Risk assessment
All employers must conduct a risk assessment. If you have fewer than five employees you don't have to write
anything down.
We have started off the risk assessment for you by including a sample entry for a common hazard to illustrate what is expected (the sample entry is taken from an office-based business).
Look at how this might apply to your business, continue by identifying the hazards that are the real priorities in your case and complete the table to suit.
You can print and save this template so you can easily review and update the information as and when required. You may find our example risk assessments a useful guide
(http://www.hse.gov.uk/risk/casestudies). Simply choose the example closest to your business.

What are the
hazards?
Who might be harmed and how? What are you already doing? Do you need to do anything else to
control this risk?
Action by who? Action by
when?

Slips and hazards in
the kitchen
The actor’s Charlotte Thornton, Elaine
Waters and Glenn Waters have to be
aware of any trips or hazards present in
the kitchen, especially when there is hot
equipment and failing this may cause a
serious injury.
I am already ensuring to tidy anything away
that looks like a hazard to the cast of my film.
Ensuring work areas are clear, eg no boxes
left in walkways, deliveries stored
immediately.
Ensuring all areas are well lit, including the
stairs.
No trailing leads or cables.
Better housekeeping in kitchen
needed, eg on spills.

Trips in the bedroom The actor charlotte Thornton has to be
aware of any trips and hazards in the
bedroom to prevent herself from tripping
or causing a serious accident.
I am ensuring to tidy everything away and
keeping my bedroom clean constantly.
Ensuring walkways out the door are clear and
well lit.
No trailing leads or cables which could cause
trips and hazards.
Better housekeeping in the bedroom
and ensuring everything is kept as
clean as possible.
Myself From now on

Falling on the road The actor’s Charlotte and Harry must be
aware of not tripping on the road when
they are running in the capture scene. If
the weather has been raining, then I may
need to reschedule if the road is too
dangerous ad slippery.
Watching out for anything in the road that
could cause any dangerous trips or hazards.
Regularly checking the weather for the film
day.
Ensuring to reschedule the shoot if the
weather has previously or currently
been bad.
Myself From now on
17/01/19
Slips and danger on
the porch
The actor’s Anthony Waters, Lucy
Clayton and Gina Elward have all got to
be aware of the slippery porch and
ensure that if the porch has been wet or
rained on then they are extra careful to
always walk and never run.
I am ensuring that the film day is going to be
dry and that there has been no rain to make
the porch slippery.
I will check to reschedule days that are
raining.
On the day if there is not a chance to
reschedule then I will ensure to mop
the porch to ensure leaves and rain is
not going to cause any dangers.
Myself From now on
17/01/19
Knife used for the
capture scene
The actor Glenn Waters uses a knife
when during the capture scene, following
Charlotte through the woods.
I am ensuring that the knife is protected with a
cloth so if anything at all happens, it is slightly
protected from risking anyone.
I will check that the weather is
appropriate so no slips can be made
and the knife cannot be dropped or
cause any dangers to actors acting.
Myself 17/01/19
You should review your risk assessment if you think it might no longer be valid (eg following an accident in the workplace or if there are any significant changes to hazards, such as new work equipment
or work activities)
