Case Study "Using Big Data to Shift from Evidence-based Practice to Practice-based Evidence"
Packard Children's Hospital at Stanford has been on a journey towards a comprehensive electronic medical record (EMR) since 2004. These efforts resulted in national attention in 2010 with the publication of the first-ever correlation between implementation of computerized physician order entry (CPOE) and a decrease in hospital-wide mortality. This landmark was followed in 2011 with a paper in the NEJM describing the first documented use of aggregate EMR data to make a real-time patient care decision. In this talk, Dr. Chris Longhurst will share the "story behind the story" of these accomplishments and highlight opportunities for using applied clinical informatics to improve the value of healthcare we deliver to our patients.

Women with mutations in BRCA1 or BRCA2 are at high risk of developing breast cancer and, in British Columbia, Canada, are offered screening with both magnetic resonance imaging (MRI) and mammography to facilitate early detection. MRI is more sensitive than mammography but is more costly and produces more false positive results. Radiotherapy technology recently employed at a cancer center in Glasgow, Scotland, has the ability to generate
individualized radiotherapy treatment plans for as many as 50 pa ents at a me within minutes, circumven ng a
process that typically takes physicians more than a day per pa ent plan, the Herald reported early this week.
The novel tech debuted at the Beatson Ins tute’s cancer center, according to the report, and cuts manual labor for
radiologists using a so ware called mul -criteria op miza on.
Garry Currie, the head of radiotherapy physics at NHS Greater Glasgow and Clyde, told the Herald he didn’t think
technology would reach this point during his career.
“Our work never stands s ll, [but] this is a real game-changer,” he said. “As recently as 2010, staff would start work
on just one treatment plan and come back in the next day—including weekends—to finish the calcula ons.”

He said humans just aren’t capable of achieving such efficiency when it comes to those complex mathema cs. Now,
he said, radiologists can instantly assess the impact a suggested treatment plan would have on radia on dose toward
the beginning of the therapy process.
“We’re at the forefront of this work,” he said. “However, the results will con nue to improve as more cancer centers
start working in the same way and more and more collabora ons take place.”
