This document contains a self-assessment survey for evaluating contributions and performance as a team member. It includes 21 questions across 5 categories: Contributing to the Team's Work, Interacting with Teammates, Keeping the Team on Track, Expecting Quality, and Having Relevant Knowledge, Skills, and Abilities. Respondents are asked to rate themselves on a scale from Strongly Disagree to Strongly Agree for each question, with an N/A option. Scores are then calculated for each category.
