Steven Welby receives services from Countryside, including a day program at their workshop. The ISSA worker, Jenny Boers, conducted an assessment of Steven during a visit on 5/22/2013. She interviewed Steven, his job coach, and father and reviewed his file. Steven appears satisfied with his living situation and services. He has friends at the workshop and positive relationships with staff. The ISSA determined that follow up is not needed at this time as Steven's needs appear to be met through his current supports.
