1. FO64- Checklist for Non- Negative Samples- NSW Douglas Hanley Moir
□ Consultant and Donor have kept the specimen in view at all times
□ Pour sample from Split Specimen Cup into DHM container with Blue Lid
□ Clearly label, on all three specimen jars- Donor’s name, Date of Birth and Date & Time of Collection.
□ Remove yellow label from blue lid
□ Pierce vacutainer with needle on blue lid to fill vial with specimen.
□ Distribute sample equally into 3 vials- (A, B & C Sample)
□ Offered the donor the opportunity to take the C sample (3

vial). Ensure this offer has been recorded
on F058- Consent to Participate and Permanent Record of D&A Screen-12 Nov 12.doc
□ If donor has requested a portion of the sample provide them with the third vial (C sample) and seal the
same as other vials. Advise donor that the sample must remain sealed or the sample becomes invalid.
If donor does not wish to take third sample, send to lab with Sample A & B/
□ Chain of Custody Form has been completed and signed by both parties.
□ Tamperproof seals that have been initialed by donor and tester have been placed over the jar with the
donor’s details remaining in clear view.
□ Samples have been placed into biohazard bag with Chain of Custody Form and sealed
□ Biohazard bags with specimens has been stored in green transportation bag and kept in a secure place
until all testing has been completed.
□ Chain of Custody Transfer form completed indicating specimens that are sealed in the transport bag
and clearly labeling the Satchel Tag Number. This form is to be attached to outside of the green bag (if
possible) so label with DOB rather than names.
□ The donor has been informed that the sample will be delivered to an Australian Certified laboratory for
GC/MS testing (confirmatory testing).
□ When all samples are in the transportation bag and testing has been completed, the bag is sealed with
the satchel tag.
Complete the Transfer Form and attach this to the plastic sleeve at the back of the satchel
□ Record satchel tag number on F058- Consent to Participate and Permanent Record of D&A Screen-12

o Ensure that the FO59 Notice of a Result that is Not a Negative has been completed and
signed by both the employee and the Supervisor
□ Contact Work Options to arrange for the sample to be picked up and taken to the lab
