● Responsible for the active direct supervision of all children assigned to unit. Responsible for
monitoring the physical health, hygiene and safety of each child in unit.
● Coordinates with Dietary Personnel to ensure that proper diet is maintained. Teach living and
social skills on a daily basis.
● Implements the multidisciplinary team plan of care,as well as individual treatment plans and
group therapy treatment for the children assigned to unit.
● Provides an ongoing assessment of changes in child status (progress notes, incident reports,
treatment received).

● As an intern, I am responsible for contacting and visiting seven or more patients and
having a weekly supervision meeting with assigned Social Work Supervisor, every week.

assessment, volunteer progress note and time sheet completion.
● Patients visits included: adhering to patient’s plan of care, providing companionship to
the patient and family, serviced patient by reading, taking walks, watching tv, writing
