• PROCEDURES BEFORE CHECKING OF FORMS
• At the Division Level (Composition of the Division Checking Committee
(DCC)
To facilitate the timely, consistent, and quality checking of school forms,
Division Checking Committee which shall be composed of
Chair : Chief of Curriculum Implementation Division
Vice Chairs: Chief of school Governance and Operations Division (SGOD
Division Planning Officer ( SGOD)
Members: Other Division or School Personnel to be identified by the chair.

Roles and Functions of the DCC
The DCC shall focus on the areas of enrollment eligibility and
assessment which include retention/promotion or acceleration to
ensure that the most recent policies are strictly observed.
The SGOD shall validate the enrolment count, number of school leavers
(dropped out) number of learners who transferred in/out were
promoted/retained and other enrolment-related indicators. The
planning officer or the Senior Education Program Specialist for Planning
and Research will conduct the checking of SF 1 and 5

4. • There will be no checking of forms at the District Level. The PSDS
• Shall provide technical assistance to all schools within his her jurisdiction.
Initial Tasks of the Class Adviser
At the beginning of the school year ,the class adviser shall collect
supporting documents to establish the identity of each learner assigned to
his/her advisory class. Supporting documents or references may include but
are not limited to the PSA birth certificate, baptismal certificate or any
equivalent document. If the learner came from another school ,the class
adviser shall coordinate the transfer of the Learner’s Permanent Academic
Record and validate its authenticity. The Guidelines of the transfer of the
learner’s academic records shall be the basis of the adviser for enrolling or
validating the said learner in the LIS.

5. • After encoding all learner information in the LIS, the class adviser can
generate SF 1 using his/her system account. This shall become the
official enrollment list of his/her class and shall be used as reference
in any other reporting that requires the list of his/her system

• The class adviser shall also download SF2 from the LIS This Learner
Daily attendance Report shall be forwarded to the school head for
assessment and consolidation, and to serve as reference for the
consolidated report on Monthly Learner Movement and Attendance
Report.

6. • At the end of school year, once the computation of final rating for
each learning area is done, the class adviser shall transfer these
grades from his/her class record into SF 10. The validated SF 10 will
be the basis for updating each learner’s status. ( promoted or
retained).
• SF 5 and SF 6 or the report on promotion and level of proficiency
respectively, for each class can be generated from the LIS using the
school level access accounts.

7. • The SF 5K shall be used for Kindergarten as validated by the Early
Childhood Care and Development (ECCD). Checklist post test result
and the kindergarten Progress Report. Class advisers in Kindergarten
• Progress Report. Class Advisers in Kindergarten are not required to
prepare SF 10-ES ( formerly Form 137).
• These ( 4)SFs ( SF 1, SF4 -,SF5 and SF6) generated from the LIS shall
be the focus of checking and should be supported by the appropriate
documents.

8. • For graduating/moving-up levels (Kinder, Grade 6, 10 and 12) the
class adviser shall also prepare awards/certificates and should be
checked in all forms for consistency.
COMPOSITION OF THE SCHOOL CHECKING COMMITTEE
A Checking committee shall also be created at the school level with the
school head as the chair. As SCC Chair, the school head shall determine
the membership of the SCC. There will be 2 Vice Chairs at the SCC. The
ICT Coordinator shall serve as the vice chair for enrolment counts and
Learner Profile. The School Head shall also appoint the most capable
school personnel for Curriculum and Assessment.

9. • Schools with more than one or more sections in each grade level may
organize sub committees comprised of the class advisers and other
subject teachers.

thorough review of all learner’s record for all classes in preparation for
the scheduled checking of Forms with the DCC.

All descrepancies found during the checking are reconciled and that
the corresponding adjustments are applied in the LIS by the ICT in
schools.

18. • After the checking of Forms A checking Report or SFCR 1 will be
accomplished.
• Prepare 2 copies, reflecting only the classes that they reviewed. 1
copy for checking and 1 copy for the school.
NOTE : VERY IMPORTANT Please submit the accomplished
SFCR to the in charge group so that the DCC will make a summary to
be submitted to the Schools Division Superintendent (SFCR 2).

RESUMPTION OF IMPLEMENTATION OF THE POLICY ON CHECKING OF
SCHOOL FORMS AS PRESCRIBED IN DEPED ORDER NO. 11,s.2018
GUIDELINES ON THE PREPARATION AND CHECKING OF SCHOOL FORMS
As the Covid – situation in the country continues to get better
resulting to gradual easing of restrictions, The department is resuming
the onsite conduct of the annual checking of school Forms for SY 2022-
2023 with the participation of the Division Checking Committee (DCC)

20. • The checking committees shall perform the physical reporting to
schools and offices. The checking committees shall perform the
physical checking of school Forms with the presence of the Class
Adviser and examine the document ,the LIS data as appropriate. Since
the checking of promotional Forms is on face-to-face SSC and DSC as
prescribed in DO 11 s. 2018. For Grades K, 6, 10 and 12 the venue of
the checking is the CID office and conference hall. There were 4 Zones
created for the checking. before the checking proper, the Adviser or
School Head will proceed to the conference hall for the checking of
the LIS and other forms.

• The department of education recognizes the high value of awards and
recognition and their profound effect on learner’s morale, motivation

32. • FILL out the School Forms Checking Report (SFCR1)At the end of the
checking of Forms, the committees findings shall be consolidated in
an observation report using the SFCR1. The DCC shall prepare an
SFCR1 in 3 copies reflecting only the classes that they reviewed. The
entries in the templates, particularly the level of correctness or
accuracy, shall be discussed by the committee .
• NOTE: That temporarily Enrolled learners example have pending
transfer of SF 10 ( formerly form 137) should not be included in Tables
1 and 2 of the SFCR1.

33. POST CHECKING
• School Level
Findings, observations and recommendations from the DCC recorded in
the SFCR 1 may be included in the agenda for the next school-based school-
based Learning Action Cell ( LAC) session.
The school head, with the technical assistance of the school system
administrator shall ensure that all adjustments made
during the checking are reflected in the LIS.
Within 5 working days after the checking conducted by the DCC, the school
shall submit one duly signed original copy of the SF4, SF 5 and SF 6. No
additional report other than what has been stipulated shall be required from
the school for submission to the DCC.

34. • At the Division Level
• The PSDS shall consolidate the SFCR1 of all schools
• In his/her School District and compute the District Accuracy Level to be
recorded in Table 1 of the Consolidated District School Forms Checking
Report ( SFCR2) Attached as Annex 1b. The SFCR2 must be signed by the
Chair and members of the DCC.
• The sfcr2 FOR ALL Districts shall then be consolidated by the DCC. This
shall be the basis for computation of the Division Accuracy Level to be
recorded in Table 1 of the Consolidated Division School Forms among with
the Findings and Recommendations( Narrative Report) must be submitted
to the office of SDS not later than the second Monday.

learning areas (DO No. 36 s. 2016). The general average grade shall be
reported as a whole number following relevant provisions on Dep Ed
Order No.8,s.2015 policy guidelines on classroom assessment for the
K to 12 Basic Education Program.

42. DEPED REITERATES THE FOLLOWING POLICIES
1. Graduation and Moving-up Ceremonies should be simple yet
meaningful. These should be conducted without excessive spending
extravagant attire, or ordinary venue.
2. Non-Academic projects articulated in DO No. 66,s. 2017 .such as
attendance to field trips, film showing, JS Promenade and other
school events should not be imposed as requirements for
graduation or completion.
3. The format of the completion and diploma must be in accordance
with Enclosure 1 pages 44 to 46 of DO No. 31 s. 2019. entitled The
Department of Education Service Marks and Visual Identity Manual

43. • For public elementary and Secondary Schools
The use of the cited completion certificates and diploma templates
are required,
Moving-up or graduating rites shall be conducted in an appropriate
solemn ceremony befitting the Graduating students in their family and

No Deped personnel shall be allowed to collect any kind of contribution
or graduation/ moving up fee. Activity shall be charge in MOOE.
