A relatively perfusion was observed in temporal and parieto-occipital areas, particularly in the left hemisfere. A normal perfusion pattern was evident in both cerebellar lobes.

Ai fini di un corretto approccio terapeutico è fondamentale differenziare la LBD dalle altre forme di demenza, soprattutto dalla malattia di Alzheimer: studi recenti infatti hanno dimostrato che circa il 50% dei pazienti affetti da LBD presenta reazioni avverse anche gravi alla terapia neurolettica a fronte di una migliore risposta al trattamento con gli inibitori della colinesterasi. Nonostante la diagnosi differenziale costituisca il cardine su cui basare una adeguata terapia, nel 15% dei casi la diagnosi corretta viene fatta solamente in sede autoptica. ATTUALMENTE .... MA...

Linee Guida sulla diagnosi di demenza e di malattia di Alzheimer Ufficialmente approvate dalla SIN (2000) SECONDA FASE - FASE DI CONFERMA DIAGNOSTICA E DIAGNOSI DIFFERENZIALE Neuroimaging cerebrale Gli esami di neuroimaging cerebrale dovrebbero essere presi in considerazione in base alle caratteristiche cliniche di presentazione. Sembra comunque ragionevole eseguire un esame CT scan o MRI cerebrale almeno al momento della prima diagnosi. Questo esame è infatti spesso indispensabile per una corretta diagnosi differenziale. Altri esami come la SPECT o la PET, che possono fornire informazioni sullo stato funzionale cerebrale, sono di grande interesse per fini di ricerca e andranno utilizzati all'interno di protocolli di ricerca.

Il Neuroimaging Medico Nucleare, sia funzionale che biomolecolare, è in grado, attraverso la somministrazione dei radiofarmaci in concentrazioni pico/nano-molari, di rilevare e riprodurre in forma di immagini le variazioni dei processi metabolici e biomolecolari senza interferire con il sistema biologico in esame. In tal modo può fornire un’informazione sensibile e specifica già in una fase preclinica della malattia (livello I, II e III) con le implicite ricadute sulla diagnosi iniziale e sulla successiva gestione della patologia
47. Una stretta collaborazione tra clinici e medici nucleari, nell’ambito di un approccio multi-disciplinare è sicuramente la condizione più favorevole per arrivare a diagnosi meno incerte, a terapie più idonee e follow up più accurati……
