INTRODUCTION - Anemia's due to Excessive Red Cell Destruction

2. Hemolytic anemia
Definition: The premature
destruction of red cells before 120
days of life in the circulation.
The Result:
free toxic iron in the plasma (Fe2)
and called plasma hemoglobin.
It is toxic
And Must be removed

How removed the toxic iron from the plasma
•Three proteins remove its from the plasma
•They produced from the liver

Proteins remove the Free Iron
1-Haptoglobin (Hp) –
first remove the toxic
iron
2-Hemopexin– second
protein remove the
toxic iron when the Hp.
depleted.
3-Albumin– binding
iron only to form
methemalbumin and not
remove iron.
The excess removed by the kidneys, reabsorbed, and the excess excreted by
urine to form “Hemosiderinuria) as indication of sever hemolysis

6. This takes place Extravascularly
in the macrophages of the
reticuloendothelial system.
Normal red blood cell (RBC) breakdown.

1-Extravascular
haemolysis:
is the route by which red
cells are normally broken
down and occurs in the
macrophages of the
spleen, bone marrow and
liver.

2-Intravascular haemolysis:
• is the destruction of red cells within the
circulation.
• It is characterized by all of the features
of extravascular haemolysis

9. Plasma Hemoglobin
•It is the Hb which existing normally after RBC death ,
•Normal plasma Hb value is 0.5 to 5.0mg/dl
The finding of free Hb in plasma is highly suggestive of intravascular
haemolysis.
•Increase after sever hemolysis

10. Tests exclusive for intravascular hemolysis
1-Plasma hemoglobin * Free Hb Increased
2-Methemalbumin Positive
3-Hemosedrinuria Positive in severe hemolytic anemia
4-Haemoglobumria Positive
5-LDH Raised of LDH …. 90 % of hemolytic
cases
6- Haptoglobin protein Depleted

Sign of sever intravascular haemolysis.
