1. BURDEN OF CARE AND
DEPRESSION AMONG PRIMARY
CAREGIVERS OF PATIENTS WITH
ALCOHOL USE DISORDER
Name: DR SAKSHI CHIRAG SHAH
Designation: JUNIOR RESIDENT 1ST YEAR
Department :PSYCHIATRY
PG Guide:
2. INTRODUCTION
• Global prevalence rates of alcohol use disorders among
adults were estimated to range upto16% in 2004
according to world health organization (WHO).Also,
alcohol use disorders account for 1.4%of the global
disease burden. Globally alcohol consumption causes
3.2% of deaths(1.8million) and 4.0% loss of disability
adjusted life years(58.3million).[1]The life time risk for
alcohol use disorder is more than 15% for men and 1-
year prevalence of alcohol abuse and dependence is
estimated to be 6% or more. [3] In 2015,the estimated
prevalence in the adult population was 18.3% for heavy
episodic alcohol use , age-standardized prevalence was
843.2 per 100,000 people for alcohol and attributable
disability-adjusted life-years(dalys) was 85 million.
3. • Care-giving and alcohol use disorder:
• The primary caregivers are those who provide care to
other family members who need supervision or
assistance in illness or disability or those who provide
unpaid care to the family members with special
needs. [4] Family plays a crucial role in providing
care for mentally ill patients .
• It is often said and known that the life of the family
challenging , in almost every
members living with an alcoholic person is
aspect . Alcoholism
affects not only the one who consumes it ,but also
those surrounding him/her.(5,6)
4. • Caregiver burden:
• Caregiver burden is the stress or strain experienced
by caregivers because of challenges of caregiving.[7]
Caregiver burden was defined by Dillehay R C and
Sandys MR as “a psychological state that results from
the combination of the physical effort, emotional and
social pressure, like the economic restriction that
arise because of providing care to the patients’’.[8]
Caregiver’s burden has two aspects:
• 1.Objective burden-effects on the house hold
including financial loss; effects on health, on children
,and family routine, time spent on care and support ;
and the abnormal behaviors shown by the patient.
• 2.Subjective burden-the extent to which relatives
felt that they carry a burden. Some researchers have
erroneously considered subjective burden as same as
stress.
5. AIMS AND OBJECTIVES
• To estimate the burden of care among the primary
caregivers of patients with alcohol use disorder.
• To estimate the prevalence of depression among the
primary caregivers of patients with alcohol use
disorder.
• To determine the correlation of burden of care and
depression with socio-demographic variables.
• To determine the correlation of burden of care and
depression with alcohol use related variables.
6. INCLUSION CRITERIA
• Primary caregivers of either gender of patients with
alcohol use disorder (aged between 18-65 years)
• Pollak and Perlick (1991) method will be used in the
study to identify primary caregiver 8 who satisfies 3
or more of the following 5 criteria:
1. Spouse, parent or spouse-equivalent
2. Most frequent contact of patient
3. Support patient financially
4. Most frequent collateral participant in patient’s
treatment
5. Person to be contacted in case of emergency.
• Participants who will give consent.
7. EXCLUSION CRITERIA
• Non-consent for participation in the study.
• Other than primary care giver.
• Age of primary care giver (< 18 years, and >65 years)
• Patients with any other serious co-morbidity which is
not the consequence of chronic alcohol use.
8. • STUDY SITE:-
• Study will be conducted in the outdoor/indoor
patients at department of Psychiatry in Smt. S.M.S.
Multispeciality hospital, Chandkheda ,Ahmedabad.
• STUDY POPULATION:-
• Study participants will be the primary caregivers of
patients with alcohol use disorder who fulfill the
inclusion criteria and those who will give written
informed consent.
9. • STUDYDESIGN:-
• This is a Cross-sectional study that will be conducted on the
primary care givers of patients with alcohol use disorder.
Participation for the study will be done only after getting
written approval from SERC committee of Dr. M.K. Shah
Medical college Research centre,Ahmedabad.
• SAMPLING FRAME:
• Average total no. of patients attending OPD/IPD was around
2400 for last 3 years. Sample will be collected by Simple Random
Sampling method.
• SAMPLE SIZEAND SAMPLING
METHOD:-
• N=Z2p(1-p)/d2
• Z=1.96,d= 5%,P=36%
• N =350
10. METHODOLOGY
• The survey is to be conducted in the Psychiatry opd
of S.M.S. Multispeciality hospital, Chandkheda,
Ahmedabad city.
• With the prevalence of 36% ,sample size is 350
primary caregivers of patients with Alcohol use
disorder.
• History and clinical examination of patients with
alcohol use disorder will be done by using the
DSM-5 Criterion.
• Then the primary care-giver will be identified using
the inclusion criteria.
11. • Severity of Alcohol dependence will be assessed by
Severity ofAlcohol Dependence Questionnaire (SADQ).
• If the patient is alone and or with other than primary care
giver, he will be asked to bring the primary care-giver on
the subsequent follow-up.
• We will give semi-structured proforma to participants that
includes demographic details, Zarit’s family burden
interview for assessing the burden among primary
caregivers.
• Study participants will be assessed for depression using
structured interview guide for Hamilton depression rating
scale(HDRS).
• All data thus will be collected, tabulated and analysed
using statistical software statistical package for social
science version 20.0.
