Personal Statement
I am an outgoing, professional, devoted and clearly dedicated individual who has an
ambition and ability to succeed in any given environment. I have vast amount of
experience in the long-term care, intensive care, family practice, emergency room
and retail clinic urgent care environments of the health care industry with positive
natural ability to quickly learn, am detail oriented, strong in computer skills, an
outside the box thinker, and a love of challenges. I have a well respected reputation
in the community and have many networking opportunities for outreach
opportunities with both patient(s) and health care personal. I work well with others
however I can work efficiently on my own. I encompass an internal self motivated
drive to learn. My employment goal is a location where I can utilize and nurture my
knowledge base, while performing my best to my employer to innovate and improve
the patients' treatment regimen thus leading the change in the health care industry
for the better.

Promoted to Clinic Coordinator I & II- managed and audited clinic, HEDIS score
management, safety monthly audits, inventory supply management, highest
achieved patient satisfaction scores. Communicated with Walgreens management
and drove marketing locally to rapidly boosting this clinic to second largest volume in
our Louisville Metro area. Provider roles included episodic care in a retail ambulatory
setting. Providing holistic health care to individuals greater than 18 months of age.
Delivering nursing and medical services while diagnosing and managing acute
episodic illnesses: includes ordering, conducting and interpreting diagnostic and
laboratory tests and prescription of medications and therapies. Educating and
emphasizing on health promotion and disease prevention. Electronic medical records
