پیرپوستی ،کارگاه آموزشی کازمتیک انجمن داروسازان Mohammad Baghaei
Congenital Hypotrichosis

Lateral condylar fractures of the distal humerus are common injuries in children that can be classified based on the degree of displacement and integrity of the cartilage hinge. Minimally displaced fractures are treated with immobilization while more displaced fractures may require closed reduction and percutaneous pinning or open reduction and internal fixation. Complications can include cubitus varus, delayed union, nonunion, and neurologic injuries if not treated properly.
The document discusses various surgical techniques and approaches used in orthopedic surgery, including:
1) The use of tourniquets, radiographs, positioning, and draping of patients.
2) Techniques for tendon-to-bone fixation such as sutures, anchors, and buttons.
3) Bone grafting procedures using autografts, allografts, and substitutes to fill defects from tumors, cysts, or fractures.
The pelvis has three regions: the false pelvis, true pelvis, and perineum. The true pelvis has an inlet, walls, and outlet. The inlet is heart-shaped and bounded by bone. The walls consist of bone, ligaments, and muscles and contain openings. The outlet is diamond-shaped, bounded anteriorly by bone and posteriorly by ligaments. The pelvic floor separates the pelvis from the perineum and is formed by muscles. Nerves in the pelvis include the sacral plexus and sciatic nerve.
This document discusses ankle fractures, including their classification, clinical assessment, treatment options, and outcomes. Ankle fractures represent 10% of all fractures and are typically caused by falls or sports injuries. Assessment involves history, exam, and imaging to classify the fracture. Treatment depends on the specific fracture but may involve immobilization, surgery, or a combination. Most patients recover well, though complications can include infection, loss of reduction, or post-traumatic osteoarthritis.
The document discusses elbow dislocations and terrible triad injuries, including simple elbow dislocations, radial head fractures, coronoid fractures, and injuries involving the ulna. Nonoperative and operative treatments are described for each condition. Nonoperative treatments involve splinting or bracing while operative treatments involve open reduction and internal fixation or joint replacement. Complications of both treatment approaches include stiffness, instability, and nerve injuries.
Fractures of the radius and ulna shafts are common injuries that can have varying classifications and treatment approaches. The document discusses:
1) Classification systems for these fractures including the OTA system that categorizes fractures as Types A, B, or C based on fracture pattern and involvement of one or both bones.
2) Specific fracture types like Monteggia and Galeazzi fractures which involve fractures of the ulna or radius with dislocations of the proximal radioulnar joint.
3) Treatment options including nonoperative management for nondisplaced fractures or operative treatment with external fixation, intramedullary nailing, or plate osteosynthesis depending on the fracture. Complications are also reviewed
1. Clavicle fractures most commonly occur in the midshaft region, resulting from a direct blow to the shoulder.
2. Physical examination reveals swelling, bruising, and deformity at the fracture site. X-rays are usually sufficient to diagnose the fracture.
3. Displaced midshaft fractures are often treated surgically using plates or intramedullary pins to restore length and alignment. Post-operatively, patients begin range of motion exercises and are weaned from immobilization over several weeks.
Dr. Mohammad Mahdi Shater provides guidelines for postoperative care following plate fixation and intramedullary nailing for femoral shaft fractures. For plate fixation, early range of motion exercises are encouraged once wounds have healed and weight bearing should be limited until radiographic evidence of healing is seen. For intramedullary nailing, early mobilization and range of motion exercises can be started immediately after surgery to prevent complications, though weight bearing depends on factors like associated injuries and fracture location. Regular radiographic follow up is recommended to monitor healing.
Injuries to the sternoclavicular (SC) joint are uncommon and can result from direct or indirect forces. Mechanisms typically involve vehicular accidents or athletics. Assessment involves history, physical exam, and imaging like CT or MRI. Injuries are classified based on anatomy and etiology. Treatment depends on type but may involve closed reduction, bracing, or open surgery for irreducible injuries. Complications can be life-threatening if not addressed promptly and can include vascular, tracheal, or neurological issues. Careful follow-up is important to monitor for late effects.
Femoral neck fractures are common in elderly patients following falls and account for 50% of hip fractures. They can be classified using systems like Garden or Pauwels. Undisplaced fractures may be treated non-operatively but displaced fractures usually require internal fixation or arthroplasty. Operative options depend on patient age and include cannulated screws, hemiarthroplasty or total hip replacement. Outcomes are influenced by achieving anatomic reduction and fixation. Younger patients presenting with these injuries often have higher energy mechanisms and require urgent treatment.
1) Pathologic fractures occur in abnormal bone that has been weakened, often by conditions like cancer or osteoporosis, and these fractures can occur during normal activity or minor trauma.
2) Evaluation of patients with possible pathologic fractures involves medical history, physical exam, laboratory tests, and imaging to determine the cause and extent of the bone abnormality.
3) Plain x-rays are usually the first imaging study and can provide clues about conditions like osteoporosis, fractures, or bone tumors, but other imaging may be needed to fully evaluate bone lesions or confirm a diagnosis.

This document discusses approaches to diagnosing and treating neonatal hypopituitarism. It begins by introducing the pituitary gland and its roles. It then describes clinical findings in congenital hypopituitarism such as hypoglycemia, temperature instability, and prolonged jaundice. It outlines the diagnostic approaches including patient history, physical exam, and endocrine evaluations of the pituitary-adrenal, thyroid, gonadal, growth hormone, and prolactin axes. Treatment approaches are also discussed for deficiencies in these axes as well as central diabetes insipidus. The conclusion emphasizes the uniqueness of assessing the hypothalamic-pituitary axis in the neonatal period.
This document discusses tricyclic antidepressant (TCA) toxicity from overdose or drug interactions. It outlines the clinical features and timeline of TCA toxicity, which include early anticholinergic effects followed later by myocardial depression, seizures, and arrhythmias. Diagnostic testing includes ECG and urine drug screens. Management involves stabilizing the patient, administering activated charcoal, fluid resuscitation, vasopressors if hypotensive, benzodiazepines for seizures, and other interventions depending on symptoms. Patients exhibiting signs of TCA toxicity such as arrhythmias or seizures require ICU admission.
1. The document provides guidelines for evaluating and treating chest trauma, with a focus on the ABCs (airway, breathing, circulation). It discusses assessing and securing the airway, treating tension pneumothorax, hemothorax, and cardiac tamponade.
2. The secondary survey involves using imaging like CXR, CT, and ultrasound to identify injuries like aortic tears, tracheal lacerations, and pulmonary contusions. Procedures like bronchoscopy and thoracotomy may be needed. Outcomes of various injuries are discussed.
3. Complications of chest trauma like empyema are outlined. Treatment depends on imaging and test findings. Pneumatoceles may become infected and
