wedge procedures because they are easier to perform
•For example, a midshaft fracture of the tibia with varus
(lateral) angulation requires a cut about two-thirds around
the cast at the level of the fracture on the medial side of the
cast.

•Closed wedge procedures are less commonly performed than
open wedge procedures.
•To treat a mid-shaft fracture of the tibia with lateral (varus)
angulation using the closed wedge technique, you must
make a wedge-shaped cut on the lateral side of the cast at the
level of the fracture and then remove the cut wedge.

•The greater the angulation of the fractured leg, the larger the
size of the wedge that should be cut out of the cast.
•Manipulate the cast with a valgus force across the wedge cut
in the cast, closing the open wedge (bringing the cut edges of
the cast closer together), which should reduce the fracture to
the anatomical position.

•Be very careful when closing the wedge, so you do not
pinch the skin and cause a pressure sore
•Check the cast padding to ensure that there is not an
unacceptable amount of bunching, which could also cause a
pressure sore
