Local authorities have been given renewed responsibility for public health. They can take action in several ways like ensuring mothers are healthy before and during pregnancy to reduce infant mortality, promoting healthy schools and education to provide returns of £7.20 for every £1 invested, and helping people find and stay in jobs to prevent depression from unemployment. Local authorities also have a role in active and safe travel, making homes safer, increasing access to green spaces, building strong communities to address loneliness, improving air quality, and planning healthy places through spatial planning.

This document summarizes a presentation given by Professor John Middleton on better evidence for better law enforcement and public health. It discusses the roles of various public health organizations in Europe, such as EUPHA and ASPHER. It provides examples of effective public health interventions including pre-school programs, harm reduction approaches, and healthy urban planning. It emphasizes taking a multidisciplinary, evidence-based approach to issues like crime prevention. The presentation addresses topics like adverse childhood experiences, mental health, climate change, and human rights in relation to public health and law enforcement.
The document discusses mental health integration and provision for supporting people with mental illness in the UK. It finds that the UK ranks second overall in the Mental Health Integration Index, scoring highly in categories like environment and governance. While each constituent part has its own policies and successes, England is the focus since it contains most of the UK population. The document outlines England's long evolution of policies from deinstitutionalization to recent efforts to achieve "parity of esteem" between mental and physical health. However, it finds that fully implementing reforms and integrating services remains a work in progress, as serious treatment gaps persist.
Suitable housing is essential for personal well-being and quality of life at all stages of life. It enables access to services, social relationships, and independence. Housing associations play an important role in supporting health by providing suitable accommodation and programs that prevent falls, reduce isolation, and encourage healthy behaviors, especially for older residents and those with disabilities or long-term conditions. Through adaptations, supportive housing, and care services, they can help people maintain independence and avoid expensive acute health services.
This document discusses the dangers of second-hand smoke exposure in the Western Pacific region. It notes that a third of the world's smokers are in this region, and that 2 people die every minute from tobacco-related diseases here. The document advocates for stronger policies to protect people from second-hand smoke, such as smoke-free laws in public places and workplaces, which have been shown to reduce tobacco consumption and encourage smoke-free homes. It outlines initiatives by the WHO to support countries in fully implementing the WHO Framework Convention on Tobacco Control, especially its protections from second-hand smoke.

Ben Hannigan gave a presentation on mental health in Wales. He discussed how Wales has developed its own distinct mental health system and policies, including legislation promoting mental health as a human right. Wales uses a planned approach to health led by regional health boards rather than England's purchaser-provider model. Recent initiatives aim to improve coordination between primary and secondary mental health care. However, independent research shows there is still work to be done to address mental health challenges, despite Wales' recognition of the issue and innovative policies.

This document provides a summary of NHS Sustainability Day 2014. It discusses several challenges facing health and social care from climate change like extreme weather events and how the sector is addressing these issues. The national legislative framework for climate change adaptation is outlined. Presentations covered topics like the health effects of climate change, the health co-benefits of lowering emissions, and initiatives to green healthcare like sustainable procurement. Hospitals achieving food quality standards through the Soil Association's Catering Mark was also discussed. The event emphasized that sustainability is a leadership issue and that national tools are in place to support local climate change adaptation action in the health and social care sector.
This document discusses emerging approaches and lessons for making an impact on public health and wellbeing in England. It notes the increasing life expectancy but decreasing healthy life expectancy. Key health challenges include non-communicable diseases driven by risk factors like diet, tobacco, and obesity. Opportunities for improving health include prevention focused initiatives, place-based approaches, integrating health and social services, and empowering communities and individuals. Public health requires a whole system approach and partnership across many sectors.
As a member of the Global green and healthy hospitals, Hull and East Yorkshire Hospitals NHS Trust hosted the day and help kick-start the NHS Sustainability road-show series, allowing the likes of Yorkshire Ambulance Service and East Riding Public Health to become involved and demonstrate their work around health and sustainability.

This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.

PGx Analysis in VarSeq: A User’s PerspectiveGolden Helix
Since our release of the PGx capabilities in VarSeq, we’ve had a few months to gather some insights from various use cases. Some users approach PGx workflows by means of array genotyping or what seems to be a growing trend of adding the star allele calling to the existing NGS pipeline for whole genome data. Luckily, both approaches are supported with the VarSeq software platform. The genotyping method being used will also dictate what the scope of the tertiary analysis will be. For example, are your PGx reports a standalone pipeline or would your lab’s goal be to handle a dual-purpose workflow and report on PGx + Diagnostic findings.
The purpose of this webcast is to:
Discuss and demonstrate the approaches with array and NGS genotyping methods for star allele calling to prep for downstream analysis.
Following genotyping, explore alternative tertiary workflow concepts in VarSeq to handle PGx reporting.
Moreover, we will include insights users will need to consider when validating their PGx workflow for all possible star alleles and options you have for automating your PGx analysis for large number of samples. Please join us for a session dedicated to the application of star allele genotyping and subsequent PGx workflows in our VarSeq software.
Congestive Heart failure is caused by low cardiac output and high sympathetic discharge. Diuretics reduce preload, ACE inhibitors lower afterload, beta blockers reduce sympathetic activity, and digitalis has inotropic effects. Newer medications target vasodilation and myosin activation to improve heart efficiency while lowering energy requirements. Combination therapy, following an assessment of cardiac function and volume status, is the most effective strategy to heart failure care.
