Definition
Urticaria is a group of diseases characterized
by the development of wheals (hives),
angioedema, or both. Urticaria needs to be
differentiated from other medical conditions
where wheals, angioedema, or both can occur,
e.g. anaphylaxis, autoinflammatory syndromes,
urticarial vasculitis, or bradykinin-mediated
angioedema including hereditary angioedema
(HAE).

3. L’Orticaria è caratterizzata dalla

4. Angioedema in urticaria patients

1. a sudden, pronounced
erythematous or skin colored
swelling of the lower dermis and
subcutis or mucous membranes,
2. sometimes pain, rather than itch.
3. a resolution slower than that of

Histamine fish poisoning, also known as scombroid
poisoning, is the most common cause of
ichythyotoxicosis worldwide and results from the

Spontaneous Urticaria
Sebbene un dettagliato esame obiettivo sia necessario, la
eziologia della orticaria acuta spontanea può essere
suggerita da una anamnesi approfondita. Le infezioni delle
alte vie respiratorie e nel infezioni virali in generale sono la
causa eziologica più comune in età pediatrica. Le allergie
alimentari o le allergie ai farmaci sono una causa sia
nell’adulto che nel bambino (NDT: Molto meno frequente!).
In generale un work up diagnostico è indicato solo quando
una allergia di tipo I è sospettata esserne la causa.

emergency room between January 2006 and December 2007 with a
diagnosis of acute urticaria, isolated or associated with other clinical
symptoms.
Results: Only 2.0% of the cases studied were associated with
severe clinical pictures. In 437 cases (53.7%), the cause of urticaria
was not determined. The infections of the respiratory tract were
the most frequently suspected aetiological factor. The diagnosis
of allergic urticaria is more defined, but belongs to a minority
group (10.8%).

Different studies reported a rate ranging from 37% to 58% of
multiple infections among 54 or 79 patients with acute urticaria,
respectively [2,3]. Another study highlighted the role of
Mycoplasma pneumoniae infection, which occurred in 32% of 65
children with acute urticaria [4].
Other reports focused on an association between acute urticaria and
streptococcal infection, hepatitis A and B viruses, parvovirus
B19, cytomegalovirus (CMV), coxsackie A9 virus, enterovirus,
influenza A and parainfluenza viruses [5-10]. Acute urticaria has
also been observed after influenza vaccination [11].

4.1 | Diagnostic work up in Acute Urticaria
Acute urticaria usually does not require a diagnostic workup, as
it is usually self-limiting. The only exception is the suspicion of
acute urticaria due to a type I food allergy in sensitized patients
or the existence of other eliciting factors such as nonsteroidal
anti-inflammatory drugs (NSAIDs). In this case, allergy tests as
well as educating the patients may be useful to allow patients to
avoid re-exposure to relevant causative factors.

years of age), worsen around puberty, and persist throughout life,
with unpredictable severity.
Attacks of hereditary angioedema usually follow a predictable
course. Many attacks are preceded by a prodrome (usually a
tingling sensation), and approximately a third are accompanied
by erythema marginatum, a nonpruritic, serpiginous rash. The
swelling classically worsens slowly but relentlessly over the first 24
hours, then gradually subsides over the subsequent 48 to 72 hours.
The arms, legs, hands,feet, and abdomen are the most common sites
of swelling… Hereditary angioedema affecting the abdomen or
oropharynx can be associated with significant risk of illness and
death.

32. The diagnostic work up of CSU
in children has the same aims as in adults:
1) Differential diagnoses should be excluded with a special focus on
Cryopyrin-associated periodic syndrome (CAPS). CAPS is a rare
disease with a urticaria-like rash that manifests in childhood
2) If possible, i.e. depending on the age of the child, disease activity,
impact and control should be assessed using assessment tools similar
to those used in adults, although it has to be noted that no validated
disease specific tools for children are available as of now.
3) Triggers of exacerbation should be identified and, where indicated,
underlying causes, which appear to be similar to those in adults,
should be searched for. In children with CIndU, similar tests for
provocation and the determination of trigger thresholds should be
performed

High ambient temperatures, such as hot baths or showers,
hot summer days or saunas, or sleeping in a warm room with
too many blankets, may bring on cholinergic urticaria.
Strenuous exertion resulting in elevation of core body
temperature will also give rise to cholinergic urticaria, but
exertion in a warm environment need not necessarily be very
strenuous in order to bring on a reaction. For example, a
casual walk on a hot summer day may be sufficient to raise
the core body temperature to bring on symptoms. Patients
with cholinergic urticaria may note that strong emotional
reactions, such as stress, anxiety, or embarrassment, will
bring on reactions

Basic considerations
1. The goal of treatment is to treat the disease until it is gone.
2. The therapeutic approach to CU can involve
a. the identification and elimination of underlying causes,
b. the avoidance of eliciting factors,
c. tolerance induction, and/or
d. the use of pharmacological treatment to prevent mast cell
mediator release and/or the effects of mast cell mediators
3. Treatment should follow the basic principles of treating as

Omalizumab (anti-IgE) has been shown to be very effective and safe
in the treatment of CSU. Omalizumab has also been reported to be
effective in CIndU, including cholinergic urticaria, cold urticaria,

as delayed pressure urticaria. In CSU, omalizumab prevents
angioedema development, markedly improves quality of life, is
suitable for long-term treatment, and effectively treats relapse after
discontinuation. Omalizumab, in CU, is effective at doses from 150
– 300 mg per month. Dosing is independent of total serum IgE . The
recommended dose in CSU is 300 mg every four weeks.
