I. PURPOSE
To establish a uniform procedure for the completion of the Field Operations Electronic
Activities Report, also known as the Officer’s Electronic Log.
II. POLICY
The Field Operations Electronic Activities Report or Officer’s Electronic Log shall be
completed by each Animal Control Officer (ACO) assigned to work in the field.

Applicable to all Field Operations personnel.

VII. PROCEDURE
A. Officer’s Electronic Log
The Officer’s Electronic Log is a form used to accurately describe all activities taken
by ACOs, Senior ACOs and Supervising ACOs assigned to field duties. Special
attention shall be given to accurately and concisely describe what actions are taken
during the handling of requests for service. Abbreviations may be used in the
completion of the Officer’s Electronic Log, provided they conform to the list of

Electronic Log Activity Result Codes and Required Information included as an
attachment to this policy.
All hand written entries shall be block printed using black ink. Felt tip pens shall not
be used. White correction fluid or correction tape shall be used to make corrections to
hand written content only. Corrections to the auto-filled sections must be made in
Chameleon and the Electronic Log reprinted. The Officer’s Electronic Logs are to be
completed as follows (refer to the correlating numbered sections on sample):
1. DAY/DATE: The weekday and date will automatically be added to the electronic
log when the ACO is entered into Chameleon at the start of the shift. The
Chameleon Shift window is accessed by clicking on the white truck icon located at
the top of the Dispatch View window (refer to the correlating numbered sections
on the Chameleon Shift window sample).
2. OFFICER: The ACO’s Person ID# and name will be automatically added to the
log when the Officer’s Person ID# is entered into Chameleon for the shift.
3. UNIT: Assigned vehicle number for the shift will be automatically added to the log
when the ACO enters the vehicle number into Chameleon for the shift.
4. START MILES: Assigned vehicle odometer reading at the beginning of the shift
will be automatically added to the log when the ACO enters the starting mileage
into the Chameleon Shift window at the beginning of the shift.
5. END MILES: Assigned vehicle odometer reading at the end of the shift will be
automatically added to the log when the ACO enters the ending mileage into
Chameleon Shift window at the end of the shift.
6. TOTAL MILES: The total miles driven during the shift will be automatically
calculated and added to the log when the start and end miles are entered into the
Chameleon Shift Window. (The total miles will not show up on the Chameleon
Shift Window itself).
7. RIDE-ALONG: First initial, last name and badge number (if applicable) of an
ACO trainee, trainer, Explorer Scout or other observer. If there was no ride-along,
enter N/A.
8. Code 7 ASSIGNED: Lunch/Dinner break starting time assigned in briefing for the
shift.
9. CODE 7 TAKEN: Lunch/Dinner break actual start time for the shift.
10. PAC: Assigned PAC set number.
11. COURT DATES: Court appearance dates as given in briefing for the shift.
12. STARTING CALLS: Number of calls received at the start of the shift.

13. # CALLS RECEIVED: Number of calls received from dispatch, supervision, FICs
and calls received from other ACOs.
14. VEHICLE DISCREPANCIES: If there is a vehicle discrepancy, enter “YES,” if no
discrepancies enter “NO.” Discrepancies are to be noted in the comments section
of the log. All discrepancies shall be brought to the immediate attention of the
Watch Commander.
15. FEES COLLECTED $$$: The total amount of fees collected during the shift.
NOTE: All blank lines at the top of the electronic log must have a positive entry or
the log will be returned for correction and/or reprinting.
16. Calls completed, cancelled, and UTMs will be automatically added to the log
when the call is disposed from the officer’s box within the Chameleon Dispatch
View window. It shall ultimately be the Officer’s responsibility to correctly
complete, update, and dispose each call. The Chameleon inputting requirements
are:
a. Complete all Chameleon Windows accurately prior to printing the Electronic
Log. The Officer will have to reprint the log if there is missing or inaccurate
information.
b. Calls will print out on the Electronic Log according to the working times and
completed times. If these fields are not stored properly in Chameleon, the logs
will have missing and/or inaccurate information and will require correction.
The total time spent on a call shall include the time spent to input the call
information (e.g. creating memos, creating Animal ID#’s, etc.) into
Chameleon.
c. All Unable To Make (UTM) calls must have a sequence indicating the UTM.
UTM calls and cancelled calls will print out first on the Electronic Log since
there are no working times.
d. All field initiated random patrols shall require the Officer to create an Activity.
Field initiated patrol checks shall be in 15 minute increments (10 minutes of
patrol; 5 minutes to input).
e. When handling calls that require an Officer to respond to multiple locations,
such as an impound resulting in transporting animals to Veterinary clinics or
rehabilitation centers, the Officer must create an additional sequence for each
address. The activity address shall be changed on each sequence to reflect each
address visited in order for it to reflect accurately on the Electronic Log. When
the change of address is for animal transport, leave the city next to the GEO
the same as the city of impound for billing purposes. The jurisdiction on the

Chameleon Kennel window shall also reflect the city in which the animal(s)
was impounded.
f. On the Chameleon Activity Window, the officer can utilize the “Comments”
line to enter the information required for a specific “result code”. For example,
for a QR you type in Owner’s name, the DOB and whether via 10-21 or
observed healthy. If the call is to be re-filed, after you have updated your
sequence, you will need to delete the information from this line so that it will
not go on to the next sequence.
Any other pertinent information obtained by a call taker or ACO will be added
to the appropriate memo field and not in the Activity Window Comment
section. The comment section is not to take the place of detailed memos but is
intended to provide basic information.
g. When impounding more than one animal, there must be a sequence created for
each animal. This allows each animal to print out on the Electronic Log.
Remember to change the quantity of impounds per sequence to “1” (i.e.: 1 UD
per sequence instead of 2 UD on one sequence).
h. Remove previously impounded “cling on” animals from re-filed Stray Trap
activities or the animal will print out on the log.
i. When inputting impounded animals into the Chameleon Kennel Window, the
Officer must ensure that the correct sequence is entered next to the activity
number for each animal. If the wrong sequence is entered, the Electronic Log
will print out incorrectly.
Note: The kennel cage number and the blue tag number will automatically
populate on the Electronic Log next to the impounded animal’s description.
17. COMMENTS: Any extended timeframes that are unaccounted for on your log are
to be documented in the comments section (i.e.: 10-19 ref safety meeting 1300-
1400 hours, Main Street Car Wash, PIP, 10-17 at OC Animal Care, special
assignment, etc.). The beginning and ending time of these activities shall be noted.
Senior ACOs acting as Watch Commander must list specific duties performed
during down time. “Watch Commander Duties” as an explanation will not be
acceptable. Lunches will be apparent on the log as an extended timeframe
coinciding with the “Code 7 Taken” time at the top of the log. Break times are to
be noted in the Comments section.
18. TOTAL NUMBER OF CALLS: The total number of calls (completed, cancelled,
and UTMs) for the shift will be automatically calculated and added to the log.

19. UTMs: The number of UTMs for the shift will be automatically calculated and
added to the log.
20. TOTAL COMPLETED: The number of completed calls for the shift will be
automatically calculated and added to the log.
21. I CERTIFY THAT… Officer’s signature and badge number.
22. REVIEWED BY: Initials and badge number of reviewer. If corrections are needed
after being reviewed, the log is to be returned to the officer with this signature only
and notes indicating the corrections necessary.
23. APPROVED BY: Initials of approving Lieutenant. This line is only to be signed
once all corrections have been made and the log is ready to be finalized.
24. DATE REVIEWED: Date Officer’s log was reviewed.
B. Printing the Electronic Log
1. From the Activity Window in Chameleon, click on Reports at the top and then run
a Report.
2. Scroll down to Field Reports and double click ACO Daily Report. Enter the date
as yyyy-mm-dd or click on the calendar icon and select the date.
3. Enter the Officer’s Person ID#. (P0123456, the “P” must be included) and click
OK. The Electronic Log for that date and Officer will generate. To print the log,
click on the Printer icon at the top of the screen.
C. Electronic Log Distribution
1. Officer’s Electronic Logs shall be completed at the end of the Officer’s shift and
forwarded to supervision via your supervisor’s box in the briefing room. If you
are unable to complete your log by the end of the shift, you are to contact the
Watch Commander at least fifteen (15) minutes prior to the end of the shift for
guidance and/or approval to complete the log the following day. Logs shall be
completed and/or corrected no later than an Officer leaving for the field prior to
their next scheduled shift. Supervision may grant permission to deviate from this
directive.

  • 1. Number: 300.48 SUBJECT: FIELD SERVICES ELECTRONIC ACTIVITIES REPORT Page: 1 Date: 3/3/08 Approved: Jennifer Hawkins DVM, Director Revised: 12/3/14 I. PURPOSE To establish a uniform procedure for the completion of the Field Operations Electronic Activities Report, also known as the Officer’s Electronic Log. II. POLICY The Field Operations Electronic Activities Report or Officer’s Electronic Log shall be completed by each Animal Control Officer (ACO) assigned to work in the field. III. SCOPE Applicable to all Field Operations personnel. IV. FORMS Field Operations Electronic Activities Report or Officer’s Electronic Log (sample). Field Operations Chameleon Shift window (sample). Field Operations Electronic Log Activity Result Codes and Required Information V. REFERENCES Not applicable. VI. DEFINITIONS Not applicable. VII. PROCEDURE A. Officer’s Electronic Log The Officer’s Electronic Log is a form used to accurately describe all activities taken by ACOs, Senior ACOs and Supervising ACOs assigned to field duties. Special attention shall be given to accurately and concisely describe what actions are taken during the handling of requests for service. Abbreviations may be used in the completion of the Officer’s Electronic Log, provided they conform to the list of

All hand written entries shall be block printed using black ink. Felt tip pens shall not be used. White correction fluid or correction tape shall be used to make corrections to hand written content only. Corrections to the auto-filled sections must be made in Chameleon and the Electronic Log reprinted. The Officer’s Electronic Logs are to be completed as follows (refer to the correlating numbered sections on sample): 1. DAY/DATE: The weekday and date will automatically be added to the electronic log when the ACO is entered into Chameleon at the start of the shift. The Chameleon Shift window is accessed by clicking on the white truck icon located at the top of the Dispatch View window (refer to the correlating numbered sections on the Chameleon Shift window sample). 2. OFFICER: The ACO’s Person ID# and name will be automatically added to the log when the Officer’s Person ID# is entered into Chameleon for the shift. 3. UNIT: Assigned vehicle number for the shift will be automatically added to the log when the ACO enters the vehicle number into Chameleon for the shift. 4. START MILES: Assigned vehicle odometer reading at the beginning of the shift will be automatically added to the log when the ACO enters the starting mileage into the Chameleon Shift window at the beginning of the shift. 5. END MILES: Assigned vehicle odometer reading at the end of the shift will be automatically added to the log when the ACO enters the ending mileage into Chameleon Shift window at the end of the shift. 6. TOTAL MILES: The total miles driven during the shift will be automatically calculated and added to the log when the start and end miles are entered into the Chameleon Shift Window. (The total miles will not show up on the Chameleon Shift Window itself). 7. RIDE-ALONG: First initial, last name and badge number (if applicable) of an ACO trainee, trainer, Explorer Scout or other observer. If there was no ride-along, enter N/A. 8. Code 7 ASSIGNED: Lunch/Dinner break starting time assigned in briefing for the shift. 9. CODE 7 TAKEN: Lunch/Dinner break actual start time for the shift. 10. PAC: Assigned PAC set number. 11. COURT DATES: Court appearance dates as given in briefing for the shift. 12. STARTING CALLS: Number of calls received at the start of the shift.

VEHICLE DISCREPANCIES: If there is a vehicle discrepancy, enter “YES,” if no discrepancies enter “NO.” Discrepancies are to be noted in the comments section of the log. All discrepancies shall be brought to the immediate attention of the Watch Commander. 15. FEES COLLECTED $$$: The total amount of fees collected during the shift. NOTE: All blank lines at the top of the electronic log must have a positive entry or the log will be returned for correction and/or reprinting. 16. Calls completed, cancelled, and UTMs will be automatically added to the log when the call is disposed from the officer’s box within the Chameleon Dispatch View window. It shall ultimately be the Officer’s responsibility to correctly complete, update, and dispose each call. The Chameleon inputting requirements are: a. Complete all Chameleon Windows accurately prior to printing the Electronic Log. The Officer will have to reprint the log if there is missing or inaccurate information. b. Calls will print out on the Electronic Log according to the working times and completed times. If these fields are not stored properly in Chameleon, the logs will have missing and/or inaccurate information and will require correction. The total time spent on a call shall include the time spent to input the call information (e.g. creating memos, creating Animal ID#’s, etc.) into Chameleon. c. All Unable To Make (UTM) calls must have a sequence indicating the UTM. UTM calls and cancelled calls will print out first on the Electronic Log since there are no working times. d. All field initiated random patrols shall require the Officer to create an Activity. Field initiated patrol checks shall be in 15 minute increments (10 minutes of patrol; 5 minutes to input). e. When handling calls that require an Officer to respond to multiple locations, such as an impound resulting in transporting animals to Veterinary clinics or rehabilitation centers, the Officer must create an additional sequence for each address. The activity address shall be changed on each sequence to reflect each address visited in order for it to reflect accurately on the Electronic Log. When the change of address is for animal transport, leave the city next to the GEO the same as the city of impound for billing purposes. The jurisdiction on the

On the Chameleon Activity Window, the officer can utilize the “Comments” line to enter the information required for a specific “result code”. For example, for a QR you type in Owner’s name, the DOB and whether via 10-21 or observed healthy. If the call is to be re-filed, after you have updated your sequence, you will need to delete the information from this line so that it will not go on to the next sequence. Any other pertinent information obtained by a call taker or ACO will be added to the appropriate memo field and not in the Activity Window Comment section. The comment section is not to take the place of detailed memos but is intended to provide basic information. g. When impounding more than one animal, there must be a sequence created for each animal. This allows each animal to print out on the Electronic Log. Remember to change the quantity of impounds per sequence to “1” (i.e.: 1 UD per sequence instead of 2 UD on one sequence). h. Remove previously impounded “cling on” animals from re-filed Stray Trap activities or the animal will print out on the log. i. When inputting impounded animals into the Chameleon Kennel Window, the Officer must ensure that the correct sequence is entered next to the activity number for each animal. If the wrong sequence is entered, the Electronic Log will print out incorrectly. Note: The kennel cage number and the blue tag number will automatically populate on the Electronic Log next to the impounded animal’s description. 17. COMMENTS: Any extended timeframes that are unaccounted for on your log are to be documented in the comments section (i.e.: 10-19 ref safety meeting 1300- 1400 hours, Main Street Car Wash, PIP, 10-17 at OC Animal Care, special assignment, etc.). The beginning and ending time of these activities shall be noted. Senior ACOs acting as Watch Commander must list specific duties performed during down time. “Watch Commander Duties” as an explanation will not be acceptable. Lunches will be apparent on the log as an extended timeframe coinciding with the “Code 7 Taken” time at the top of the log. Break times are to be noted in the Comments section. 18. TOTAL NUMBER OF CALLS: The total number of calls (completed, cancelled, and UTMs) for the shift will be automatically calculated and added to the log.

UTMs: The number of UTMs for the shift will be automatically calculated and added to the log. 20. TOTAL COMPLETED: The number of completed calls for the shift will be automatically calculated and added to the log. 21. I CERTIFY THAT… Officer’s signature and badge number. 22. REVIEWED BY: Initials and badge number of reviewer. If corrections are needed after being reviewed, the log is to be returned to the officer with this signature only and notes indicating the corrections necessary. 23. APPROVED BY: Initials of approving Lieutenant. This line is only to be signed once all corrections have been made and the log is ready to be finalized. 24. DATE REVIEWED: Date Officer’s log was reviewed. B. Printing the Electronic Log 1. From the Activity Window in Chameleon, click on Reports at the top and then run a Report. 2. Scroll down to Field Reports and double click ACO Daily Report. Enter the date as yyyy-mm-dd or click on the calendar icon and select the date. 3. Enter the Officer’s Person ID#. (P0123456, the “P” must be included) and click OK. The Electronic Log for that date and Officer will generate. To print the log, click on the Printer icon at the top of the screen. C. Electronic Log Distribution 1. Officer’s Electronic Logs shall be completed at the end of the Officer’s shift and forwarded to supervision via your supervisor’s box in the briefing room. If you are unable to complete your log by the end of the shift, you are to contact the Watch Commander at least fifteen (15) minutes prior to the end of the shift for guidance and/or approval to complete the log the following day. Logs shall be completed and/or corrected no later than an Officer leaving for the field prior to their next scheduled shift. Supervision may grant permission to deviate from this directive.
