families in an individualized program to determine goals and create programs of activity using the AHA
Family Wealth Plan instrument.

• Maintain confidential files of records, activities and accomplishments of each family member. Reports are
updated per contact for the inclusion into the data bank of the program. Prepares reports for and attends

• Provides guidance to Family Support Assistants assigned to their cases. Participate in the training, working
sessions and internal staff meetings to provide support for families. Perform other duties as assigned by
supervisor or other Integral designees.
• Develop working partnerships with service providers, and other agencies in the wider community as may be
needed to ensure resources for enrolled families.
• Work with the AHA relocation staff, private management staff, and Housing Choice to provide a continuity
of support for families.
