During the last breakout session of the day, at Edge of Amazing 2016, a panel came together to discuss the interdependencies that are not the responsibility of any single organization, but are required if we want to achieve population health. They featured the many ways community is linking to the delivery system, including an overiew of the Plan for Improving Population Health and the Practice Transformation Support Hub.

Dear Feifan,
I am writing to say thank you for all of your hard work and assistance as a Collabriv intern
at the American Heart Association/American Stroke Association over the last six months.
I want to especially mention four projects to which you contributed:
Under the direction of Western States Affiliate Health Equity Vice President Sang-
Mi Oh, you worked with the Young Professional Society volunteer committee in
developing a business plan to engage young professionals with the work of the
American Heart Association.
Also under Sang-Mi’s direction, you helped write scripts for a web series on
cardiovascular health topics. We hope to produce this series in the future when
funds become available.
Working under the Greater Bay Area Social Market team, you performed market
research and created an engagement action plan for the inaugural San Francisco
PULSE Ball, a fundraising event for San Francisco’s leading group of young
professionals.
You conducted and presented market research in preparation for the inaugural Bay
Area Go Red Expo, a day-long health conference targeted at underserved women
that will take place in 2014. This project was also done for the Bay Area Social
Market team.
Your efforts on these and other, smaller projects over the last six months contributed to the
mission of the American Heart Association: Building healthier lives, free of cardiovascular
diseases and stroke.
We are deeply appreciative of your hard work and commitment.
Sang-Mi, Kimberly, Caitlin, Anders and Antoinette join me in wishing you the best of luck
in all of your future endeavors.
Sincerely,
Julie Kaufmann
