I created mockups for the Distributed Health Labs @ CalIT2's Patient Health Record mobile app. The application aims to increase health literacy by making sense of confusing health records, and by placing medical data in the hands of patients.

1. PATIENT HEALTH RECORD APP
Over 85% of adults have less than proficient health literacy.
Medical records are needed to help doctors and nurses make the right
decisions for your health. But that’s just the problem. Medical records are
just records, and we often deliver them from doctor to doctor without
understanding what’s inside.
This app uncovers the information buried in your medical records,
empowering you to better understand your body and make smarter
decisions about your health.

2. BOLD COLORS
An app that is simple and modern,
one that you will actually want to
use every day.
NO MORE NUMBERS
Icons and colors give meaning to numbers, making them less abstract and
more understandable.
KEEP TRACK OF THE EVERYDAY
Tracking your mental and physical health is
easy with a questionairre you fill out every
day. Results are graphed over time, so
you can detect patterns and report health
problems to your doctor more accurately.

3. DATA THAT MEANS SOMETHING
A progress meter gives you a goal to work towards.
Colors and icons are more concrete and easier to
understand than abstract numbers.
GET THE BIG PICTURE
Graphs put medical data into your hands,
allowing you to track changes in your
health over time.
TAKE ACTION
The “About” section promotes medical
literacy, while suggestions on ways to
improve empower you to do something
about your health. These suggestions can
also be added to your health planner in
the app.

Email and phone notifications
ensure you follow your regimen.

You will no longer need to search through
a label only to try and decipher confusing
instructions.

5. MEDICAL HISTORY AT A GLANCE
Review your medical history visually, instead of having to
read a wall of text. Icons allow you to find specific types of
events quickly. Zoom out to get an even bigger picture.
HAPPY BIRTHDAY
Birthdays are a natural way to make sense
of time. Displaying birthdays allows you to
relate other dates to something concrete,
and may also help you determine causality.

Your days of leafing through pages to get
to the information you need are over.

information with your caretaker or guardian.
