It shows that spending on healthcare has increased dramatically as a percentage of GDP while K-12 education has remained steady. Spending on higher education has also increased but remains a smaller portion of GDP. California needs to produce more degrees and credentials by 2025 to meet workforce demands but is on track to fall short. For-profit institutions produce a large portion of healthcare credentials but underrepresented groups rely more on public institutions. The supply of degrees does not fully match the demand for healthcare jobs in California. More must be done to improve access and affordability, especially for low-income and minority students.

BTG interns managed and ran 6 PAP offices across Philadelphia, serving as advocates for low-income, uninsured patients. Agriculture bangladesh
Climate change poses serious threats to Bangladesh due to its heavy reliance on natural rainfall and temperature for agriculture. Rising sea levels are increasing floods, erosion, droughts, and salinity intrusion, damaging homes and farmland. Bangladesh contributes to climate change through unsustainable resource use, urbanization, and industrialization. To combat these issues, Bangladesh is developing climate-resilient crops, alternative farming methods, coastal barriers, early warning systems, and seeking greater international cooperation.
Public Participation in Bangladesh
Bangladesh is one of the most vulnerable countries to climate change due to its geography, population density, reliance on agriculture and fishing. A rise in sea levels could inundate 25% of land and displace 18 million people. More intense storms and droughts will further impact agriculture and food security. While public participation is important for environmental decision making, there are many barriers that discourage civic engagement on climate change in Bangladesh, including psychological, social, political, and structural factors. Effective communication is needed to overcome these barriers by framing climate change as a widespread problem, demonstrating the need for solutions, and appealing to social values of cooperation.

A Day at Nyaya-FINAL
Nyaya Hospital in Nepal provides medical care to the local community. Nyaya Health is a non-profit organization that provides free healthcare in rural Achham district of Nepal. It operates Bayalpata Hospital in partnership with the Nepali Ministry of Health. The hospital provides outpatient and emergency services, an inpatient ward, laboratory services, and community health outreach. Nyaya Health aims to strengthen Nepal's public health system and develop a replicable model of healthcare delivery. It is funded through donations and employs over 30 full-time Nepali staff. Upcoming projects include expanding surgical and community health services and increasing solar power capacity. Nyaya Health recently received a $75,000 matching grant and is seeking help leveraging additional support.

Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.
