10. Incidents, accidents and hazard reports
When an accident or near-miss incident occurs, the first priorities are:
1. To make the area safe, prevent escalation and further injury or damage
2. To render assistance to injured parties, obtain help from trained personnel
(SRT First Aiders, Ambulance, Medical Practitioners etc.)
3. To preserve relevant evidence for any investigation
4. To report the incident in accordance with established procedures.
All staff, students, contractors or visitors must immediately report all
accidents and near-miss incidents at TAFE or Workplace to their Lecturer or
Manager.
All reports must be made by the lecturer on the online
OSH Management System (MYOSH)

Insurance and Ambulance Cover:
SRT does not accept financial liability for students who have suffered an
injury, except where legal liability arising from defects in our buildings,
equipment, or grounds, or from negligence on the part of the SRT or a
staff member has been proved.
Students should be encouraged to take out their own private accident
insurance policy.
If a student is involved in any accident and an injury or illness is incurred,
any claim for medical or other benefits must be made through their
private medical insurer before submitting claims to the SRT all such claims
should be directed through the OSH Consultant
If a student has been assessed as requiring an ambulance but does not
have ambulance cover, the SRT will cover the cost of the ambulance.

learning environment while attending South Regional TAFE
18. Day One: Rights & Responsibilities
Each student is responsible for:
Taking all action to avoid, eliminate or minimise hazards
Report hazards, and near miss, illnesses and injuries immediately
Use of all safety devices and wear PPE
Not place anyone at risk
Seek OSH information or advice before carrying out new or unfamiliar work
Being familiar with emergency and first aid procedures
Complying with legislation
19. Day One: Attendance
• If you cannot attend classes, we expect you to notify your
lecturer.
• A Medical Certificate should be provided if you are unwell
for a required assessment.
• Attendance is recorded in every class.
• If you are under the age of 18 and you are absent from a
scheduled class, your parent/guardian will be notified.
20. Day One: Duty of Care
Under 18’s
• Your absence will be reported to your parent or guardian.
• If you are an apprentice, your absence will be reported to your
employer.
• A lecturer must obtain permission from your parent or guardian
for you to participate in any off campus excursions.

• Alternative formats for texts and other learning materials
• Alternative assessments and adjustments such as extra time, note-taker
support or oral assessment

22. Day One: Counselling Services
• A free student counselling service is available to enrolled students
• In person or on the phone
• The service operates between 8:30am to 3:30pm on Tuesday, Wednesday
& Thursday each week that classes are scheduled.

Twenty scholarships of $500 each for students studying full time at South Regional TAFE’s
Albany, Katanning or Denmark campus.
Western Power Scholarship
Up to 3 scholarships students studying the Certificate II in Electrotechnology at the Albany
campus.
Don McLeish Memorial Scholarships - Sponsored by Albany Halfway House
Incorporated
A scholarship to the value of $1000, for a student studying the Certificate IV in Mental
Health, full time at the Albany campus.
26. Day One: PPE
Personal Protective Equipment
Wear PPE and work clothing as required by the PPE Guidance Note
Comply with site-specific PPE requirements of your work area
Comply with PPE as identified within relevant work instructions,
Safe Job Planning documentation, Safety Data Sheets or other documentation
27. Day One: Plant, Tools and Equipment
Students are not allowed operate or use plant, tools or equipment,
until they have been appropriately trained and authorized to do so
by your lecturer.
28. Day One: Laboratory, Studio & Workshop Rules
Workshop rules
• Student affected by drugs or alcohol are not permitted in the workshop;
• Students with any health problems that may affect workplace safety (e.g. medication, epileptic fits) must report these
conditions to their lecturer;
• No food or drink is allowed in the workshop;
• Wear the correct protective clothing for the workshop and additional protective equipment for identified pieces of
equipment;
• All chemicals (e.g. glues and paints) must be checked through Chemwatch and with workshop staff before use
• Immediately notify the lecturer of any faulty or broken equipment;
• Seek information or advice from your lecturer where necessary before carrying out new or unfamiliar work;
• Make sure your work piece is fixed securely before work commences,
• Keep leads up off the floor,
• Keep clear of any person operating tools and machinery (bumping an operator or get tangled in the lead could cause
serious injury to you or the operator)
• Do not talk to anyone operating electrical equipment and machinery
• Keep your work area tidy
• Clean up any spills immediately
• Wash hands after using equipment and materials
• Report all accidents, hazards and 'near-miss' incidents that occur to your lecturer
• Making proper use of all safety devices and personal protective equipment

Evacuation procedure: Staff will advise their students of the assembly point and the route to be taken to this area. There are Fire Wardens in each building who will assist with evacuation. You should remain in your class groups to assist the identification of all people on campus. You must not re-enter a building until instructed to do so by the chief fire warden. Your lecturer will be the last to leave the room and will follow your group to the evacuation area. Go to the nearest fire exit in an orderly fashion. Do not rush. Follow any instructions issued by fire wardens and close any doors on the way out. Be prepared to assist people with a disability, people who are injured or the elderly. When outside, go to the nominated assembly area and remain until the all-clear signal is given. Do not leave the campus grounds or move vehicles until the all-clear is given. After evacuation you will be formed into a group, the roll checked and medical assistance given if necessary. You must stay with your group until notified otherwise.
Emergency and evacuation procedures In the case of fire, bomb threats, earthquakes, floods, gas leaks or chemical spills. expect one of the following types of signal: A series of intermittent beeps followed by verbal instructions A whooping tone – the evacuation signal – followed by verbal instructions A continuous whooping tone – evacuate immediately – no verbal instructions. If you have a disability or medical condition and want to develop a personal emergency evacuation plan, talk to your lecturer.
If you hear the evacuation alarm or you are directed to evacuate by an emergency Fire Warden or emergency services personnel, you must do so promptly and go to the assembly area until the all clear is given.

All campuses own a defibrillator and oxygen bottle for use in the event of sudden cardiac arrest. They are located at Main Reception
When an employee, student or contractor sustains an injury which is a minor first aid injury or provides first aid for a discount injury (an injury requiring first aid that was not sustained at a campus then the employee or supervisor shall:
Ensure that any injury is promptly attended to and appropriate first aid is rendered
Complete either the:
Register of Injury Form within a first aid kit or;
Complete a MYOSH First Aid or Discount Injury Report.
Immediate Supervisors or Managers need to review these forms before signing off and either:
Refer to an OSH Rep for further investigation
Sign off the report.
WHERE INJURIES REQUIRE MEDICAL TREATMENT, THE FOLLOWING ACTIONS MUST BE TAKEN:
Ensure that any injury is promptly attended to by a First Aider and then referred to the appropriate medical practitioner;
Notify the immediate supervisor or manager;
If a serious personal injury or a loss of life occurs OSH Consultants and Human Resources must be immediately notified;
The parent or guardian or employer of a student should be notified of the accident as soon as possible;
Complete the MYOSH Injury Report Form. The manager can arrange for the completion of the report, as soon as possible, if the injured party is unable to do so.
Make sure they are aware of all warning signs & their meanings
Restricted Access:There are some restricted access areas on each of the campuses, for example:
high voltage switch rooms, plant rooms, most laboratories, workshops or roof areas.
Electrical equipment:
are inspected and tagged. This ensures that the non-compliant electrical equipment does not enter into service.
Inspect the equipment before you use it. If the equipment is overdue for inspection or damaged, it is the user's responsibility to ensure the equipment are reported to your lecturer, tagged out of service.
Privately owned electrical equipment to SRT premises must have proof of testing.
Students can access multiple device chargers in the library.
Visitors:
If you have visitors they must report and sign in at main reception – as per signs on campus
Parking/cars:
The speed limit on all campuses is 10 km/h. Please ensure your vehicle is securely locked and parked in a designated parking bay. Penalties and fines apply
Smoking:

A Medical Certificate should be provided if you are unwell for a required assessment. We recommend you discuss with your lecturer about rescheduling an alternative date for the assessment. For students under the age of 18 the South Regional TAFE has an extra duty of care which we explain in our Duty of Care Policy for Minors policy. This duty of care will be explained with you and your parent or guardian at the time of enrolment. Key points to remember are: • your absence will be reported to your parent or guardian. • if you are an apprentice, your absence will be reported to your employer. • a lecturer must obtain permission from your parent or guardian for you to participate in any off campus excursions. • if sickness develops during the day you may ask your lecturer to leave class early to go home in which case the South Regional TAFE will provide a quiet waiting area while we contact your parent or guardian to collect you from the campus.

Counselling service:
A free student counselling service is available to enrolled students across all 12 campuses who are experiencing any difficulties that may affect their training. A counsellor will meet with students in person or on the phone, and can also make referrals to community and health support groups and organisations. There are two counsellors, who are each based in Albany and Bunbury. The service operates between 8:30am to 3:30pm on Tuesday, Wednesday & Thursday each week that classes are scheduled.

Guidance Note: Personal Protective Equipment and Clothing, on COVEO
Students will be required to come to TAFE equipped with similar personal protective equipment as that worn by staff.
Lecturers:
Ensure students are informed of PPE and work clothing requirements for the work area.
Ensure the use of identified PPE by students
Report non-compliance with PPE requirements to Manager.
Ensure employees, students and contractors dress in accordance with site requirements.
Ensure students are trained in the use and maintenance of PPE.
Students are not allowed operate or use plant, tools or equipment, until they have been appropriately trained and authorized to do so by your lecturer.
All equipment at SRT have a Standard Operating Procedure (SOP) on or near the equipment. Before you are authorised to use plant tools and equipment your lecturer will review this process with you to ensure you understand the specific safety requirements.
JSA –Job Safety Analysis will be undertaken for all hazardous tasks at SRT to ensure all students are aware of the hazards and controls in hazardous tasks.
Read the safety rules carefully to ensure students understand the requirements before entering.
If possible, give as a hand out.
