35. CASE STUDIES O SOME PROBLEMS ENCOUNTERED IN PLANT DURING
INSTALLATION AND MAINTENANCE
36. Resolution on each of the Case studies of some problems encountered during
installation, Test, Commissioning & Maintenance of Electrical /Instruments/ Grounding
Protection Circuitry
• The case of short circuit caused by rodents and reptiles, birds, natural wind were drastically reduced by sealing properly all entrance holes of cable
underneath panels, switch boards. Proximal tress/shrubs were cut off while wind breakers were installed strategically.
• Flash overs at terminal / lug points were curtailed using shielding tapes called semi-conducting tape. Cleanliness was encouraged.
• Burn outs at spliced points on surface installation in trays & conduits was curtailed by using specified cable sizes&conduit size correspondence
• Burn outs at termination and splice point on downhole pump circuitry was resolved by application of mastic sealing bituminous tape
• Short circuiting due to water / chemical ingress at joints and terminal points was resolved using same mastic bituminous tape
• Short circuiting due to plant inherent activities was minimized drastically using proper and adequate sign post for some busy locations
• Short circuiting due to poor administering of workmanship and or technical know-how was solved using experienced and qualified person
• Failure due to non compliance strictly to the plant design and minimum project requirements was resolved by referencing drawings before installation, test
and commissioning
• Potential shock risk during test / commissioning due to poor earthing / ground protection system was resolved by initially make sure that the Ground pit
electrode were tested continually to make sure that the total resistance remains as low as possible.
• Damages to Cable, Facilities due to spontaneous electromagnetic waves was eliminated following up the dedicated plant design as well as segregation of
cables
• Damages to cable , Facilities due to very high level of electrostatic charges resultant from lightning and surges was resolved by making solid bonding
between the cables armour, Facility chassis and general earth pit
• usage of neutral grease to lubricate switch and engagement points of switchgears system both for the low voltage and high voltage operated system.

37. Safety on Electrical Activities
• Electrical lines/equipment (no matter how low the voltage) shall be de-energized prior to work/repairs being performed.
• Only qualified electrical technicians when authorized by site management shall perform special “HOT WORK”.
• Field repairs or tampering with any electrical equipment by unauthrozied persons not permitted
• High voltage electrical equipment shall be properly protected and identified by tags etc.
• All electrical cords, equipment is to be visually checked by the user daily or before use.
• Temporary electrical cords and cables shall be covered or elevated to protect them from being damaged by personnel or
equipment and to eliminate tripping hazards.
• When performing maintenance on electrical systems, lockouts and tag outs must be used
• No electrical tool or equipment is to be used unless it is in proper operating condition
• Cord sets must be free of splices and have an adequate ground conductor.
• All electrical equipment is to be formally inspected every 90 days with written document.
K
jis Global. Serv.
o
38. • Motor control boxes and disconnect switch handles should be checked with the back of the hand
before grasping.
• Keep adequate clearance between electrical lines and object being moved or handled.
• All portable electrical tools/equipment shall be three wires grounding type and protected by earth
leakage circuit breaker, assured ground system, or double insulated
• Never break in to concrete when digging or excavating unless approved by authorized personnel.
Some but not all electrical conduits are covered with coloured (red) concrete as a warning that
electrical lines are in the area.
