Trauma

1. Respirator Care in burns:
• Assess airway, Respiration and
breath sounds.
• Removal of pulmonary secretions.
•Chest physiotherapy, deep breathing
and coughing.
• Frequent position changes and
suction.

2. Pharmacologic considerations:
• Bronchodilators and mucolytics.
• Circumferential chest burns can
impair ventilation so in this case
escharotomy may be required.

3. Cardiovascular Care:
• Increase capillary permeability
• Capillary Leakage Syndrome
• Fluid shift from intravascular to
interstitial space as a result blisters
and massive edema will occur.
• Excessive insensible loss through
burn wound is 3 - 5 lit/d.

4. • Finally hypovolemia if untreated
will cause Burn Shock.

5. Severity of burn
injuries:
• We will treat the burn according
to severity of injury.
• Severity is determined by:
• Depth of burns
• TBSA involved

6. • Site- face, hand, feet, face, or
perinium.
• Age.

7. Prognosis of burn( Baux Score):
Sum of age in years
+
Area of burn in %TBSA.
< 80 good
80 - 100 life threatening
> 100 bad

8. Revised Baux score:
Formula: TBSA + Age + [17×R]
R=1 if patient has inhalation
injury.
R=0 if not.

9. Resuscitation Phase of burn:
• First 24- 48 hrs after initial burn
injury or untill spotananeous
diuresis occurs.
Resuscitation phase characterized
by:
• Life threatening airway
problems.
• Cardiopulmonary instability

10. • Hypovolemia
Goal of Resuscitation:
• Maintain vitag organ function and
perfusion.
