Duties: Collaboration with Medical Center Leadership (Hospital Director, Chief of Staff,
Nurse Executive and other leadership) to develop and implement an in-patient Hospice
Palliative Care (HPC) Service for the Lexington VAMC. Provision of care as hospice/geriatric
care provider to Veterans in the sub-acute Community Living Center. Continuing assessment
and revision of this program with collaboration of the regional and national Veteran Health
Administration (VHA) stakeholders.
Daily duties consist of Primary Care Provider to in-patient adult/geriatric/hospice
population, sub-acute care including chronic disease management with daily
management of CHF/COPD/Diabetes/Neuromuscular Disease/HepatoRenal
Failure/Substance Abuse/Cancer Syndromes; management of Veterans admitted to
sub-acute rehab; management of Veterans admitted for Hospice/End-of-Life Care. Also
collaborate with contracted community agencies who provide Veteran care across the
state and as one of the first responders to all acute care HPC consults within the VA
Medical Center. Position requires strong assessment skills, f/u of
labs/radiology/respiratory therapy/PT/OT/Speech therapy; ability to manage pain and
other EOL symptoms, chronic disease management, rehabilitation care and
individualize patient treatment plans in collaboration with other medical services.
Responsibility as Program Coordinator for the Lexington VAMC Hospice Palliative Care
Program includes answering Palliative Care Consults in hospital, long term care and out-
patient settings; marketing services to all interdisciplinary clinical staff through in-services and
coordination of Hospice Palliative care across all service lines to insure expert patient care
delivery; and collaboration with the regional and national Hospice Palliative Care VHA
leadership.

Family Nurse Practitioner Take Care Clinic
Walgreen’s Health Care Clinics, Northern Kentucky
Duties: Providing episodic and emergent care for patients ranging for 18 months to 100
years old. Focus on everyday family healthcare needs provided in a clinic setting, including
episodic care for infections, dermatitis, pain, flu. Also provide episodic management of chronic
disease. In this setting, the NP provides the whole array of services from sign in to discharge
plus assuming role of office manager, marketing specialist and housekeeping. Time of clinic
visit from start to finish is 15-20 minutes per patient. Personal rotation through 4 clinics
located in Florence, Burlington, Crescent Springs and Highland Heights demonstrates
enormous flexibility and critical thinking skills.

Nurse Practitioner Hospice Palliative Care

Duties: Collaboration with Medical Center Leadership to develop and implement an in-patient
Hospice Palliative Care Service for the Cincinnati VAMC. This program was interdisciplinary
with the patient caseload located in "scattered bed" format within the hospital and a consult
palliative care service in acute care. The impact of this program on the medical center was
earlier transition of appropriate patients out of critical care to the in –patient unit and /or home
hospice, with less number of days on diversion.
Daily duties consist of Primary Care Provider to hospice residents with weekly and prn IDT
rounds, performance of admit and discharges, first responder to all acute care HPC consults
for medical needs and daily rounds with HPC team in acute care to follow referrals. Both
campus duties include collaboration on a daily basis per face-to-face, phone, e-mail and/or
CPRS contact with my collaborating physicians.

doing admits/discharges and on-going assessments and daily interventions/care-planning for
critically ill hospice patients in pain and symptom management through to discharge back to
home/LTCF or death. Patient population ranged from young adult to geriatrics. Afternoon:
Palliative medicine clinic sees average of 7-10 patients per afternoon, doing H&P, review of
PMH, labs, tests, chief complaint for new clients prior to MD exam and f/u visit on established
patients.

Director: Hospice of Northern Kentucky/Nurse Practitioner Hospice Palliative Care

Duties: Direct supervision of seventy plus administrative and clinical interdisciplinary staff
responsible for providing expert end-of-life care to all-aged population in a six county service
area in Northern Kentucky. Practice sites were rural and urban, home, LTCF, assisted living
facility, hospital. Successes during my four years in this position included success in meeting
all state and national regulatory requirements per on site surveys, increase in patient census
(doubled), establishment of an in-patient hospice unit at St Luke East Hospital, establishment
of two hospice units within long term care facilities and an increase of revenue over expenses.
As an ARNP, I practiced independently and collaboratively with the hospice medical director
and other attending physicians within the five county service areas. My duties included
consultative patient assessment and establishing and revising patient plan of care, staff
supervision, staff development and some marketing.

I have guest lectured for surrounding universities, other health care facilities and have
presented seminars at state and national conferences.

follow up care. In this university setting I worked with chemotherapy and radiation trial
protocols, data collection for research, provided acute in-patient care for blood and marrow
