บทคัดย่อ
การนอนไม่หลับเป็นปัญหาสุขภาพที่มีความ
สาคัญและพบได้บ่อยที่สุด ในบรรดาปัญหาของ
การนอนหลับ การนอนไม่หลับส่งผลกระทบต่อการ
ดาเนินชีวิตของบุคคลทาให้เกิดผลเสียในด้านร่างกาย
จิตใจ สังคม อาชีพ และ เศรษฐกิจ ในบทความนี้มี
วัตถุประสงค์เพื่ออธิบายลักษณะและกลไกการเกิดการ
นอนไม่หลับจากปัจจัยต่างๆที่เกี่ยวข้อง โดย
ครอบคลุมตามแนวคิดการนอนไม่หลับของ
สปิลแมน ซึ่งมีปัจจัยร่วมกัน 3 ปัจจัย อันได้แก่ ปัจจัย
โน้มนา (predisposing factors) ปัจจัยกระตุ้น
(precipitating factors) และ ปัจจัยคงอยู่
(perpetuating factors)

Abstract
Insomnia, the most frequent complaint of
all sleep disturbances, is an important health
problem. Insomnia can adversely affect a person’s
life causing significant physical, psychological,
social and professional, and economic
repercussions. The aim of this article is to describe
the characteristics and mechanism of each factor
related to insomnia following the conceptual model
of insomnia by Spielman, classified into 3 factors,
predisposing factors, precipitating factors, and
perpetuating factors.

บทนา
การนอนไม่หลับ (insomnia) เป็นปัญหา
สุขภาพที่พบได้บ่อยที่สุดในบรรดาปัญหาของการนอน
หลับ การนอนไม่หลับ หรือ การนอนหลับ ไม่
เพียงพอ หมายถึง การมีอาการดังกล่าวต่อไปนี้อย่าง
น้อยหนึ่งอาการ หรือ มากกว่าหนึ่งอาการ
ร่วมกันระหว่างการนอนหลับยากเมื่อเริ่มต้นเข้านอน

เหมาะสม เช่น พยายามที่จะนอนหลับตอนกลางวัน
ทดแทนการนอนที่สูญเสียไปในเวลากลางคืน ทาให้ใช้
เวลานานขึ้นบนเตียงนอน ดังจะเห็นได้จากความ
ต้องการการนอนหลับกับการนอนหลับที่หลับได้จริงมี
ความแตกต่างกันมากในผู้ที่นอนหลับได้ไม่ดี
เปรียบเทียบกับในกลุ่มที่นอนหลับไดัดี (Morin &
Gramling, 1989) การใช้เวลาบนเตียงนอนที่มากขึ้น
ก่อให้เกิดเงื่อนไขของการนอนไม่หลับกับเวลานอน
และห้องนอน เป็นเหตุให้การนอนไม่หลับ
รุนแรงขึ้นและเพิ่มความตึงเครียดทางอารมณ์และ
ตอกย้าความเชื่อที่ว่าการนอนไม่หลับนั้นเป็นปัญหาที่
ไม่สามารถควบคุมได้ ส่งผลรบกวนการนอนหลับเป็น
วงจรเช่นนี้เรื่อยไป
สรุป
การนอนไม่หลับมีปัจจัยหลายประการที่
เกี่ยวข้อง การเข้าใจถึงปัจจัยต่างๆ ดังกล่าวมาแล้ว
ข้างต้น ทาให้บุคลากรทางสุขภาพ สามารถให้การ
ดูแลผู้มีปัญหาอย่างครอบคลุม นอกจากนี้ปัจจัย
หลายๆปัจจัยดังกล่าวข้างต้นเป็นปัจจัยซึ่งสามารถ
ปรับเปลี่ยนได้โดยการเปลี่ยนแปลงพฤติกรรม และ
เป็นบทบาทที่สาคัญของพยาบาลที่สามารถส่งเสริม
การมีพฤติกรรมที่เหมาะสมเกี่ยวกับการนอนหลับและ
พัฒนาวิธีการเพื่อการส่งเสริมการนอนหลับที่ดี และ
สามารถและการป้องกันการเกิดปัญหาการนอนไม่
หลับที่มีประสิทธิภาพต่อไป
