2. 24 hr stomach ache word cause shivers in
the person, family but the fears are
baseless. Mostly surgery is not required. Let
us see what it is. Appendices (Udukam) is in
junction of big & small intestine of 8 cm. Its
opening is very small food particles, feces
will not enter in. Its sticky secretions come
out mixes with intestines. Occasionally
•Feces, worms may block its opening & the
contents may result in swelling
•Bacteria in food & water may reach can
cause infections.

3. In these situations appendicitis pain starts.
Initial predominant symptom is stomach
ache. Next fever, vomiting, anorexia are
also common.
Stomach ache: Starts around naval travels
to right towards appendix called as shifting
pain. In 6-8 hrs reaches lower stomach
region. This is typical symptom.
Temperature: Always at 100.4 constantly. It
never recedes.
Vomiting: Some will have some may not
Anorexia: Don’t feel like eating food.

4. How to diagnose?
Today many modern diagnostic tools are in
place. Physical examination of the patient
by an expert Dr is very vital. US scan, blood
tests will help in accurate diagnosis.

appendicitis there is a formula. Imagine a
line from iliac spine to naval called as spino
umbilical line divide it into 3 parts 1st
point
in 3rd
part is vital because below this
appendix lies. If you press here the patient
experience unbearable pain.

5. The patient may not react this sharp when
the Dr press in other areas of stomach. This
is a very clear indication to diagnose. This is
called Mc. Barnees point.
Peritonitis: slowly the infection spreads to
other layers resulting in peritonitis. Then
the stomach will be hard when pressed.
Called as muscle guarding.
Pain while coughing: In appendicitis
patient experience pain when they cough.
They can point out the exact pain spot.

6. Moving right leg will be a big problem: Coz
here iliasoyas muscle has to move which
touches appendix causing severe pain while
moving right leg.
Chubby children may not react like this
hence some times the appendicitis becomes
chronic with sepsis formation bursts inside
and cause many complications. Hence Drs
must be extra careful in such children while
examining.

7. Ultra sound scan: To locate appendicitis
swelling 100% in diagnosis USS helps. If the
appendix walls are more than 6 mm
indicates swelling of appendix. If it is more
than 9 mm diagnose the problem 100%.
Blood Test: When blood test is done in
appendicitis the white cells will increase in
its count. If they are more than 10000/ Cu
mm with stomach ache it confirms
appendicitis definitely. When diagnosis is
clear therapy will be specific

8. Treatment: If U S Scan confirms swelling of
appendix, blood test, symptoms confirm
plan for surgery under expert care. If it not
specific in diagnosis by above tests patient
has to be admitted in hospital observed for
24-48 hrs putting on IVFs. If this relieves
pain with medicines then no surgery. If
pain is not controlled then surgery is the
only answer.
•Once it is recedes with medicines may
recur then surgery can be done.

9. •Surgery: There are 2 types. 1 cut open the
stomach remove infected appendix. 2
Laparoscopic. By putting key holes insert
camera tube conduct surgery. Both are
accepted. Laparoscopic is preferred because
there is no cut. Recovery is faster.
•When pain is severe tests confirm
appendicitis earlier the better is surgery.
Otherwise it may burst open infection
spreads. Even if it is drained possibility is
intestines may stick to each other.

10. 24 Hr dragon net: Don’t fall prey to it. Previously
in the absence of effective antibiotics Drs use to
think it may burst open in 24 hrs it is not true in
majority of cases. It is better to wait & watch for
24-48 hrs on antibiotics under expert care in a
hospital then plan for surgery as needed.
All pains are not Appendicitis: We have many
parts in our stomach pain may be from any part.
Right side pain in the stomach also may not be
appendix always. UTI, Diarrohea, infections in
kidneys, swelling of lymph glands in stomach,
pneumonia in lower lungs can cause similar pain.

11. It is not totally un wanted: Once it is
thought that appendix has no role to play
in body metabolic activities. Though there
is no role in digestion it paly a supportive
role in body immunity. In some surgery it is
used as binding layer. Hence it has to be
removed only when there is no alternative
to give relief to patient
