Throughout this semester I completed an internship in psychology at Jefferson Center for Mental
Health. Specifically I spent my time in a short term Hospital Alternative Facility (HAF). HAF is a
residential facility that aids in transitioning home from the hospital, another Jefferson Center for
Mental Health facility or a crisis situation. All of our clients have current mental health diagnoses,
the most common of which are depression, anxiety, bipolar disorder, personality disorders, and
schizophrenia. Clients stay for only a short time and the main objectives for their stay normally
revolve around stabilizing meds and finalizing housing and living plans for the future. HAF
provides services that are vital in that clients can have a stable residence so they can focus on
managing symptoms and figuring out the best strategy to integrate themselves, with new goals back
into their life. However, with all of HAF's outstanding and vital qualities, there are also areas that I
would suggest for improvement given what I have learned through my undergraduate education in
psychology. My first critique stems from my exposure to both Psychopharmacology and Clinical

Rosa stated that, "Teamwork and communication is a must when achieving quality care for the

for Rosa consists of admitting patients, taking patients vital signs, drawing blood, bathing and
dressing patients, moving patients from bed to chairs, transporting patients to various departments
for test, and discharging patients. The most stressful days in her line of work arise when other nurses
call out for scheduled work days and miscommunications. When nurses call out from work, the
burden for other nurses becomes overwhelming. The other nurses must take on more patients for

Abstract Pressure ulcer prevention is a major task of direct care personnel in hospitals around the
world. Even with evidenced–based polices currently in place at many facilities, patients still
continue to develop pressure ulcers. Patients with pressure ulcers require the care of a multi–
disciplinary team, ranging from nursing assistants to nutritionists to wound care physicians. Pressure
ulcers are expensive to treat, can lead to infections and other illnesses, and sometimes cost patients
their lives. A literature search was conducted of five articles related to pressure ulcer prevention in
order to find a solution to implement on a medical–surgical unit at a local hospital. The proposed
change was selected using Lewin 's Change Theory. The proposed solution was to train nursing
assistants in assisting registered nurses (RNs) with monitoring patients ' skin and providing care to
prevent the development of pressure ulcers. The pros and cons of this solution, required resources,
costs of implementation, education required, and evaluation of the change were all discussed.
