Permission consent form for the participation of ………………………….. to be involved within Grace Anna Elizabeth
Crawford’s Extended Project Qulification.
You are being asked to participatewithin a conducted experiment involving surveillance, determining thequestion of “Can
we be under surveillance without realising?” During the course of this experiment, you shall be watched daily from the
Monday 17th
November 2016-Friday 31st
March 2017, during Manshead school hours of 08:45-6:00pm Monday-Friday. You
have been selected due to thereliability within your job role as full time teacher within the educational department of
Manshead School.
This experiment is being conducted by Yr 13 student Grace Anna Elizabeth Crawford, whom is currently undergoing EPQ at
Manshead School, Dunstable, Bedfordshire LU1 4BB. With the assisted guidance of Jamie Bishop, supervisor of Grace
Anna Elizabeth Crawford for this conducted experiment and area of study.
Study Purpose
The purposeof this study is to test if we are being watched throughout our daily lives without us knowing. How surveillance
has incorporated within our lives that we have learnt to accept this as the norm and have accepted the fact that we our being
watched throughout our normal lives consistently.
The study willinvolve student Grace Anna Elizabeth Crawford, following and documenting percipient around the school
with a digital camera throughout weekdays within school grounds and times of Manshead Upper School. In no way shall
documenting and following be performed outside of specified times and days outside the school grounds nor will personal
photographs and information of the percipient be shown to others outside of this experiment for the privacy and safety of the
said percipient.
You shall be unaware of this experiment being conducted throughout these times. If participatenotices themselves being
watched than it determines the question of we can’t be surveyed without us knowing, however if participatefails to realise
what is happening than it outlines thequestion that the government can survey us without us realising throughout our lives.
Study Procedures
If you agree to participatewithin this study regarding surveillance, you are aware that:
You shall unknowingly be followed at times throughout the school day starting from Monday 17th November 2016-Friday
31st March 2017 during thetimes of 08:45-6:00 on Mondays-Fridays weekly within theschool term.
Your daily activities shall be documented unknowingly and stored upon an SD card that you can have direct access to upon
request if needed.
The documented photographs of your activities throughout sixth form over the5 month period will be gathered and
transformed into an artefact for pupilGrace Anna Elizabeth Crawford to present her results upon and explain how this
conducted experiment regarding participant benefited her within her study of surveillance.
Be discussed about throughout thefinal presentation and the participant’s involvement throughout this experiment over the
course of the five month period and how this has transformed into an artefact to aid within Grace Anna Elizabeth’s Crawford
study of focus.
You should understand the importance of prompt attendancethroughout the5 month period and how it’s vital for participant
to be in school regularly to allow Grace Anna Elizabeth Crawford to document daily for the purposeof her artefact and
experiment into surveillance.

The decision to allow you to become involved within Grace Crawford’s work will not affect
Any information that is obtained within this study and the purposeof this Extended Project Qualification will remain
confidential and will be disclosed only with your permission.
Your decision to allow yourself to become participated within this experiment will not affect your present or future
relationships with further parties such as AQA.
If you have any questions or concerns about theentirety of your involvement within this study, call Supervisor Manager
Jamie Bishop at Manshead School.
You may keep a copy of this consent form.
You are making the decision about allowing your life within Manshead School to become participated within this conducted
experiment regarding surveillance. The experiment will last from 08:45-6:00pm Monday- Friday
I confirm that I have read and understood the information sheet for the above study regarding surveillance within school
hours and have theopportunity to ask questions at any time regarding this experiment.
I understand that my participation is voluntary and that I am free to withdrawal at any time, without giving a reason.
You have my confirmation in agreeing to take part within this conducted experiment involving surveillance and how I shall
be watched throughout school periods, Monday-Friday duringthe following hours of 08:45-6:00.
