I. PURPOSE
To establish a procedure for completing and explaining the Free Health Exam form issued to
adopters.
II. POLICY
A Free Health Exam form must be completed and explained to the customer when they adopt a
dog or cat.
III. SCOPE
Applicable to all OC Animal Care (OCAC) staff who are responsible for processing adoption
transactions.

VII. PROCEDURE
A. Completion of Form:
1. Animal ID Number: Enter the Animal ID assigned by the Kennel Window.
2. Date of Adoption: Enter the date the animal leaves with the owner.
3. Species: Enter “canine” for dog or “feline” for cat.
4. Breed: Enter the primary and secondary breed as entered in the Chameleon
Animal ID Window.

5. Age: Enter the approximate age of the animal as entered in the Chameleon
Animal ID Window.
6. Sex: “S” for female spayed and “N” for male neutered.
B. Items to be Explained:
1. Free Health Examination paragraph:
a. This notice entitles the adopter to a free health examination within
seventy two (72) hours with any veterinarian that is associated with the
Southern California Veterinary Medical Association (SCVMA) in LA
AND ORANGE COUNTIES ONLY.
b. SCVMA Members are found on the attached list or on internet at
www.scvma.org.
2. Right of Exchange:
a. OCAC grants a Thirty (30) day exchange period if a medical or
behavioral problem is discovered once the animal has been adopted.
b. The original sales receipt must be submitted at the time the adopted
animal is returned to OCAC.
c. A 30-day grace period is provided to give the adopter the opportunity to
select the same type of animal (i.e., dog is adopted and exchanged with
another dog).
d. Any fees incurred after the adoption is the sole responsibility of the
adopter.
e. Adoption fees are non-refundable.
