Codesain CEO Rachel de Sain presented at the 2019 HIMMSEurope conference held in Helsinki in June. She shared information about the Australian Digital Health Journey and reminded us that we need to ensure the investment in transforming the health sector through digital to achieve anytime anywhere health ensures its for everyone. We can and should also look at this transformation as an opportunity to not only achieve the quadruple aim, but extend that to fuel innovation and new growth for the economy.

Thank you for the introduction and thank you to you all for attending today,
In the minute that it took for that very lovely introduction an average of 1.1million swipes on tinder happened, over 38million whats app messages were sent and billions of microtransactions be they financial, systems, data, media or other were actioned.
In fact some of you may have contrinbuted to that number yourselves.
But I don’t need to tell you that, as attendees of HIMMS Health2.0 you’re well aware of the impact industry4 is having across multiple verticals – be it finance, automotive, entertainment and even traditional stalwarts like Justice and Education are starting to create new service models driven by the connected revolution.
But what about Health? We are starting to hear about some great things, especially at this conference and already in this session.
But today I’m here to talk to you about the Asutralian perspective.
A vast giant landmass, we’ve been in many ways forced because of our geography to really think about this.
Can we deliver Anytime, Anywhere access and importantly for everyone? That is something I want to really make a point of, we cant not stand aside and not accept that digital does tend to create services for those who can afford it, and we should be actively looking to ensure that it closes the digital divide and enables more equity of access for all.
Australia is an island continent with over 16,000 miles of coastline, 24 million inhabitants and a strong history of innovation - the black box recorder, inventors of wifi, cochlear implants and even the selfie……..
the world’s largest producer of gold, iron ore and uranium
a major global producer of agricultural commodities
a leader in investment management, with the sixth largest pool of funds under management in the world
the world’s third most popular destination for students
the seventh largest international tourism market.
And while our beautiful island continent has its benefits, its landscape creates some challenges that have forced us to rethink how we deliver health and care to all. we’ve tackled the inequality of access that affects our regional and remote based community members with a range of innovative models of healthcare – many of you will have heard of the royal flying doctors, that continue to service many of our remote, rural and regional people.
We started back in 1999 offering Gp practices an incentive to digitise their practise and this led to many buying computers and starting to use digital solutions for managing patient records.
As part of the 2010/11 Australian federal budget the Hon. Nicola Roxon (Minister for Health and Aging) announced the PCEHR as a "key building block of the National Health and Hospitals Network”
Ans as you can see things have been ramping up
Mention a few of them
The Landscape in Australia is still in build mode, while we’ve had bipartisan support and significant amounts of money spent over $2billion Australian in the last 6 years, developing the national infrastructure that’s not including state, local and private sector spend.
Certainly behind countries like Denmark, and I’d say finland no tonly with yoru record but for me something I am very interested in is your approach to secondary uses of data and empowerinfg patients to be able to become a broker of their own health information.

How we will achieve that anytime, anywhere for everyone goal through the strategy
In 2017 we embarked on a project to work with industry, government, clinicians and of course the community to understand what they wanted our strategy to be and we developed and gained agreement on the 2018-22 strategy which focuses on these 7 pillars…

Earlier this year we shifted our national record system called the my health record from Opt in to Opt out, and now have 90% of eligible Australian residents, that’s about 20M people with a record, and over 15million clinical documents, 39million medications documents and over 15,600 health orgs connected incluind hospitals, aged care, pathology, GP, diagnostic labs, aged care and speicalists.

So that anyone, at anytime from anywhere can with appropriate permissions and consent can access a patients information.
But the mhr is but one compnonent of the national infrastruce the foundations and connectivity and interoperability toosl and standards are in place to start to shift from being the only car on the highway to being an enabler to others..

But I think, particarly int eh informatics community we can forget that Digital Health is a lot more than records, clinicians want to access a patients data, yes but they also need to hacve hardware and software to do that, and as we start to see digital therapeutics and connected medical devices play a larger roel in teratments we see the life sciecnes and biotech medtech sectors tarting to play alarger role in Digital Health. We also see that the GAFA or GAFAM big 5 of tech are entering the fray although Microsoft di shyt down healthvault, the invest in verily is pretty impressive nad deepmind is doing some good thigns,.
For me really I see this as just health evolved with Data being the fluid that moves across the system and identity and authentication as the gates and levers that allow it to do so.
So to give you an example of how this might work in rbinging together different data sets and solutions to create user centric solutions that sit across the national infrastructure
Anyone suffer from Asthma here? well perhaps like me your asthma can be triggered by high pollen count. The Bureau of Meterology has a pollen count geospatial database which has an open API. Meaning Asthma Australia or in fact any company could create an app that with a patients consent accesses their MHR data, links this with the BOM pollen count - AND uses the GPS on their phone, or run keeper or whatever mapmy run type service they use and say - Hey Rach, we see you haven’t filled your inhaler script for a while and we can also see that the pollen count is going to be high on your walk to work today, maybe go pick up another script, or we’ve mapped an alternative route to walk to work that doesn’t go down that street with all the tree’s on it.
Talk about addition of things like Precision medicine
And I think we can also start to look beyond just Health, to look at really transforming what it means to have a healthy happy society, we have to shift from find and fix to predict and prevent.

So my final note I want to leave you with is the idea of the 5 star aim. 20 years ago we have the triple aim, so anytime government was planning to invest heavily in health it should achieve the triple aim of improving patient outcomes, overlal population health a nd of course reducing cost, that evolved about 10 yeasr ago to add in that ilarge funded nitatives should also seek to improve wellbeing for the health care providers, clinicians and staff.
I belvie in 2020 it should become commonplace to also think of the investment as an opportunity create, or help to fuel a new growth industry for a country as well.
