Impact of influenza exposure among pregnant women dodds cmaj 2007Ruth Vargas Gonzales
This study examined the impact of influenza exposure on rates of hospital admissions and physician visits for respiratory illness among pregnant women in Nova Scotia from 1990-2002. The researchers found that during influenza seasons, the rate of hospital admissions in the third trimester of pregnancy was 7.9 times higher for women with comorbidities and 5.1 times higher for women without comorbidities, compared to the year before pregnancy. Hospital admission rates in the third trimester were highest during influenza seasons at 7.4 per 10,000 woman-months for women without comorbidities and 44.9 per 10,000 woman-months for women with comorbidities. The editorial calls for a renewed national commitment to address this issue, including establishing a comprehensive national immunization authority and public awareness campaigns to promote vaccination and hygienic practices.

Extraordinary advancements in mobile technology and connectivity over the last several decades have provided the foundation needed to dramatically change the way healthcare is currently practiced. While the utilization of mobile telecommunication technologies for the delivery of healthcare (mobile health, or mHealth), is still in its earliest stages of development, the evidence supporting its potential to impact the delivery of care, to improve outcomes and lower costs is apparent. For healthcare consumers, mHealth technologies can allow for the more convenient, rapid and personalized diagnosis of some of the most common acute symptomatic illnesses. In individuals with chronic medical conditions, who account for nearly 90% of healthcare spending, mHealth technologies can transform their care through improved monitoring, tracking, and engagement of critical biometrics during routine daily activities, not only in doctor’s offices. In addition, despite greatly empowering patients to better care for themselves, this transformation in care will actually reinforce the doctor-patient relationship by allowing physicians to re-establish their role as diagnostician and educator for their patients. mHealth technologies have the potential to radically change every aspect of the healthcare environment but clinical trial evidence of its overall benefit to the patient and the healthcare system are still needed.

The benefit of mobile technologies to intervene and improve health is increasingly becoming a topic of discussion. Through the use of examples from three primary chronic conditions, this report will critically analyse the scientific findings of the use of these sensors and the possible impact they might have on future diagnoses and improving the overall efficiency and accuracy of healthcare management. This report will specifically interrogate the multiple technologies which can be integrated for cardiovascular problems, diabetes and chronic obstructive pulmonary disorder. The overall findings concluded that these novel technologies would provide considerable improvements to healthcare, but would have to overcome several obstacles prior to total integration. A cost-benefit analysis would have to be done to overcome the previous more traditional approach.

The chronicity of COPD allows for self management by sufferers. (Spencer & Barcomb 2014). The self management goal is reduced hospital admissions and improved life quality (Bedra et al 2013). Sufferers should have access to a wide range of skills available from the multidisciplinary team. Those include exacerbation limitation, respiratory failure, chronic productive cough and anxiety and depression.
Symptom Recognition.
Patients discharged from hospital are susceptible to readmission (Bedra et al 2013). Understanding the condition and knowing when they are having an exacerbation is imperative for self management, and what to do in the given circumstances, and when and what medication to take, or realise they need hospital treatment.
Treatment.
The main form of treatments comes from inhaled therapies and explained below would be when they would be administered and their understandings are a major factor in self management.
For breathlessness and exercise limitations: A short acting Beta2 agonist (as required) or short acting muscarinic antagonist (as required).
For exacerbations or persistent breathlessness: A long acting beta2 agonist, long acting muscarinic antagonist, to – long acting beta2 agonist + inhaled corticosteroid (Combination Inhaler) OR a long acting muscarinic antagonist (must discontinue short acting antagonist once this is commenced).
(Remember if using Corticosteroids, this has no evidence of long terms benefits).
If experiencing persistent exacerbations or breathlessness. Long acting Muscarinic antagonist + long acting beta2 agonist and inhaled corticosteroid (combined inhaler).
Niesters et al, (2012) describe how oxygen therapy can also be used, but awareness of inappropriate oxygen therapy with COPD patients is imperative as this can cause respiratory depression.
Self Monitoring.
The British Thoracic Society (BTS) have identified five high impact actions that can improve outcomes for people being discharged after an acute exacerbation of COPD. The form is a quick way of identifying patients need for those interventions, ensuring their needs are met. The aim is for lessened hospital readmission rates with self monitoring patients. The five actions are;
Review of medication and demonstration of inhalers they will be using.
Provide a written Self Management plan and Emergency drug pack.
Asses and offer referral for smoking sensation.
Assess for suitability for pulmonary rehab.
Arrange a follow up call within 72 hours of discharge.
Educational Interventions.
Reardon et al, (2005) explain pulmonary rehabilitation as programs which work with patients to help manage their condition, muscle strength, ability to cope with their disease, help with social requirements as people can become quite isolated.

• Over 6 months “connected” cohort had 68% more
PCP contacts and an average of 7.5 calls with a
primary care nurse c/w 0 in control cohort.
• Connected patients were much more satisfied
(p<0.001) but no difference in QOL.
• Connected patients experienced significantly greater
readmissions (p=0.005) per month and hospital days
(p=0.041).

• Intervention clinics incorporated mobile devices;
BP, pulse ox, scales, glucometers… plus symptom
questions and educational messages.
• At 12 months sig decrease in admissions (OR 0.82,
p=0.017) and unadj. mortality (OR 0.54, p=<0.001).
• However, the difference in admissions was limited
to only the first 3 months and was due to a marked
increase in control admissions.
• No change in PCP contacts, QOL or psychological
