Health Rules for Parent

Health 4th and 5th grade Class Rules Mrs. Misencik
1. No talking when the teacher is talking.
2. Raise your hand if you have a question.
3. Respect one another’s opinions and answers.
4. Cooperate with your classmates.
5. Be prepared for class (must have pen or pencil, health book, health folder, student planner, and any
homework that may be due)
6. Participate in class discussions and activities.
7. Complete class/home assignments on time. All homework assignments will be worth up to a total of 10
points. Also, not any one assignment/assessment will be worth more than 25% of the student’s overall
grade. If the assignment is not turned in the day that it is due, then a parent signature stamp will be
placed on the assignment to designate that it was an incomplete assignment. Also, a missing assignment
sticker will be placed in the student’s planner. Parents should have their child complete the
assignment first, and then place their signature on the page in order for the child to receive credit
on the assignment.
*All Late Homework Must Be Signed By a Parent
The total amount of points that a student can possibly receive on a late assignment is 8 out of 10 points.
Continually, not doing assignments or turning assignments in late could result in a poor mark on your
report card.
8. If you are absent on a health day and an assignment is due, you must turn in the assignment the next day
that you return to school. Assignments must be placed in the health room in the designated classroom
bin or directly to Mrs. Misencik. Mrs. Misencik will not look for a student to track down his or her
homework. It is the student’s responsibility to complete the assignment and to turn it in!
9. Check the homework board or with Mrs. Misencik after an absence to determine what was missed.
10. If the above rules are not followed, behavior marks will be given. Three or more behavior marks given
in a nine-week period will result in a needs improvement mark given on your report card.
Grades
A = 100 – 90%
B = 89 – 80%
C = 79 – 70%
D = 69 – 60%
E = 59% - Below
