3. ROOT CAUSE:
 Limit switch of the Crane No.1 was by-passed to
enable the crane jib to be lowered fully to position
lashing materials farther aft of Hold No.2
 The Decision to bypass limit was not informed to
Officer on duty or Chief Officer.
 Nobody pay attention to the possible hazard when
the by-pass of the jib is override it will touch the jib
rest which caused the damage.

4. PREVENTIVE MEASURES:
 Do not By-pass the limit switch without Master and Chief
officer permission.
 Always conduct proper risk assessment to determine
possible hazards before commencing operation.

6. ROOT CAUSE:
 Cause of the cargo cannot be lifted at the loading
port was due to the Crane No. 2 Hoisting limit
was activated because of very long sling attached
to the spreader. (Lifting plan was provided by
MOL Port Captain)

7. PREVENTIVE MEASURES:
 Instructed all vessel to strictly follow company procedures
for checking and preparation of ships crane before arrival
every port.
 All Masters and Chief Officers are instructed to carefully
check all lifting plans provided by Port Captains.
 Circulated to all management vessels for crew awareness.

9. ROOT CAUSE:
 Inadequate/Improper Risk Assessment Conducted
 Malfunction of Equipment – the hose slipped out from the pump
due to strong pressure
 Modification of Equipment – the fitting of the Wilden pump has
no groove to hold the Hose tightly.
 Improper positioning of equipment – instead on main deck, the
drums should be positioned inside the crane post so that in case
of events like this, the spillage/spray will be confined.
 Failure to check all equipment before putting into operation.

10. PREVENTIVE MEASURES:
 Ensure that proper risk assessment are carried out as per
company procedures.
 Disseminate to all management vessel for their awareness.
 To properly check the equipment before using and ensure
that correct equipment is being used for the job.
 Shore personnel will visit the vessel to conduct education to
the crew to avoid any short-cut of the procedures.

12. ROOT CAUSE:
 Improper Risk assessment was conducted.
 Improper Communication between Stevedores and
Ship’s crew.
 Unable to thoroughly check Pontoon Lifter
equipment prior operation.
 Lack of proper maintenance for each part of the
pontoon lifter.

13. PREVENTIVE MEASURES:
 Always conduct proper Risk Assessment
 Establish a proper and clear communication between stevedores
and crew before commencing operation.
 Thoroughly conduct proper checking as per modified Safety
Checklist RC-004 and Cargo Work Safety and Securing Checklist
CK-016 for additional items to be checked prior using and
securing of deck cranes and pontoon lifters.
 Follow and conduct maintenance as per revised PMS/MMD which
is now includes of the Pontoon Lifter maintenance and load test
date.

16. ROOT CAUSE:
 Damaged Windlass motor because it was left engaged after
dropping the anchor. (Not Following proper procedures
before and after Anchoring operation).
 Master readily follow the instruction from sub-charterer
operator without confirming first from disponent owner.
Also no strict monitoring from disponent owner.
 When the weather deteriorates, Master failed to act timely
by using engine to ease tension on the chain or leave the
anchorage area before bad weather approaches.

17. PREVENTIVE MEASURES:
 Always “Disengage” Windlass after anchoring to avoid
damage on the motor.
 When weather deteriorates, engine must be used to avoid
too much tension on anchor chain.
 Early decision to leave anchorage in case the weather
condition worsen.
 Strict supervision and monitoring from operator of the
disponent owner and early intervention to avoid vessel
being placed on an unsafe area.

incompatibility of old brake component with new luffing motor installed.

21. ROOT CAUSE:
 Separation of Hydraulic motor with Brake Assembly
upon installation from previous trouble. May result to
un-even fittings of new motor with old brake assembly
component.
 Failure to manually fill the Luffing motor with Hydraulic
Oil to lubricate internal parts of the motor and brake
which may cause damage to initial start up running it
on a dry condition.

22. PREVENTIVE MEASURES:
 Reminders to crew regarding the proper replacement of
parts as per maker’s instructions.

24. ROOT CAUSE:
 Material fatigue due to long time of service
accompanied by wear and tear.

25. PREVENTIVE MEASURES:
 Proper Maintenance of the unit as per PMS of the
Company should be observed without fail and always keep
spare parts onboard for immediate use.
