  3. 3. What Happened On the 14th May 2013 at approximately 11:15am a contracting company were progressing work to install four 9.0m “A” frame height restriction barriers. The completed works were to provide protection to the newly installed Demineralised Water and Steam Pipework systems. The installation of the second “A” frame required it to be transported approximately 50 metres to its final position on the concrete plinths. The “A” frame (weight approx 688kg) was attached to the telehandler using a 2 – tonne, 2-meter single strop, which was attached directly to the offside fork of the telehandler. Due to the dimensions of the frame, the boom length of the machine had to be extended to 75% of the maximum reach. As the operator was reversing the telehandler (at crawl pace) the vehicles offside rear wheel or wheels came off the kerb causing the “A” frame to swing on the strop. As a consequence the centre of gravity was displaced resulting in the vehicle overturning. The operator sustained a minor injury to the back of his head after being thrown through the offside window that shattered upon impact with the road. Conclusions · Telehandler was driven with the boom extended however the operating manual states the machine must not be driven with the boom extended. · Failure to recognise the need for a designed lifting appliance that would have prevented using the offside fork. · Telehandler was not suitable for the application, the weight ratio and travel distance was not considered. · Inadaquate Job Method Statement – too generic and did not include a lifting plan. · Job Method Statement should not have been accepted by the Tata Project Engineers. The JMS should have been challenged. · On the day of the incident the ATWC was signed by a Project Engineer who had no direct involvement with the work. · The machine had audible and visual warning alarms that were activating to warn operator that the machine was outside off the S.W.L. parameters. · Operator was not wearing his lap belt to prevent fall from the vehicle when it overturned. · There were no clear instructions or communications within the working party. · Telehandler was in a servicable condition prior to the incident. · Supervision failed to control the behaviour of the operator. Recommendations Person Responsible Completion Date * All Telehandlers owners on this site are to ensure the operators are aware of the manufacturing capabilities of each model – in this case the JCB telehandlers of this model must not travel with boom extended.
