  • 1. 2013 CDA Dagupan Extension Office Annual Narrative Report For the year 2013, there were four Major Final Output (MFOs) consisted of the following: a) Efficient Registration of Cooperatives and Amendments to Articles of Cooperation and By-Laws; b) Effective Enforcement & Compliance to Rules and Regulations; c) Effective Support Services for Cooperatives; d) Efficient, Effective and Responsive Regulatory and Developmental Agency for Cooperatives. The programs, projects, activities conducted to achieve Efficient Registration of Cooperatives and Amendments to Articles of Cooperation and By-Laws (MFO 1) were as follows: Registration of Cooperative, Membership Expansion, Establishment of Cooperative Branches/Satellites, Formation of Laboratory Cooperatives, Advocacy for Mergers and Consolidation, Encoding of CAPRs/AFS/Performance Report/Social Audit Report, Monitoring of Compliance of Cooperatives to Administrative Requirements, Evaluation of Financial and Institutional Records of Cooperatives, Records Keeping and Archiving of Cooperative's Records, Posting of Official List of Cooperatives. Table 1 presents the performance of CDA – DEO per program, projects and activities as well as the overall performance of MFO1. Promotion on Merger and Consolidation of cooperative banks and b. Monitoring of the Divestment Plan of CDA, Implement the Cooperative Marketing System for Sustainable Agriculture thru On-line Market Place for Coops, Implement Programs to Strengthen Housing, Water Service Cooperative, Implement Programs to strengthen coops of the elderly, youth and people with special needs, Implement Programs to Strengthen Transport Cooperatives, Strengthening of Federation and Unions and Coordinate with other Government. Agencies, NGOs, Foreign Institutions for Financial and Other Forms of Assistance to Promote the Viability of Cooperatives and Sound Management of Loan Receivables and Unliquidated Grants and Subsidies, Public Information Services and Coordination and/or Conduct of Summits, Congresses, Trade Fairs, Anniversaries, etc. Table 3 shows the 2013 Annual Performance on MFO4 of the CDA – DEO. The ISO 9001:2008 was awarded to CDA – DEO on July 22, 2013 at Quezon City. This is a manifestation that CDADEO is competent in terms of being able to implement international standards in the area of registration processes. The certification was not easy. It started from a dedicated and committed leadership, the cooperation and sacrifices of the staff, the commitment of resources for the quality workplace physical arrangement and the mentors from Central Office headed by Atty. Niel A. Santillan, Chief Leighlany Cariaso and Dir Marieta Hwang and their staff for their support and
  • 5. encouragement and to Chairman Emmanuel M. Santiaguel and the Board of Administrators for their motivation and inspirational guidance. The Civil Service Commission conducted a Customer Satisfaction Survey on an Anti Red Tape Act and based from the findings, the CDA-DEO clients rated CDA-DEO with a final numerical rating of 87.94 with final descriptive rating of “GOOD”. In summary, the CDA-DEO has an overall performance of 107.58 % Accomplishment for the year 2013. The Authority excelled in the following: ISO Certification, Efficient, Effective and Responsive Regulatory and Developmental; Effective Enforcement & Compliance to Rules and Regulations. Figure 1: Performance on the Various MFOs and the Overall Percentage Rating The remaining indicators were rated “Good”, however, there still room for improvement in the following MFOs namely: Effective Support Services for Cooperatives; Efficient Registration of Cooperatives and Amendments to Articles of Cooperation and By-Laws; and Efficient, Effective and Responsive Regulatory and Developmental Agency for Cooperatives. Figure 1 presents the overall performance of CDA-DEO for the FY 2013. Qualitatively speaking the descriptive equivalent for the CDA-DEO 2013 performance is shown in Figure 2 where 3 MFO: Effective Support Services for Cooperatives; Efficient Registration of Cooperatives and Amendments to Articles of Cooperation and By-Laws; and

Efficient, Effective and Responsive Regulatory and Developmental Agency for Cooperatives were rated “GOOD”; whereas 2 MFOs such as: Efficient, Effective and Responsive Regulatory and Developmental; Effective Enforcement & Compliance to Rules and Regulations were rated “BETTER” and the ISO certification was rated “BEST”.
