What is the NHS Transformathon?
• It’s a free, 24 hour, online event/conference.
• It’s a live event.
• It will feature change experts, health and care staff, patients
and service users.
• It will feature people from across the world.
• It will feature a mixture of speakers, films, question and
answer sessions, debates and discussions.
• It will be on a website that most people can access via their
computer or mobile, and we will be using YouTube and
Google hangouts.

• The talks have international reach, speakers and attendees

• Topics featured have been from patients stories to ‘Rebels at Work’ to

8. Change Model refresh
The NHS Change Model has been around for a couple of years and the
time has come to review and refresh the model.
On 14 October, Zoe Lord (improvement manager in Horizons) is leading an
innovative hack event – where around 80 people, from within the NHS,
social care and other sectors, will be coming together to help shape the
refresh.
A survey was also produced to help gather feedback from across the
system, and will be fed into the process of refreshing the model.

11. The Directory
The Horizons team leads and coordinates a virtual team of representatives from across
the NHS, health, social care and other sectors.
An outcome of this work has involved developing a directory of organisations, groups
and teams that support innovation, improvement, leadership development and systems
leadership, and how they could potentially support new care models and other priorities
in the Five Year Forward View. It aims to signpost those vanguard sites involved in
new care model development and anyone else interested in innovation and
improvement.
Also as part of this work the horizons team run monthly
Leadership development and improvement community
webexes to join together national and regional organisations
involved in improvement and innovation in a collaborative
dialogue on currents themes and topics.
