It is a hormone produced by the pancreas. This
hormone’s function is to help the body utilize the
sugar in the blood. Sugar in the blood comes from the
digestion of starchy food, fruits and sweets and from
its storage in the liver; the insulin’s role is to enable
the sugar to enter the cells and be used for energy.
Insulin is a protein that can be easily digested in the
intestinal track; therefore the only way to take insulin
is by injection.
Types of insulin
Every type of insulin is basically the insulin hormone
manipulated chemically to vary the length of time
it is available in the body.
We have:
• Rapid acting insulin
• Regular or short acting insulin
• Intermediate acting insulin
• Long acting insulin
• Pre-mixed insulin
Each of the above types has a specific onset, peak
time and duration of action.
For example the very short or rapid acting insulin
starts its action within 5 to 15 minutes after it is
injected and works at its maximum capacity in one
hour after its injection and is kept active in your body
for 2 to 4 hours. This will affect the timing of your
meals. Ask your diabetes educator about it.

• It is injected under the skin usually but can be
given intravenously in an emergency.
• Sites of injection can be in the arms, thighs,
buttocks and abdomen. The abdomen is the
most ideal site of injection because of the quick
absorption of insulin.
• Rotation of the injection site is necessary to lower
the possibility of the negative side effect of insulin
on the tissues at the site of the repeated injection.
• Many factors affect the insulin absorption such
as the location of the site, the depth of injection,
physical activity especially if it involves the
muscle in which it was injected and the
temperature of the site of injection.
Storage and Safety
• Insulin can be stored in the refrigerator (2-8
degrees) and can also be stored at a controlled
room temperature (25 degrees), away from
heat or direct sunlight for a maximum of 28 days
once opened. Keep in mind cold insulin hurts
upon injection, so it needs to be warmed
between hands.
• Insulin should not be frozen.
• Do not keep insulin in the car, glove compartment
or on top of the TV or anywhere that it can
get exposed to extreme temperatures or
constant vibrations.
• Wash hands thoroughly with soap and water.
• Roll the insulin bottle between your hands.
• Clean the rubber cover with an alcohol swab,
wait for the alcohol to dry.
• Pull the plunger of the syringe to the dose
required to fill it with air then inject the air
into the vial.
• Clean the area of injection with soap and water
if needed then inject under the skin with 45-90
degree angle, depending on the thickness of the
skin and the area injected.
Patient’s advice
• In mixing insulin in the same needle, pull the clear
insulin type first.
• Syringes are a one time use only, do not re-use.
• Check the insulin every time for signs of
deterioration. Check expiration date of the
unopened vial.
• Do not change insulin type without your
doctor’s advise.
• Insulin Glargine and insulin Detemir can not be
mixed with any other insulin in the same syringe.
• Dispose of the syringes appropriately.
• Keep out of the reach of children.
