write a paper about an intervention that you carry out with a patient. The intervention
needs to be related to a complex psychosocial situation or a behavior change intervention,
and be an example of using one or the models or theories you have read about in the
workshop.You will begin the process during this workshop. First read the materials
assigned. Then select a patient or family to apply the theory or model with, including
assessment, intervention and evaluation. The paper will not be due until day one of
Workshop Six in order for you to have time to select a patient/family, assess the situation
using the model, carry out the intervention, evaluate it, and write the
paper.________________________________________Instructions1. You will select a patient or family
that you provide care for as the subject as this paper. The patient/family should either have
a complex psychosocial situation the affects their health, or have need of a significant
behavior change, or both. The patient or family must benefit from and be willing to work on
a behavior change intervention, and you work with them on the behavior change
individually or as a family.2. You MUST use Pender unless you have permission from your
course faculty to use another behavior change model specific to the patient’s issue. For STD
prevention, the CDC STD Treatment Guidelines have specific questions that should be
addressed in the assessment, but Pender is still appropriate for the behavior change
plan.a. Introduction: Describe the patient/family briefly, including the behavior change
selected, And how Pender’s model applies to that particular behavior change. What stage of
change was the patient in? Describe how their stage of change influenced the strategy you
used.b. Assessment: Include your assessment data. The assessment must include previous
attempts to change this behavior, and what caused them to not be successful long term. It
must include an identification of patient and family strengths and weaknesses. It must
include specifics about the behavior. If diet, you must include a diet history. If smoking
cessation, you must talk about when the patient most craves a cigarette, and activities that
have always involved smoking for them. It must include an assessment of their culture in
relation to this specific behavior. Culture could include culture in their family, at work, in
their community, or related to ethnicity.c. Goal and Plan: Help patient develop at least one
SMART goal (Specific, measurable, attainable, relevant, and time-based). Describe how you
and the patient arrived at a plan, and give the plan, including follow up. Give specific details
of the conversation that illustrate your use of the theory. If you use MI, for example,
describe how you expressed empathy, developed discrepancy, rolled with resistance, and
supported self-efficacy. What resources did you provide the patient (handouts, quit-line
information, referrals, medications, etc..)?d. You need at least two additional contacts with
the patient after the initial visit to continue the intervention. Describe the patient’s

experiences.e. Outcome: What was the outcome? Use a follow up visit, a phone call, or
the chart to provide the most recent information about how successful the intervention
was.f. Learning: Describe what went well as well as what could have gone better, and
