1. Midterm Evaluation for
Jeremiah L. Mitchell
Evaluation for:
Jeremiah Mitchell
2. In order to receive academic credit for an internship experience, a student must complete 135 hours of on site work. Students must log the number of hours worked. The log
must be approved by the internship site supervisor. At this point in time, is the student on target to fulfill the 135 hour requirement to receive academic credit for the experience?
Yes.
3. Attitude toward work
5
4. Comments
Jeremiah is enthusiastic about each project that has been assigned. During a planning meeting for an event--he did research and was prepared to offer suggestions to the
committee. He has designed and completed a directory of our leadership team with little direction. He researched the members, designed the brochure and immediately
accepted the feedback and made changes appropriately.
5. Attitude toward superiors
5
6. Comments
Jeremiah has his own office and has specifically introduced himself to various staff that he encounters. He listens carefully in meetings and offers suggestions and input at
appropriate times.
7. Attitude toward peers
5
8. Comments
Jeremiah has fit in to the culture and environment since arriving his first day.
9. Quality of work accomplished
4
10. Comments
His designs and research have been excellent. I have provided feedback on the importance of proof reading everything that is generated with his signature. There have been
minor spelling and grammitical errors--we have discussed ways to slow down and review.
11. Quantity of work accomplished

12. Comments
Every project he has worked on has been immediately started, drafts shared and suggestions incorporated. He has been doing a variety of projects and has kept up to date on
all of them. He sent me a draft late at night for the brochure project because he had design ideas and wanted to get them to me.
13. Ability to learn on the job
5
14. Comments
Jeremiah is bright, asks appropriate questions and has picked up on the importance of project management to a learning professional. As a learning professional, I am a
"learning junkie." I have certainly witnessed this with Jeremiah as well.
15. Ability to get along with others
5
16. Comments
As stated earlier--he introduces himself to other employees, fits right into the committee that I am currently running and is enjoyable to have as a co-worker.
17. Ability to communicate
4
18. Comments
Jeremiah can speak very quietly at times. Many of my meetings are held via conference call or in small groups. Participants were having difficulty hearing what he had to
contribute. At the end of the first week I provided feedback about the importance of being heard and that I noticed people do have to ask him to repeat himself. He has taken
that feedback and is working to be louder on the conference calls and during meetings.
19. Ability to think and work independently
5
20. Comments
I was in NJ for 3/5 days last week. We talked about what assignments needed to be done and I set up time for him to spend in other areas of the company. He kept in touch
with me via email and phone calls, reported back what he had learned and accomplished everything we talked about.
21. Willingness to learn
5
22. Comments
He is a sponge--as any good learning professional should be. Because he picked up on the project management aspect of the job--he has asked questions about how to
manage, what type of tracking etc.
2. 23. Dependability
5
24. Comments
Jeremiah has shown up on time each day and communicated to me if he would be a few minutes late. He has been extremely reliable.
25. Conformance to organizational policy
5
26. Comments
I am not sure what you are looking for. If you are referring to dress code and appropriate behavior in an office environment--he is top notch.
27. Maturity
5
28. Comments
Jeremiah has worked hard to earn his degree, manages a family, is working a job after theinternship and is doing a great job during his internship. He would not be able to
manage all that is on his plate without being a mature employee.
29. Confidence in ability to perform job
5
30. Comments
Jeremiah works with little direction and supervision, takes feedback very well and completes everything asked of him.
31. Appropriateness of appearance
5
32. Comments
Dresses appropriate for our office.
33. What are the intern's strengths? Areas to work on in the future?
Strengths - willingness to learn, willingness to accept feedback, technical abilities, and strength of character.
Areas to work on - he should continue to work on asserting his voice--so that his good ideas and comments are heard, he should continue to work on proof reading skills--we all
can use more of that!
34. Overall rating
4
35. Based on the above evaluation, this intern...
IS welcome to return in the future.
36. Would you be willing to have other HU interns work with you in the future?
Yes
37. Additional Comments/Suggestions
This has been one of the busiest months I have experienced--Jeremiah has not "gotten in the way" he has enabled me to accomplish some of the tasks that I would have
struggled to fit in. This experience helps me to be a better learning professional when I have to step back and make sure that I am doing what I need to do--and also provide a
good learning experience for another professional in my field. He is well trained and I would definitely recommend him for employment.
