(Include date and time of day)

5. Where did it occur?
(Be specific)

6. What was the conduct
(Be specific on the details of what
actually happened)

7. Write down the names
of all involved
(Include all witness names)

8. Indicate type of
interaction
(Phone, email, in person, social media, etc)

9. How did the incident
make you feel or
affect you?
(Scared, nervous, stressed, threatened, etc)

10. What was your response
and reaction to the
harasser
(Ignored, got angry, sent an email back, etc)

11. Make and retain a copy
of your notes for your
record
(Make sure to sign and date your
documentation)

12. When you are ready,
issue a formal
complaint in writing,
signed and dated
Depending on the situation and your
organization, you can provide your
documentation to management, HR or lawyer
