The field of transcatheter mitral valve repair (TMVr) for
mitral regurgitation (MR) is rapidly evolving. Besides the
well-established transcatheter mitral edge-to-edge repair
approach, there is also growing evidence for therapeutic
strategies targeting the mitral annulus and mitral valve
chordae. A patient-tailored approach, careful patient
selection and an experienced interventional team is crucial
in order to optimise procedural and clinical outcomes. With
further data from ongoing clinical trials to be expected,
consensus in the Heart Team is needed to address these

Antibiotic Stewardship by Anushri Srivastava.pptxAnushriSrivastav
Stewardship is the act of taking good care of something.
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
WHO launched the Global Antimicrobial Resistance and Use Surveillance System (GLASS) in 2015 to fill knowledge gaps and inform strategies at all levels.
ACCORDING TO apic.org,
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
ACCORDING TO pewtrusts.org,
Antibiotic stewardship refers to efforts in doctors’ offices, hospitals, long term care facilities, and other health care settings to ensure that antibiotics are used only when necessary and appropriate
According to WHO,
Antimicrobial stewardship is a systematic approach to educate and support health care professionals to follow evidence-based guidelines for prescribing and administering antimicrobials
In 1996, John McGowan and Dale Gerding first applied the term antimicrobial stewardship, where they suggested a causal association between antimicrobial agent use and resistance. They also focused on the urgency of large-scale controlled trials of antimicrobial-use regulation employing sophisticated epidemiologic methods, molecular typing, and precise resistance mechanism analysis.
Antimicrobial Stewardship(AMS) refers to the optimal selection, dosing, and duration of antimicrobial treatment resulting in the best clinical outcome with minimal side effects to the patients and minimal impact on subsequent resistance.
According to the 2019 report, in the US, more than 2.8 million antibiotic-resistant infections occur each year, and more than 35000 people die. In addition to this, it also mentioned that 223,900 cases of Clostridoides difficile occurred in 2017, of which 12800 people died. The report did not include viruses or parasites

The dimensions of healthcare quality refer to various attributes or aspects that define the standard of healthcare services. These dimensions are used to evaluate, measure, and improve the quality of care provided to patients. A comprehensive understanding of these dimensions ensures that healthcare systems can address various aspects of patient care effectively and holistically. Dimensions of Healthcare Quality and Performance of care include the following; Appropriateness, Availability, Competence, Continuity, Effectiveness, Efficiency, Efficacy, Prevention, Respect and Care, Safety as well as Timeliness.
7. The Future of Surgery !
Ablation over excision
Image-guided over visual
Reconstruction without suturing
Access via natural orifice or blood vessel over incision
8. 2002-TAVR (Transcatheter AorTc Valve Replacement)
Image-guided over visual
Reconstruction without suturing
Access via blood vessel over incision
9. Key Concept
In Baseball, Tie Goes to the Runner !
In surgery, Less Invasive Beats More Invasive
as Long as the Outcomes are Equivalent !
13. Start of My Surgical Residency
-1976
Acute MI- one month hospital stay
Surgical hip opera<ons- 6 weeks recovery
No knee replacements
Acute leukemia-no cure
Cholecys<<s-6 week return to work
Heart transplants- headlines to
abandonment
No interven<onal cardiology
Heart valve surgery rare
14. Fast Forward 41 Years
Mortality from heart disease declined by 50%
Minimally invasive total hip replacements rou<ne
Bilateral total knee replacements outpa<ent
procedure
Most leukemia is cured
Lapcholy is an outpa<ent procedure
Heart transplant and VAD’s now rou<ne
Percutaneous Heart Valves with same day discharge
17. Reasons Why Most Surgery Should Be Done Minimally Invasively !
Less physiologic and systemic insult to the pa<ent
In general…
• quicker recovery
• shorter hospital stay (or outpa<ent)
• quicker return to normal ac<vi<es
18. Reasons Why Surgeons May Not Adopt
Minimally Invasive Surgery
• Requires a different skill set
• May have trained before the MI procedure was developed
• There is oben a “learning curve”
• Forces adop<on of a new technique when one has already been mastered
• May force operators to get out of their “comfort zone”
• May not be rigorous scien<fic evidence of a posi<ve benefit/risk
• Beger? (or even equivalent)? (oncologic surgery)
19. Other Reasons Why a Reluctance or ReTcence to Adopt MI Procedures
• Procedure may be less efficacious (fewer lymph nodes during VATS lobectomy)
• May be associated with a new set of complica<ons
• Risk averseness in face of public repor<ng of outcomes
• Financial incen<ves may not be aligned (may take longer and pay less)
• May require more extensive and expensive equipment (robo<cs) not readily available
(cost effec<veness)
• Higher percent of MI surgery may be done by subspecialists but most surgery is done by
“generalists” (access to care)
• VATS lobectomy
• Laparoscopic esophagectomy or colectomy
22. Why NOT Make MI Surgery a Quality Metric
(or What are Possible “Unintended Consequences”)
May force surgeons out of their “comfort zone”
May result in worse outcomes in the short term
Adequate “risk stra<fica<on” models may not exist
Not every procedure can be done minimally invasively
If a MI procedure is adopted too early and outcomes are not
equivalent, nega<ve halo effect on the field
Can create turf wars when a less invasive procedure is performed by
another specialty
