- Khảo sát trào ngược + đè xẹp: gần SFJ, giữa đùi, khoeo. Nếu 2 trong 3 vị trí
cho kết quả khác nhau: khảo sát thêm 1 vị trí nữa ở giữa 2 điểm đó. Hoặc
khảo sát mỗi 3-5 cm. Sử dụng Doppler màu + xung, mặt cắt dọc (hoặc ngang)
● Tại chỗ nối, ± giữa đùi: nghiệm pháp Valsalva

The GSV drains into the common femoral vein approximately 2.5 cm below the inguinal ligament at the SFJ. It is important to have a detailed understanding of the anatomy in this area, as there are at least six other tributaries draining to the GSV at the level of the SFJ. These tributaries can be the source of primary or recurrent varicose veins. It is not usually possible to identify all of these tributaries by ultrasound.

Typically the saphenopopliteal junction is located 2-5 cm above the popliteal knee crease. However, the anatomy can be extremely variable. The saphenopopliteal junction may be located proximal to the popliteal fossa, draining to the above-knee popliteal vein or distal femoral vein. The SSV can insert directly into the gastrocnemius vein or share a common confluence.

Re-entry occurs through perforating veins or healthy saphenous veins. They may become secondarily dilated because they carry the same extra volume as the dilated superficial veins which are pathologically filled. This may lead to secondary incompetence.
The probe may slip from the groin while the patient performs the Valsalva manoeuvre. The absence of a flow signal may mean either that the valve is competent or that the probe has slipped.

In our experience flow augmentation should be sufficiently strong to produce a transient peak flow velocity of >30 cm/s in the main superficial vein trunks so that valve closure should be rapid on the squeeze release. However, this velocity can be difficult to achieve in very small veins. If an inadequate calf squeeze is performed, flow augmentation may be very poor, and this can be a source of conflicting results among different sonographers.

When a large incompetent vein empties into a small capacitor, peak vein velocity is high, but duration is short. When the refluxing vein is small and the capacitor is large, velocity is low and retrograde flow duration is long….and no definite conclusions can be made.

In transverse section, it is often possible to see the GSV running directly into varicose areas. Large perforators and tributaries are relatively easy to identify.

If the upper and lower lid of the saphenous eye cannot be identified in association with a vein in transverse view, it is not the great saphenous vein. Any tortuous structure visible from the exterior is a tributary. The great saphenous vein is straight and cannot become tortuous within its fascial compartment because it is anchored by the saphenous ligament and surrounding connective tissue. Tortuous and dilated veins are always tributaries. This difference is very important for establishing the Hach Class.

The fascia compartment at the mid-calf is fairly thick, with space on each side of the SSV.
This is a left-sided reflux evaluation. This shows incompetence of the saphenofemoral junction (SFJ) that dumps into the posterior accessory, flowing into the Giacomini vein and the varicosities of the lateral posterior calf. A key point is that the great saphenous vein (GSV) below the posterior accessory is normal, without reflux, and the small saphenous vein (SSV) is normal from the popliteal distally.
A right-sided saphenous incompetence on the depth mapping form. This shows a saphenofemoral junction (SFJ) that is incompetent with gross reflux into the proximal great saphenous vein (GSV). At about zone 3, the reflux leaves the saphenous canal, descending in an epifascial (and axial) GSV tributary. The key here is that the GSV in the sheath (and at a depth of 7 mm) is only about 4 mm without reflux. The epifascial varix maintains a 6–7 mm diameter and is just beneath the skin as far as depth, with 4+ seconds of reflux along the medial thigh. It crosses over the saphenous vein proper below the knee, and feeds the varicose complex of the tibial segment of the leg. Note: The GSV below the knee does not have any reflux.

The anterior accessory saphenous vein (AASV), sometimes called the anterolateral thigh vein, drains flow from the anterior and lateral aspect of the knee and lower thigh and runs across the anterior aspect of the thigh into the SFJ. However, it can sometimes join the GSV at a variable level below the junction. The AASV is usually easy to identify with ultrasound and it is contained within its own fascial compartment. In the upper thigh, it runs in the same line as the femoral vein: this is referred to as the alignment sign and helps to distinguish the AASV from the GSV that lies medial and posterior to the AASV.
