! Placentacapaciteit om galzuren te elimineren werkt minder
goed.
! Onrijpheid lever: galzurenexcretie kan alleen door moeder.
! Hoge concentratie galzuren in vruchtwater,
navelstrengbloed, meconium ! direct nadelig effect op:
-Foetale cardiomyocyten: aritmieën/ventrikelfibrilleren
-Chorionvenen: vasocontrictief effect.
Afname foetale perfusie en oxygenatie ! foetale asfyxie

! Perinatale sterfte afgenomen na invoering actief beleid in 1980 (CTG-bewaking en electieve inleiding à therme)
! Normaal CTG sluit optreden IUVD op korte termijn NIET uit
! IUVD is vaak acuut.
-Bij obductie kind: afwijkingen wijzend op acute hypoxie.
-Placentahistologie: milde hypoxie en afwijkingen wijzend op veranderde membraantransportmechanismen.
-Bij toenemende [galzuren] neemt [intracellulair calcium] ook toe.
Suggestion: cholic acid could inhibit activity of CMs by causing calcium overload, thereby leading to the sudden fetal death in ICP. (17)

26. Beleid medicamenteus
! Behandeling voornamelijk maternaal gericht: jeukreductie.
! Ursodeoxycholzuur:
-Herstelt verstoord transplacentair galzuurtransport
-Normaliseert hoeveelheid toxische maternale en foetale galzuren
-Stimuleert galflow (hepatocellulaire secretie)
-Stabiliseert levercelmembranen tegen schadelijke effecten toxische galzuren.
! Niet bewezen dat dit ook verbetering foetale uitkomsten veroorzaakt.
! Startdosering: 10 mg/kg/dag. Max. 20/mg/kg/dag.
! Colestyramine: onderbreking enterohepatische kringloop galzuren (vormt in darm onoplosbaar
complex, wordt uitgescheiden via feces). Alleen geregistreerd behandeling van jeuk.
Maar: -Minder effect op jeuk en lab parameters en meer bijwerkingen dan Ursodeoxycholzuur.
-Gelijktijdige toediening met urso: resorptie urso omlaag.
! Antihistaminica: sedatief effect ! betere nachtrust, minder slaapdeprivatie. Geen significant effect op
jeuk.
! Bij optreden steatorroe of bij colestyraminegebruik: vitamine K toedienen. 1dd 10 mg oraal

27. Beleid – foetale bewaking
! Vervolgen van galzuren geïndiceerd. Op enig moment >40
μmol/L ‘hoog’ risico obstetrische complicaties
! CTG, echoscopie, amniocentese, amnioscopie voor
vaststellen meconiumhoudend vw, maternale registratie
kindsbewegingen GEEN betrouwbare voorspellers IUVD.
Normaal CTG niet voorspellend ivm mechanisme van acute
anoxie
! Bovendien gaat ICP niet gepaard met intra-uteriene
groeirestrictie, uteroplacentaire insufficiëntie of abnormale
dopplerflowprofielen
! Continue foetale bewaking tijdens partus

28. Actief vs. expectatief beleid
! Bevalling vanaf week 37-38 geadviseerd door
verschillende studies o.b.v. het feit dat foetale sterfte
merendeels optreedt na 37 weken. Vooral bij ernstige ICP
! Actief beleid:
-Vervolgen galzuren
-Regelmatige CTG-bewaking
-Onderzoek mate v foetale longrijping
-Aanwezigheid meconiumhoudend vw
-Electieve inleiding bij amenorroeduur 36-38 weken
! Foetale sterfte significant lager bij actief beleid

30. Postpartum- en preconceptionele
adviezen en feiten
! Normaliseren labparameters binnen enkele weken. Checken adhv lab
! Borstvoeding kan gegeven worden
! Urso kan kort na bevalling gestopt worden. Door sterke eiwitbinding niet over in
moedermelk.
Indirect effect i.v.m. interfereren van urso met vetoplosbare vitaminen
! Colestyramine wordt nauwelijks geresorbeerd. Geen nadelige effecten. Zelfde indirecte
effect
! Vrouwen met ICP in vgs hoger risico op krijgen van (cholesterol)galstenen/cholecystitis
! ICP recidiveert vaak in volgende zs (45-70%)
! Na ICP is kans op cholestase tijdens gebruik van combinatiepil verhoogd. Kan wel
worden voorgeschreven. Stoppen indien klachten passend bij chstase.
(Progestageen bevattende anticonceptiekan zonder bezwaar worden voorgeschreven.)

32. Conclusie
! ICP aandoening 2e helft zs, waarbij toename foetale complicaties en late IUVD.
! Associatie hoogte galzuren en foetale complicaties
! Hoogrisicoptn: > of gelijk aan 40 μmol/L.
! Bij niet behandelde ICP (galzuren > of gelijk aan 10 μmol/L) galzuren om 1 – 2
weken vervolgen.
! Ursodeoxycholzuur is voorkeursmedicijn.
Aanwijzingen dat het gunstig effect heeft op foetale prognose.
! Baring nastreven vanaf 37 weken bij ernstige en systemisch behandelde ICP
! Zs nooit voorbij 40 weken

33. Waarom geen check-up foetus na
partus?
! Als geen hoog risico ptn. (galzuren <40) dan geen extra risico na partus
! Indien wel hoog risico dan electieve partus bij bv 36 weken. Dus dan
ook geen kans gehad om IUVD te krijgen, aangezien dit vooral
plaatsvindt bij 38 weken.
! Kleine kans dat net na geboorte de concentratie galzuren ineens
fataal zouden worden, terwijl eigenlijk gelijk afname optreedt van
galzouten omdat geen aanvoer meer.
