendees, the food never arrived, or the
speakers forgot their talking points. In others, the room was
packed and the crowd was on their feet
giving thunderous applause for a life- and career-changing
event. After so many setbacks, it was
finally all within reach, and it was going to be glorious. Except
now, there was a new and
unprecedented scenario to consider – canceling the conference
for a potentially monstrous
snowstorm, when there was not a cloud in the sky or a
snowflake on the ground.
At 1:30pm on Thursday, February 7, SWIM’s three co-
presidents and two conference directors, who
had been in varying states of misalignment and frustration for
so long, met with Marco in his office to
sort through their options. Weather reports had been monitored
from different networks. Now, there
was every indication that a storm would hit Boston at some
point, but most networks expected the
storm to hit after 12:00pm on Friday, while others said it could
be as late as Friday night. It was
simply too early to tell.
Janet and Merrill collected a status update of where each
speaker was, and when planes would begin
taking off toward Boston. They confirmed that all 15 speakers
were unfazed by weather reports and
were ready to attend the conference the next day, regardless of
the potential snowstorm. One had
already changed her ticket to fly in earlier to avoid any airport
closings, and a second could not be
reached as she was already on a flight from London.
At precisely 2:00pm, vendors would begin setting up the venue
and SWIM would be liable for the
costs. Further, if MIT closed, the rental fees would triple from a
one-day rental to daily rentals
through the weekend, when the rentals could be picked up. Lea
did a back of the envelope calculation
and quickly saw that if the conference was canceled after
vendors arrived, the costs could increase
significantly. The catering company had already warned her that
it was too late to cancel the
perishable food items that had been prepared for the conference
– it was unclear what percentage of
the $13,000 catering bill would be recoverable. Lea knew what
her vote was, but she was curious to
see which way her two co-presidents were leaning.
The MIT administration had not yet closed the Institute, and
would not comment on if they would.
MIT rarely closed its doors, but if it did no events would be
allowed on campus, including the
conference.

What would happen if the conference was not canceled, and
MIT closed later that night? What if the
conference was canceled, MIT did not close, and the storm only
hit Friday night? What would be the
fallout with attendees and corporate sponsors? Rachel was
particularly concerned about the
ramifications for the sponsors, whom her team had worked so
hard to secure. Would SWIM’s
reputation be impacted? Would they be able to secure
sponsorship funds in the future? Tarini
wondered if attendees would show up for the conference, even
if it was not canceled, and if they did,
was there a risk of them being snowed in? If the team opted to
cancel the conference, would they be
forced to give back ticket revenue, and, if so, how would that
affect the bottom line? Could they
really give up on the event that they had sacrificed for and had
devoted the last year of their lives to?
The entire conference revolved around the theme of coming to
the brink of failure and still finding a
way to succeed. With graduation around the corner, there would
be no second chances.
Given that most weather forecasts suggested that snowfall
would only begin at 12:00pm Friday, and
would not escalate until early evening, Merrill began
championing the idea of a half-day conference.
Could they run the events concurrently instead of sequentially,
giving attendees the option to choose
which speaker to hear from? Since the event space would be set
up for one speaker at a time, would
it even be feasible? Would attendees show up for a conference
that only lasted three hours, and if not,
would low attendance be disrespectful to the high profile
speakers?
The conference directors and co-presidents narrowed it down to
three choices:
1. Cancel the conference before 2:00pm, thus saving operational
costs that would otherwise be
incurred.
2. Hold a half-day conference with concurrent speakers.
3. Wait until Thursday night to make a decision, based on more
accurate forecasts and updates
on speaker travel plans.
While initially on the table, rescheduling was not an option for
the team. With only three months left
before the conference leadership team graduated it would be
impossible to find a date that worked for
all, or even most, of the speakers. Additionally, because the
venue had no availability in March,
April, or May, the team would need to find a new location for
the conference. They would in effect be
planning an entirely new conference in one-quarter of the time
it took to plan the initial conference.
For these reasons they knew rescheduling would not work.
In his facilitator role, Marco Esquandolas remained an impartial
observer, telling them,
The way I look at clubs on campus is that it is your chance to
practice management. You put this
all together. You know best if you should cancel or go forward.
In all my years here, this situation
is unprecedented. If it is canceled, it will be devastating. If
there is a storm, it will be devastating.
If there is no storm, it will be devastating. I will support
whatever decision you make.

The women discussed their options at a rapid pace, knowing the
minutes were winding down. How
would speakers react if the conference had to be canceled after
they had already flown in? Would
companies expect sponsorship money to be returned? What was
best for the safety of attendees? How
much of the setup cost would be sunk if they postponed the
decision? If first year students never got
to see a conference executed, what would a cancelation mean
for the legacy of SWIM? What alternate
venues could accommodate the conference if MIT closed?
Would it even snow?
The only thing they were sure of was that whatever decision
they made would have a long-lasting
impact on the event and organization that they were so
dedicated to.

home tomorrow. I am pregnant and not

HEALTHCARE ENVIRONMENTHigh-quality healthcare organization

  • 1. HEALTHCARE ENVIRONMENT High-quality healthcare organizations are driven by attention to their customers, excellent workmanship, empowered workforces, and innovation and change as a response to their environment. The Patient Protection and Affordable Care Act of 2010 established the mandate of increasing access to care while decreasing costs. Ongoing healthcare reform emphasizes the importance of collaborative teams in the redesign of healthcare delivery systems that provide high- quality, client-centered, mutually accountable care across the lifespan (Roussel, Thomas, & Harris, 2016). Consequently, healthcare providers are challenged to focus on quality, cost-effectiveness, and responsiveness to the voice of the consumer in all aspects of care. The overall goal of transforming the healthcare system is to develop a seamless, coordinated care approach for clients by: 1) improving population health and client outcomes, 2) enhancing the client experience, and 3) continued improvements in providing high- quality services at a lower cost (Stiefel & Nolan, 2012). Traditional measures of these outcomes are now being combined into value-based competitive reimbursement models from third-party payors and regulatory agencies. It is generally accepted that in order to survive financially and competitively, healthcare organizations must break down traditional organizational hierarchies and embrace a culture of
  • 2. collaboration and teamwork among providers and stakeholders (including consumers) at all levels. High-quality healthcare organizations recognize the environmental factors inherent in maintaining competitive advantage and ensuring financial viability include an examination of organizational culture with an emphasis on team-building, group dynamics, conflict resolution, negotiation, reframing historical sources of power, environmental issues, cultural competence, and diversity. ORGANIZATIONAL CULTURE AND ENVIRONMENT It is impossible to promote high-performing teams without true commitment to teamwork at all levels. Shared vision, open dialogue, trust, and a sense of belonging are critical elements for overcoming organizational challenges and ongoing strategic development. Consequently, it is important to insure that the culture, alignment of corporate objectives, and educational/technical supports are congruent with establishing a culture of teamwork and collaboration. Longstanding paradigms of organizational hierarchies, privileged conversations, and discipline- specific silos must be replaced by an organizational climate that fosters equalization of power, shared governance, participative management, transparency, open communication, and individual/group contributions to organizational goals. Synergy of vision and desired outcome is essential. Team-related activities and organizational objectives must also be aligned. Stakeholders must understand how their contributions align with corporate objectives and feel that their efforts are valued. It is increasingly recognized that alignment of the culture
  • 3. and capabilities of an organization are derived from philosophies and practices related to the strategic management of human capital (including the free flow of information, job security, selective hiring practices, elimination of status differences between groups, decentralization of authority, and team-building) as basic elements of organizational design (Pfeffer & Veiga, 1999). TEAMBUILDING “A team remains the most flexible and most powerful unit of performance, learning, and change in any organization” (Katzenbach & Smith, 2003, p. XIX). The abilities to express one’s ideas clearly and decisively, to listen attentively and respectfully, and to invite a range of opinions are among the communication skills that help managers build team cohesiveness. Teams usually are developed reflecting the diversity and culture of the organization, and although the chief executive may be fundamental in the formation of high- performance teams, he or she is not always a team leader. According to Katzenbach and Smith (2003), developing a team is the inclusion of “a small number of people with complementary skills who are committed to a common purpose, performance goals, and approach for which they hold themselves mutually accountable” (p. 275). A team develops around the task to be accomplished while taking into account the mix of personalities and competencies; the desired performance outcomes; and the processes of communication, involvement, performance orientation, and enabling leadership (Higgs, 2006). Central to a team’s success will be its members’ work behaviors
  • 4. such as constructive listening and giving the benefit of the doubt to others (Katzenbach & Smith, 2003). Team members need to not only be mature and self-motivated, but also be aware of their social and cultural differences, appreciate each other’s cultural diversity, and have a basic understanding for each other’s value systems. Only through an active display of this understanding and open and nonjudgmental communication can a team develop its highest potential to meet group goals and complete its mission. The team leader can facilitate this process by using good meeting skills and appropriate meeting behaviors. The task of reminding team members of their mission, goal, and tasks falls to the leader as well. GROUP DYNAMICS Teams tend to evolve through a predictable development process that was first described by Tuckman in 1965 and then modified by Tuckman and Jensen in 1977 (Smith, 2005). The five stages include: Forming, Norming, Storming, Performing and Adjourning. In the Forming stage individuals come together and begin to establish team relationships. While there is often a sense of enthusiasm for the task at hand, individuals tend to be cautious in their communication with one another; as they may be relative strangers and may need purposeful teambuilding activities. A skilled team leader or facilitator may be needed to clarify team purpose, goals, and to assist the team in establishing rules of engagement. As the group proceeds through the maturation process, it arrives
  • 5. at the second stage, known as Storming. In this stage, members of the group compete for position, power, and status; informal leaders may emerge. When team members come from differing backgrounds there is a tendency to approach critical issues from differing standpoints and opinions that cause uneasiness and conflict among group members. These differences need to be openly confronted and addressed proactively so that effective resolution can be achieved occur in a timely fashion (Kelly, 2008). The storming phase of group work is critical to team success. Katzenbach and Smith (2003) assert that real teams don’t emerge unless individuals take risks, address conflict, and develop a sense of trust and interdependence. The third stage of group formation, Norming, is where the rules for working collaboratively are made explicit; structure, roles, and relationships are further clarified resulting in a sense of cohesion. Team members become more adept at resolving conflict, respect differences in opinion and work cooperatively to achieve team goals. The leader’s role during this stage of group development focuses on relationship building and assisting the group in overcoming barriers. As the group matures, it enters the Performing stage. In this phase, group cohesion, collaboration, and solidarity are evident. Personal biases are overshadowed in an effort to achieve team goals (Kelly, 2008). The leader’s role is to provide feedback on the work that the group is
  • 6. accomplishing, redirect group energy when necessary, and further cultivate interpersonal relationships. The final stage of team development, Adjourning, addresses the individual and group tasks associated with completion of the team’s specific charge and dissolution of the formal team structure. During this phase of termination and consolidation, the ream reviews their activities and evaluates the outcome of their efforts (Kelly, 2008). The development of strong interpersonal relationships inherent in team-building and the loss of team structure may be difficult for some team members. Consequently, a focus on closure is paramount. At this stage it is appropriate to recognize team accomplishments and to facilitate an ongoing culture of collaboration and teamwork (Roussel, Thomas, & Harris, 2016). Celebration of both successes and failures are essential for building a culture of inclusion and cooperation by: valuing the process and the contributions of individuals involved, supporting an ongoing, proactive culture of change, celebrating insights gained and providing a sense of renewal that will serve as the cornerstone for ongoing collaboration and teamwork necessary to meet the inherent challenges of redesigning healthcare delivery now and in the future. Team development and function rarely proceed in a completely linear fashion. Changing circumstances and intervening variables may have a profound impact on team function, alignment, and outcomes. Consequently, management intervention may be needed when teams regress or encounter seemingly insurmountable barriers to performance. In these instances the leader should consider strategies to assist teams in
  • 7. overcoming obstacles, evaluate whether modifications to team membership are needed, and in extreme cases, determine whether planned abandonment of the team activity is needed. Experts have also suggested that the group formation process, and thus the productivity of the group, can be accelerated under the guidance of a skillful facilitator or in the face of actual or fabricated crisis (McKnight, Kaney, & Brewer, 2010). CONFLICT AND CONFLICT RESOLUTION Conflict occurs naturally in and among groups and individuals; it is inevitable and a condition essential to change. Although conflict is often is viewed negatively, if managed constructively it can provide a sound foundation for the evolution of high-performing teams as members bring their unique insights, perspectives, and values to develop a shared vision and plan of action. New paradigms for constructive conflict resolution include replacement of old attitudes of competition with new approaches that foster collaborative problem-solving, shared accountability, respect, open and honest communication, a willingness to let go of past paradigms in favor of exploring new possibilities; and developing a mutually agreed-upon plan of action (Katzenbach & Smith, 2003). Conflict may be intrapersonal (within oneself), interpersonal (between the self and another person), intragroup (among members of a particular group), or intergroup (among members of two or more groups). Other types of conflict include competitive conflict and disruptive or destructive conflict. In both instances, the desired outcome is to
  • 8. overcome one’s opponent (i.e., to “win”). Conflict management occupies a significant portion of a leader’s work. Some suggest that at least one-quarter of the leader’s time is spent in conflict management activities. The challenge for the leader is, of course, to help her or his subordinates reach a “win–win” outcome in which the parties to the conflict each believe they have come away from the encounter with a sense of resolution. Strategies include • Focusing on goals, not personalities; • Meeting the needs of both parties, equally if at all possible; and • Building consensus. Achieving a win–win outcome is much easier in the abstract than in the workplace setting. Nonetheless, it is a worthwhile goal. Conflict is a state of disharmony or disequilibrium. Managing conflict requires maintaining the level of conflict neither too high nor too low, with the ultimate goal being to stimulate growth and coping behavior without reaching the point where the conflict seems overwhelming. Common conflict resolution strategies include compromise, competing, cooperating, smoothing, avoiding, and collaborating. The choice of the most appropriate strategy depends upon many variables such as the urgency of the decision, inherent power and status disparities, and the maturity of individuals involved in the conflict (Marquise & Huston, 2009). NEGOTIATION Negotiation can be thought of as a formal process; one example is the negotiations that take place at the time of contract
  • 9. deliberations between unions and management. Negotiation also can be thought of as a political process in that it is a “power play” among individuals who compete to “win” but generally compromise in the end. The keys to successful negotiations include the principles of • Separating the people from the problem, • Focusing on interests, not positions, • Inventing options for mutual gains, and • Insisting on objective criteria. Interest-based negotiation is a somewhat newer concept, the principles of which were outlined by Fisher, Ury, and Patton (1991) in Getting to Yes: Negotiating Agreement Without Giving In. Bizony (1999) has distilled the principles listed below from Fisher and colleagues’ work on the Harvard Negotiation Project; their value has been convincingly promulgated through the years. • Treat people as equals. • Resolve issues on their merits. • Define issues with a definition that is acceptable to all parties. • Focus on interests, not on conclusions or positions. • Develop options that may meet the interests of both parties. • Apply objective standards to resolve conflicting interests. Twelve points of negotiation can prepare leaders for successful negotiations. 1. The greatest failure in negotiation is failing to negotiate. 2. The most important person to know in negotiation is yourself. 3. Everyone has power in negotiation.
  • 10. 4. Single-issue bargaining leaves both parties unsatisfied. 5. Urgency drives decisions. 6. Agreement is the end; trade-offs are the means. 7. The best results are obtained by keeping the other party on a need-to-know basis. 8. The value of something is always in the eye of the beholder. 9. Success in negotiation is directly related to the amount and kind of preparation preceding it. 10. Being able to walk away or select an alternative to a negotiated agreement puts a negotiator in a very strong position. 11. Two sides can always agree on something, even when they are far apart on major issues. 12. Conflict is a part of meaningful negotiation. Mediation as a method of dispute resolution uses a neutral third party who attempts to bring parties together to solve a conflict. The mediator’s role is to fact-find, make suggestions, and draft a solution that is agreeable to both parties (Epstein, 2003). SOURCES OF POWER AND EMPOWERMENT Power is the capacity to act and the energy to mobilize resources to create change and achieve goals (Kelly, 2008). Whether power has a positive or a negative connotation depends on how it is used. The most widely accepted power-base classification identifies five sources of power inherent in interpersonal interactions. The categories include: legitimate power, reward power, coercive power, referent power, and expert power (Huber, 2010). • Legitimate power is the right to command within an organizational structure or setting based upon the position held. The reciprocal nature of this power obligates employees to comply with
  • 11. legitimate orders. • Reward power is based on the ability to deliver desired rewards or the ability to compensate others in some way. Compensation need not be monetary. • Coercive power is derived from the ability to threaten punishment or deliver penalties. It is a source of power used to apply pressure so that others will meet what is demanded. • Referent power means that the individual has characteristics (charisma) that appeal to others resulting in the ability to influence without having to offer rewards or threaten punishment. • Expert power means that the individual has specialized expertise or knowledge that can influence the actions of others toward certain outcomes or goals. All sources of power are potentially important, and the assumption is that the most powerful leaders are those who have high legitimate, reward, or coercive power. However, one should not underestimate the strength of referent and expertise power. These sources of power are more closely related to personal motivation and may, in the long run, make a greater and more lasting difference within a company or organization. It is not necessarily the leaders whose names are well known or who sit atop the organizational chart who exert the most power of this type. In today’s healthcare environment it is important that all staff
  • 12. are empowered to make decisions, especially those decisions that positively influence client outcomes. An understanding of the concepts of power and empowerment is also essential if nurses are to realize their full potential for leadership and ongoing professional advancement. ADAPTING TO CHANGE Change is the province of leaders. It is the work of inspiring people to do things differently, to struggle against uncertain odds, and to persevere toward a misty image of a better future (Kouzses & Posner, 2012). Consequently it is imperative that nurse executives understand and use strategies that will support staff members in proactively adapting to change. Appreciative Inquiry Appreciative inquiry is the process in which an organization asks questions of its members in seeking information that can be used to anticipate and identify areas of potential strengths. The four characteristics of appreciative inquiry include • Appreciative, • Applicable, • Provocative, and • Collaborative. Appreciative inquiry is best described through the 4 Ds: 1. Discovering the best of what is, 2. Dreaming what might be, 3. Designing what should be, and 4. Creating a destiny of what will be. Appreciative inquiry is to organizations what creative
  • 13. visualization is to individuals; both are positive approaches to foster change. Appreciative inquiry is considered to be a “soft business strategy” that can be used to create organizational visions, build cultures, and align groups to achieve organizational goals. Through imagination and thoughtful analysis, appreciative inquiry can contribute to measurable results for organizations. Crisis Management A crisis is anything that has the potential to significantly affect an organization. Organizations with crisis management plans are better able to work effectively with local responders, promptly attend to the needs of those affected, assist investigating bodies without jeopardizing the organization’s legal position, provide for accurate and timely information, and minimize damage to the organization’s reputation. The four objectives of crisis management are 1. Reducing tension during the incident; 2. Demonstrating organizational commitment and expertise; 3. Controlling the flow and accuracy of information; and 4. Managing resources effectively. Managing a crisis begins with the creation of a crisis management team and assessing potential crises before they occur. Managing a crisis also includes • Developing crisis management team plans;
  • 14. • Establishing guidelines for gathering information and beginning an internal investigation; • Providing periodic crisis training evaluation; and • Developing guidelines for crisis communication. The key points for crisis management of any dimension include • Having a flexible structure capable of responding to any cr isis quickly, decisively, and in a coordinated manner; • Preparing operational contingency plans; • Creating and communicating a document retention policy (e.g., nothing is thrown away that might enhance an investigation or document the event); • Developing training that includes addressing legal issues before they occur, developing investigational procedures, identifying necessary equipment and systems before a crisis, and developing good media relations skills; • Preparing to communicate with a variety of constituents, including employees, the media, neighbors, investors, regulators, and lawmakers; and • Preparing a business contingency plan to minimize disruption and damage. Because crisis management is the manner in which organizations respond to unexpected events over which they have little or no control, and “uncontrollable” situations occur with relative frequency in health care, developing purposeful intervention strategies seems prudent. The “truthful disclosure” approach is one that healthcare organizations take in order to maintain public trust.
  • 15. A classic example of a crisis management in healthcare products can be seen in the Johnson & Johnson’s handling of the Tylenol® tampering crisis (Kaplan, 1998). In 1982, seven people in the Chicago area died after taking Extra-Strength Tylenol capsules that had been injected with cyanide. The containers had been tampered with after they left the manufacturing plant. Rather than claim that the company was not to blame, Johnson & Johnson immediately launched a public relations program to preserve the integrity of the product and the company. Marketing experts believed that Tylenol would disappear from pharmacy shelves, never to be mentioned again except in negative terms. However, Johnson & Johnson’s leaders put public safety first and worried about financial impact later. They alerted consumers throughout the nation to avoid the consumption of any Tylenol product until the extent of the tampering could be determined. They stopped production of all Tylenol products and recalled all Tylenol capsules from the market at a cost of more than $100 million. They offered to replace any Tylenol capsules people had already purchased with Tylenol tablets. They quickly began working with the Chicago Police Department, the Federal Bureau of Investigation, and the Federal Drug Administration. They put up $100,000 in reward money to help catch the perpetrator of the crime. Not only did the company survive and thrive, Tylenol remains one of its biggest-selling and most profitable products. The forthright approach of the company reassured the community that its safety came
  • 16. first and that Johnson & Johnson cared enough to be publicly open and truthful about the crisis (Kaplan, 1998). Crisis management by companies such as Qantas Airlines, Exxon-Mobil, and Boeing demonstrates the effectiveness of the four objectives of crisis management in responding to the crisis in an organized format, protecting an organization’s reputation, communicating effectively through company channels to resolve the crisis, and normalizing operations following catastrophic events. ENVIRONMENTAL FACTORS Cultural Competence Cultural competence can be defined as the ability to interact effectively with people of different cultures with a focus on personal awareness of one’s own culture and attitude toward culture, and development of knowledge and skill across multiple cultures. Cultural competence is essential as a leadership strategy in today’s healthcare environment. Our “shrinking” planet, globalization, and population movement bring people from different cultures throughout the world together in the workplace, and people may have widely divergent understandings of identity and society. Globalization has increased the need for awareness of cultural ideas and expectations other than our own in an environment where intermixing national, religious, and ethnic identities may lead to conflict. The world’s population is expected to double by 2050 (Kotlikoff & Burns, 2004). Industrialized nations are “graying,” while 80% of the world’s population growth is in developing nations. Challenges for the workplace include language and cultural differences, increasing
  • 17. incidence of chronic illness, generational issues, maximization of resource use, and ethical differences. Strategies for success in cultural competence include • Know your own culture, values, and biases. • Listen and observe. • Emphasize corporate values at all times. • Be a teacher and a learner. • Hold up your end of the bargain by displaying awareness of personal cultural practices. • Give clear directions and provide resources. • Delegate the responsibility for outcomes. • Give the big picture. • Consider the rules and procedures from all perspectives. A leader’s responsibility in cultural competence includes • Managing personal expectations, • Providing straightforward steps for decision-making, • Being courageous and displaying correct behavior, • Applying leadership and management skills according to values and attitudes, and • Providing employees the opportunity to grow. Because today’s workforce consists of four generations (i.e., mature, baby boomers, generation x, millennials), for the first time, a leader also must consider generational diversity as well as cultural diversity. There are challenges relating to work ethics, duty, and sacrifice for the job. A leader’s role in cultural and generational competence in today’s healthcare organization includes
  • 18. • Creating a culture that has a diversity-sensitive orientation; • Building the blocks of culturally and linguistically competent healthcare delivery; • Recruiting and retaining a diverse workforce; and • Ensuring the success of a culturally competent organization. Effective leaders are aware of and make use of cultural competence to maximize the benefits of diversity in their staff and client population and minimize the costs in delivering healthcare services today. In nursing practice, nurse leaders should encourage members of other cultures to become nurses, discuss the benefits of nursing, and support efforts of nurses from other cultures to assimilate into nursing’s culture. The time to consider cultural and generational diversity is now. The nurse leader must consider the change in the population demographics, that industrialized nations are “graying” and the workforce is too. A survey of staff demographics can help the nurse leader prepare for an increase in chronic illness among the workforce as well as generational workforce issues and language and cultural challenges that might disrupt optimal effectiveness of operations (Kotlikoff & Burns, 2004). Leveraging Diversity Leveraging diversity is the ability of an organization to become culturally competent and, in doing so, value diversity from both a personal (the organization) and business (the customer base) perspective (Shipp & Davidson, 2001). This business strategy links the workforce, the workplace structure, and the marketplace and displays the
  • 19. ability of organizations and their leaders to recognize and use every advantage to ensure success. Leveraging diversity maximizes all talents and intellectual capital within an organization and ensures that all persons are included in every aspect of the organization. Leveraging diversity is known to boost employee morale, reduce grievances, and enhance problem-solving and decision- making abilities. This approach to management of human capital in an organization reduces barriers, enabling all employees to fully use their talents on behalf of the organization. Leveraging diversity in the marketplace includes serving customer groups with a sensitivity to their cultures and allows an organization to maintain a multicultural perspective. Cultural diversity can be leveraged for bottom-line impact and therefore ties directly to business strategy and practices. There are three leadership behavioral elements of leveraging diversity. 1. The cultural element includes exerting influence within the organizational culture to set the tone for underlying values, beliefs, and principles. 2. The leadership element includes giving employees a sense of direction, meaning, and purpose to navigate organizational waters as well as recognizing and using the full potential of all persons in the organization. 3. The connectivity element includes building a bridge between cultural and leadership elements and organizational intellectual
  • 20. capital, maintaining vision clarity, sharing power, and identifying and assessing problems. The end result of leadership support and participation in leveraging diversity for an organization is an increase in productivity, efficiency, and quality, and ultimately an improved work environment. The nurse executive is a role model in setting the tone and direction of an organization’s daily work effort. The nurse executive acts as a coach and supporter of diversity among all stakeholder groups to achieve the end goals and objectives of the organization and its constituencies. Personnel Matters While Leveraging Diversity in the Workforce Leveraging diversity in the workforce overlaps with human capital and human resources management functions. Areas to consider include writing and using objective documents, including job descriptions and performance appraisals. Leveraging diversity in the workplace should be a consideration during personnel selection efforts and orientation and training of new employees. Addressing behavioral issues and guiding employees to organizational support mechanisms such as employee assistance for alcohol, drug, or other issues are also times when leveraging diversity can increase an organization’s productivity, efficiency, and quality, and ultimately lead to an improved work environment. Providing the assistance or the mechanism for employee assistance to all employees promotes increased productivity and leverages diversity. Vulnerability audits and exit interviews offer opportunities to
  • 21. gather data about the work environment and consider whether this should be the time of change for job enrichment, engineering, rotation, or enlargement opportunities. Enhancing the work environment through job enrichment makes jobs more interesting and challenging, maximizes efficiency, and adds more variety to daily tasks. Goal-setting with employees provides objectives to structure the job while considering that job rotation adds variety (Slocum, 1981). Organizational Transparency Current management literature defines organizational transparency intentionally shared information exchanged between two parties. It is distinguished by the degree of information disclosed, clarity and accuracy (Schnackenberg &Tomlinson, 2014). A transparent organization encourages behaviors that support access to information, participation, and decision-making. These behaviors create a higher level of trust among all stakeholders. Organizational transparency implies a trusting environment wherein transparency and trust are interconnected. Cultural trust in the organization requires clarity and consensus about what constitutes success, open access to information, and confidence in the competence of all involved. Leaders should consider creating an environment that fits the definition of success within the organization and for the organization. Other important leadership behaviors include
  • 22. • Practicing nonmanipulative leadership, • Communicating frequently and repeating important information, • Opening up access to documents regarding decisions, • Sharing background information about important decisions, • Providing clear financial reporting, and • Hiring and appointing trustworthy people. Challenges to organizational transparency include • Risks from potential distortion of the truth through increased access to information, • A slower-than-usual decision-making process, and • Additional organizational vulnerabilities. Organizational transparency does not guarantee that the right decisions are being made and may require additional time and resources at all organizational levels. Transparency may at some point have diminishing returns, when communication, information-sharing, and trust levels reach a status quo and information flows in all directions. In the final analysis, the value of organizational transparency lies in the protection and promotion of an organization’s reputation, a corporate culture of communication among all stakeholders, and the involvement of current and future members in decision-making processes (Fung, Graham, & Weil, 2007). Transforming healthcare delivery requires robust data collection, and analysis of a broad range of performance measures. Transparency is critical is establishing a culture of safety embraced by all levels of staff and key stakeholders. Transparency affords healthcare organizations with a complete view of operational performance that enables
  • 23. comparison with other like facilities and standards of practice (Kerfoot, 2009). The publication of nursing outcomes and program evaluation reports is an example of organizational transparency. Nurse executives serve as role models and should support transparency, promote communication in all directions, and participate in shared decision-making within the organization. Lateral Violence (Nurse Bullying) Lateral violence is aggressive and destructive behavior, in this case, of nurses against each other (Woelfle & McCaffery, 2007). The end result is damage to another’s dignity, confidence, and self- esteem. Often, those who experience lateral violence in the workplace then transfer it to others. Lateral violence in nursing can consist of a variety of behaviors, ranging from unintentional, thoughtless acts to purposeful, intentional, destructive acts meant to harm, intimidate, or humiliate a group or individual. Behaviors can range from random instances to a pattern, and such behaviors can create a hostile work environment. Examples of behavioral lateral violence include • Talking behind others’ backs; • Scapegoating; • Criticizing a colleague in front of others; • Excluding a coworker from group interaction; • Withholding pertinent information; • Violating a coworker’s privacy and confidentiality; • Making inappropriate, condescending remarks; and • Displaying inappropriate nonverbal language, such as making faces or raising one’s eyebrows.
  • 24. Any time there exists an “us vs. them” attitude or when an imbalance of power occurs, conditions are prime for lateral violence. Examples of relationships in which lateral violence can occur include nurse manager to a staff nurse, nurse executive to a nurse manager or a staff nurse, nursing faculty member to a student nurse, and peer to peer. The consequences of lateral violence can manifest themselves as physical symptoms, increased absenteeism and, in extreme cases, suicide of the targeted employee. Increased turnover of staff is also a symptom (Beecroft, Kunzman, & Krozek, 2001). Lateral violence can place clients at risk for poor care and outcomes; for example, what might happen to a client when one nurse deliberately withholds pertinent information about the client’s care that needs to be shared with another nurse? Nurse executives must be aware of and address lateral violence in the workplace and implement strategies to prevent and stop such behavior, such as • Educating all staff and managers about this behavior; • Disciplining any manager or staff member who engages in this behavior; • Creating a culture that does not tolerate this behavior; • Implementing organizational transparency, or having an open culture; and • Implementing research to study this behavior in their own organizations. The Health Work Environment The health work environment is a topic that addresses nurses’ working conditions, which are linked to the quality of care that
  • 25. is provided to clients and clients’ safety. Accrediting organizations and Magnet Recognition Program® include standards that address working conditions because they are associated with health and safety outcomes for nurses and other healthcare providers (Geiger-Brown & Lipscomb, 2010). Employers are charged with the responsibility to assess and address aspects of the nursing work environment that have been linked to hazards and adverse exposures for nurses and the most common health and safety outcomes of nursing work. It is the responsibility of the nurse executive to ensure a healthy work environment while facilitating a culture that values diversity and recognizes individual and group contributions to organizational goals. Sloan Student Life Office (SLO), where the three Sloan Women in Management (SWIM) club co- presidents and two conference directors were gathered.1 SWIM’s 3rd annual conference, 11 months in the making, was scheduled to take place the next day. A winter storm was looming, with weather predictions varying widely from three to 50 inches of snow in the next 48 hours. It was almost 2:00pm and a decision had to be made – setup was slated to begin in minutes, and speakers were boarding their flights for Boston. Should the conference be canceled, modified, or go forward as planned? SLO Associate Director Marco Esquandolas looked at the five women seated in front of him and asked, “Ladies, what is your final call?” Background on SWIM The Sloan Women in Management club was founded in the 1980s and had become one of the largest clubs at MIT Sloan. The organization’s primary goals were to create an inviting and supportive community for all Sloan women; advance the careers of current female Sloan students; work with faculty, administrators, and the greater business community to increase opportunities for women in business; and, attract top female students to MIT Sloan. 1 The Student Life Office provides programs and services to foster a graduate student community based on collaboration and contribution. This includes
  • 31. oversight of student organizations and support on student events and initiatives. SINK OR SWIM Lauren Ankeles, Marine Graham, Roberta Pittore and Priyanka Ramamurthy July 5, 2013 2 In an effort to meet all of these goals and cater to the broadest set of women, SWIM hosted a variety of year-long programming including networking events, speaker series, professional development workshops, mentorship programs, and community building events. In addition, the club held three flagship events, one of which was an annual conference where students could dialogue with leading women in business. As of February 2013, SWIM had approximately 300 members,2 550 Facebook fans, 640 Twitter followers, 270 LinkedIn group members, and 300 individuals on the alumnae mailing list. The annual conference had quickly become the organization’s largest event, despite its inception just two years earlier. After the relatively small-scale initial conference featuring panelists primarily from the MIT Sloan community, the 2012 conference directors sought to create the gold standard in women’s business conferences. The “Innovating through Adversity” 2012 conference brought several renowned speakers including Fortune 500 CEO Laura Sen (BJ’s)
  • 32. and VP of Google Local/Maps/Localization Marissa Mayer to a sold-out crowd of over 400 attendees. All who participated hailed the conference a tremendous success. Male and female students who attended agreed that of the roughly 30 annual MIT Sloan conferences, SWIM’s was the best conference of the year. There was strong support and interest from the administration due to the high profile of 2012’s conference. As SLO Director Christine Pugh noted, “The 2012 conference knocked it out of the park – probably based on some learnings from the 2011 conference. I heard it was just phenomenal, enough people really raved about it that I’m very confident in saying that.” The 2013 Conference Leadership Team The 2013 SWIM club co-presidents, Rachel, Lea, and Tarini, were eager to build on the previous year’s success and, immediately after their election to their leadership roles in February 2012, they began planning for the 2013 conference.3 As the conference was the club’s largest event of the year, Rachel, Lea, and Tarini agreed that they wanted to maintain the previous year’s structure of heavy involvement with the event, taking on the roles of conference CFO, COO, and CMO, respectively. They would next need to select two directors who would oversee the content of the conference, including selecting the theme and speakers, motivating a large team to execute the event according to their vision, and serving as the masters of ceremonies for the event.
  • 33. The 2013 conference directors, Janet and Merrill, were selected in March 2012 by all of the co- presidents, both past and present, and the previous year’s conference directors. According to Janet, “I 2 Members include women from various Sloan programs (MBA, Leaders for Global Operations, Master of Finance, Sloan Fellows, Executive MBA). 3 Names of people from MIT Sloan have been disguised for the purposes of this case. Something I wanted to do was make a substantial impact on Sloan. There were lots of ways I could do that. I decided that one of my goals would be to work on the SWIM conference which has a very tangible output with a group of people I really respect.” The theme for the 2013 conference was set as “Dare to Fail: Taking Risks When it Matters Most.” Soon after deciding the theme, the directors finalized a shortlist of keynote speakers, began brainstorming speakers for panels, and scheduled meetings with
  • 34. relevant Sloan faculty and administration. The past presidents warned Rachel, Lea, and Tarini that they experienced significant tension the previous year over who had final authority in the decision- making process. Despite this warning, the same dynamic quickly developed with the new team. The co- presidents wanted be included in all key decisions, believing they needed to have a say in the selection of speakers and topics, both for the purposes of the conference, as well as to ensure integration with the SWIM brand and overall organizational strategy. The conference directors felt that they were being micromanaged for an event that they were directing. As the SWIM leadership team headed off for the summer to five different cities to pursue internships, expectations were set that Merrill and Janet would focus on recruiting speakers, and Rachel, Lea, and Tarini would work on sponsorship, operations, and marketing (including ticket sales), respectively. The five loosely agreed to check-in calls, though specific dates and milestones were not set. Maintaining contact during the summer proved challenging, as the group of five were in different time zones and had different work schedules. Throughout the summer Merrill and Janet continued to chip away at speaker recruitment without much success. Despite sending out multiple keynote requests, they continued to hit dead-ends due to scheduling conflicts and non-responses. On August 10 the first keynote was confirmed – Four Star General Janet Wolfenbarger – and the team breathed a
  • 35. small sigh of relief. As they all geared up to return to campus in the fall, however, there was still significant concern from the entire team about the lack of progress that had been made over the summer in finalizing speakers. Shortly after General Wolfenbarger was confirmed, the group faced yet another unanticipated setback. Merrill, whose summer internship had been at a start- up, had been offered significant equity to leave MIT Sloan and continue in her role at the start-up. On August 20, she sent the following email to the co-presidents: …I am nothing less than obsessed with this event – and with working with Janet and each of you – and I seriously considered returning to school JUST to direct the conference. However, that didn’t seem like the most logical decision, and I ultimately didn’t feel that giving up my stake in SINK OR SWIM Lauren Ankeles, Marine Graham, Roberta Pittore and Priyanka Ramamurthy July 5, 2013 4 the company was worth the conference, regardless of how amazing the conference is going to be. I do, however, have a proposal… I would like to ask to continue
  • 36. to co-direct the conference with Janet. I am committed to this event’s tremendous success, am passionate about the theme, and will work incredibly hard to develop speakers and panels that are worthy of SWIM. I’ve spoken with Janet, and she is in support of this. While it was not clear how involved Merrill would be moving forward, the co-presidents agreed to this arrangement. They were concerned about finding an alternate director given the late stage in the planning process and trusted that Janet and Merrill had discussed managing their time appropriately. The five-person team committed to weekly meetings to ensure that communication lines would remain open and progress would continue. Conference Planning Despite these challenges, the team maintained a positive outlook upon returning to campus for the fall semester, and worked hard to harness the energy of the first year MBA students. The annual conference was historically a crucial way to engage new students and identify future leaders for the organization. Committees were created to focus on speaker recruitment (led by Merrill and Janet), marketing (Tarini), operations (Lea), and sponsorship (Rachel). Speaker Recruitment Over the course of the fall semester, it became increasingly clear that Janet and Merrill were struggling to find speakers. The plan had been to secure at least two keynote speakers by the end of
  • 37. the summer, and then form teams of first year students to take the lead in recruiting four to five speakers for each of the two panels. The panel teams were moving forward with mixed success. Speakers were responding well to the theme of failure, but were unable to commit to the conference date. This had a significant effect on the rest of the conference planning, particularly the marketing team, which struggled to accurately frame the conference. With Merrill on leave from MIT, a disproportionate amount of work was placed on Janet. Though angry words were never exchanged, tension grew between the conference directors and the co-presidents. While the presidents had each other to rely on, Janet shouldered much of the pressure on her own, searching for a way to push forward and maintain an energetic exterior to keep the conference team engaged. By the end of November 2012, the panel teams were pushing out multiple speaker requests and tapping all of their networks for introductions.   • 38. before they left for winter break, as everyone would be spread out traveling the globe until the beginning of February. By the end of December, nearly all the speaking engagements were confirmed. On January 23, 2013, with two weeks left until the conference, original keynote General Wolfenbarger canceled her participation due to government budget-cuts. The team, exhausted from the search for speakers, scrambled once again to fill the gaps in the lineup and manage the fallout from attendees who had bought tickets intending to hear the General speak. The co-presidents were convinced that there was no way to find a keynote at this late date, but to their surprise a new keynote speaker who was a growing force in the venture capital world was quickly confirmed. This was just what the team needed to sell their last few tickets. Marketing In 2012, an aggressive marketing campaign had led to a sold out conference (400 tickets) nearly two months prior to the event. For the 2013 conference, the directors wanted to sell 500 tickets, at a slightly higher price – $25 for students and $50 for professionals. The main challenge for the marketing team, however, was not the price point, but that marketing needed to begin before speakers had been confirmed. Falling steadily behind 2012’s numbers, Tarini commented, “I don’t even know why the current attendees are buying tickets. Right now, I’m just selling air.” To compensate, the marketing plan from the prior year was beefed up to include
  • 39. social media, outreach to other schools, and mailing lists targeting professionals. December was a busy month for ticket sales thanks to the team individually emailing over 100 professionals in the area, including every female professor associated with MIT Sloan, launching an aggressive social media campaign, and revamping the promotional material each time a new speaker was added to the roster. However, ticket sales still lagged behind the previous year, and Tarini checked daily as the fear of a half-empty event continued to grow. The weekend before the conference, the team confirmed that the event was sold out. Operations The conference event space, the premier location on campus and a highly sought after venue, had been booked in June 2012. Lea formed a team to help with operations including vendor management (catering, facilities, custodial services, security, furniture rental, audio-visual needs, lighting, videography), day-of logistics management, and preparation for the VIP dinner, traditionally held the night before the conference to welcome speakers and thank corporate sponsors for their support. Students who had been active participants in conference planning were also invited to attend. To cut costs and improve logistics, the team opted for a new dinner venue close to campus, which meant building new relationships with vendors and planning the budget, menu, and event space from the ground up. A non-refundable deposit had been put down to hold the space, and the final payment for the dinner, estimated to be $3,000, would be due upon completion of the VIP dinner.

SINK OR SWIM Lauren Ankeles, Marine Graham, Roberta Pittore and Priyanka Ramamurthy July 5, 2013 6 During the planning period, the leadership team identified a major glitch – a classroom in the same building as the event venue, traditionally used as an overflow space for conference attendees, was booked for a class during the conference. Preparations had to be adjusted accordingly to accommodate the reduced seating and inflow of non-conference attendees that would be coming through the building entrance. By the end of January, all of the work orders and purchase orders had been set up, though no formal contracts had been signed, and vendors were scheduled to begin arriving at 2:00pm on Thursday, February 7 to set up (Exhibit 2). The MIT Sloan administration had agreed to host an alumnae breakfast at the conference. In-kind donations had been solicited from coffee and pastry shops in the area, who would be delivering items on the day of the conference. However, on January 24, the MIT administration notified Lea that they could no longer sponsor the alumnae breakfast as originally promised due to budget constraints. With just two weeks until the conference, the team scrambled yet
  • 41. again to adjust. Sponsorship The sponsorship team, led by Rachel, began planning for the conference in late June 2012. Initially a small group worked on drafting promotional materials that would be shared with potential sponsor organizations and would provide a summary of the 2012 conference, the vision for 2013, and information about the benefits of sponsorship. With this information in hand, the team began reaching out to the prior year’s conference sponsors to gauge their interest in sponsoring once again. The team had a fundraising goal of $40,000. This amount, in addition to the revenue from ticket sales, would enable them to cover all conference costs and have a buffer, which could be applied to the following year’s conference should the funds not be required. While many of the previous year’s supporters were interested in partnering with SWIM again, many companies did not have the same budget resources available to them, and as such the sponsorship team was hard pressed to raise enough money to fund the conference. Rachel turned to the first year student committee to help brainstorm a list of new sponsor organizations and reach out to their network of contacts. Despite getting off to a slow start, a driven first year student stepped up taking on virtually all fundraising tasks and, in late January, signed the final sponsor, bringing the fundraising total to $38,000, enough to fund the 2013 conference. Miraculously the week before the conference, speaker recruitment, marketing, operations, and
  • 42. sponsorship were all coming to a successful close. Final Preparations Monday, Feb. 4 Five days to go. On Monday, February 4, students arrived back on campus after their January break, ready for a new semester. The energy level was at an all -time high as the full conference team, 20 SINK OR SWIM Lauren Ankeles, Marine Graham, Roberta Pittore and Priyanka Ramamurthy July 5, 2013 7 members strong, assembled to discuss final details on the event that was now 11 months in the making. Much like a dress rehearsal, the operations team walked through all events for the week, hour-by- hour, to finalize last minute details. The marketing team confirmed that the event was officially sold out with 500 tickets disbursed (Exhibit 3), and the sponsorship team officially closed out their sponsorship efforts with $38,000 raised, confirmi ng that the conference would easily break even on costs (Exhibit 4). All 15 speakers had recently been confirmed and information on their travel logistics was being collected. Outside of the five-member leadership team, most students had been
  • 43. focused on their own assignments until now, and this was the first time they were able to see the breadth of work that had been completed to date. At the meeting, final decisions were collectively made regarding social media strategy for the day of the conference, speaker biographies were approved for the programs, and signs for the event space were designed and sent to the printers. Tuesday, Feb. 5 On Tuesday, between and after classes, the co-presidents focused on last minute details for the conference including printing nametags, determining the room- flow for the event space, communicating with the photographer, assigning handlers for VIP attendees, and setting up interviews between speakers and members of the media. Volunteer assignments, that took into consideration both the schedules and personal interests of 50 student volunteers, were confirmed. Late that afternoon, murmurs began that a snowstorm was approaching, but in New England, predictions of inclement weather were an ordinary occurrence. Weather reports were vague, varied, and contradictory across networks (Exhibit 5). The conference team continued operating according to the original schedule, knowing that since 1997 MIT had closed only twice due to snow. As Janet remarked, “Boston is Boston. There is really no way of knowing so let’s just keep going. It’s just go go go. There is a lot to be done.” However, as night approached, weather predictions became more distressing.

Wednesday, Feb. 6 Forty-eight hours before the start of the conference, weather predictions across different networks began to indicate the possibility that a major storm would hit the Boston area some time on Friday. Starting at 8:00am, the team began operating on two streams: (1) continuing to prepare for Friday’s conference with the finalized set of speakers, and (2) identifying alternate local speakers should flights be canceled. At 9:00am, an email was exchanged among the conference leadership team: SINK OR SWIM Lauren Ankeles, Marine Graham, Roberta Pittore and Priyanka Ramamurthy July 5, 2013 8 Weather looks like it’ll be ok in the AM on Friday but get steadily worse for the evening. Hopefully this means speakers will be able to arrive and leave without issue. I know an MIT professor who could possibly do a talk last minute (I’ll ask today) Thirty-six hours before the event, the conference team sent a message to all attendees notifying them that the conference would continue as scheduled and reminding them of key logistics (Exhibit 6). Since Monday, the team had been receiving emails from people on the waitlist for tickets, asking if there was anything they could do to get off the waitlist. Worried that people might be scared off by a potential storm, the marketing team began releasing tickets from the waitlist although the event was at full capacity. The remainder of the day unfolded rapidly: • 4:47pm: One of the corporate sponsors of the conference and a core sponsor of SWIM emailed to say their three representatives, who were coming from upstate New York, would not be attending due to the weather forecasts. Thursday morning began early, though none of the presidents or conference directors had been getting a full night’s sleep that week. The team pushed forward, operating under the assumption that either the conference would continue as planned, or face speaker cancelations. Snow won’t get bad until afternoon.” At 9:00am, Lea, Rachel, and Merrill attended a meeting at the SLO, at which time multiple contingency scenarios were discussed. The following email was sent to the entire leadership team to summarize the meeting and next steps: We talked about a couple of different scenarios with Marco Esquandolas, Christine Pugh, and Sharon Knope this morning: A. Move forward as planned – expect some speakers not to make it, and low attendance. Potentially hold a half-day conference. B. Cancel the conference this afternoon. Marco Esquandolas is doing some recon for us today on whether MIT is likely to close, but there is no indication that this will happen yet. C. At 11:30am, conference discussions were temporarily put on hold while Rachel and Tarini attended a SWIM event on personal finance and career decisions for women, unrelated to the conference. It was a longstanding goal that the conference not detract from SWIM’s general programing at any point during the year. As a rule, at least one, if not all three, of the club’s presidents attended any of the events the club held. Lea was not present, as she was initially planning to print the 500 nametags needed for the next morning. As a last minute decision, the team decided to hold off on this task, and she instead checked in with Marco to see if he had received any more information regarding the likelihood of an MIT closure. Ramamurthy July 5, 2013 10 message suggesting she had received some good news: “For now, please notify all vendors that we are moving forward as planned - I will let you know immediately if anything changes!” However, at 12:36pm, Lea sent a short, cryptic, and concerning text message to the co-presidents: “Just spoke with Marco - Can you come now?” Decision Time During the many months of planning, each member of the leadership team had visualized dozens of possible scenarios for the conference. In some, there were no attendees, the food never arrived, or the speakers forgot their talking points. In others, the room was packed and the crowd was on their feet giving thunderous applause for a life- and career-changing event. After so many setbacks, it was finally all within reach, and it was going to be glorious. Except now, there was a new and unprecedented scenario to consider – canceling the conference for a potentially monstrous snowstorm, when there was not a cloud in the sky or a snowflake on the ground. At 1:30pm on Thursday, February 7, SWIM’s three co- presidents and two conference directors, who had been in varying states of misalignment and frustration for so long, met with Marco in his office to sort through their options. Weather reports had been monitored

Now, there was every indication that a storm would hit Boston at some point, but most networks expected the storm to hit after 12:00pm on Friday, while others said it could be as late as Friday night. It was simply too early to tell. Janet and Merrill collected a status update of where each speaker was, and when planes would begin taking off toward Boston. They confirmed that all 15 speakers were unfazed by weather reports and were ready to attend the conference the next day, regardless of the potential snowstorm. One had already changed her ticket to fly in earlier to avoid any airport closings, and a second could not be reached as she was already on a flight from London. At precisely 2:00pm, vendors would begin setting up the venue and SWIM would be liable for the costs. Further, if MIT closed, the rental fees would triple from a one-day rental to daily rentals through the weekend, when the rentals could be picked up. Lea did a back of the envelope calculation and quickly saw that if the conference was canceled after vendors arrived, the costs could increase significantly. The catering company had already warned her that it was too late to cancel the perishable food items that had been prepared for the conference – it was unclear what percentage of the $13,000 catering bill would be recoverable. Lea knew what her vote was, but she was curious to see which way her two co-presidents were leaning. The MIT administration had not yet closed the Institute, and would not comment on if they would. MIT rarely closed its doors, but if it did no events would be
  • 51. allowed on campus, including the conference. What if the conference was canceled, MIT did not close, and the storm only hit Friday night? What would be the fallout with attendees and corporate sponsors? Rachel was particularly concerned about the ramifications for the sponsors, whom her team had worked so hard to secure. Would SWIM’s reputation be impacted? Would they be able to secure sponsorship funds in the future? Tarini wondered if attendees would show up for the conference, even if it was not canceled, and if they did, was there a risk of them being snowed in? If the team opted to cancel the conference, would they be forced to give back ticket revenue, and, if so, how would that affect the bottom line? Could they really give up on the event that they had sacrificed for and had devoted the last year of their lives to? The entire conference revolved around the theme of coming to the brink of failure and still finding a way to succeed. With graduation around the corner, there would be no second chances.

Given that most weather forecasts suggested that snowfall would only begin at 12:00pm Friday, and would not escalate until early evening, Merrill began championing the idea of a half-day conference. Could they run the events concurrently instead of sequentially, giving attendees the option to choose which speaker to hear from? Since the event space would be set up for one speaker at a time, would it even be feasible? Would attendees show up for a conference that only lasted three hours, and if not, would low attendance be disrespectful to the high profile speakers? The conference directors and co-presidents narrowed it down to three choices: 1. Cancel the conference before 2:00pm, thus saving operational costs that would otherwise be incurred. 2. Hold a half-day conference with concurrent speakers. 3. Wait until Thursday night to make a decision, based on more accurate forecasts and updates on speaker travel plans. While initially on the table, rescheduling was not an option for the team. With only three months left before the conference leadership team graduated it would be impossible to find a date that worked for all, or even most, of the speakers. Additionally, because the venue had no availability in March, April, or May, the team would need to find a new location for the conference. They would in effect be planning an entirely new conference in one-quarter of the time it took to plan the initial conference.
  • 53. For these reasons they knew rescheduling would not work. In his facilitator role, Marco Esquandolas remained an impartial observer, telling them, The way I look at clubs on campus is that it is your chance to practice management. You put this all together. You know best if you should cancel or go forward. In all my years here, this situation is unprecedented. If it is canceled, it will be devastating. If there is a storm, it will be devastating. If there is no storm, it will be devastating. I will support whatever decision you make.
