As a manager in Ford Rehabilitation centre, I have encountered several challenges in both external environment and internal environment that have greatly challenged the increasing demands of my patients’ services as well as failure of the reimbursements of funds by the insurance providers. Environmental conditions normally affect human health in varied means. Interactions between the environment and human health usually lead to very complex ethical queries that are related to health policy decisions. There are various factors in the environment that can lead to risks and the same time benefits. They include genetically modified plants, nanotechnology, bio fuels and other technology. There is a body of evidence that have emerged saying that environment can affect the health of human being and at the same time human health can have impact to the environment.
The external factors are factors in the environment that cannot be controlled by an organization. There are several external factors that affect many health organizations; these factors include political conditions, government policies and regulations, technological environment and social environment. In my organization the two key external factors affecting my company are the social environment and technological environment. Social factors have developed challenge in the Ford rehabilitation centre. This is because many patient customers have varied and different types of beliefs which make the relations in the health centre challenged. It have become problematic to deal with some patients since it is difficult to know the type of services they need based on where they have come from. Various patients have diverse transformation in attitude towards health care. The patients are however very demanding in my organization because each one of them needs to be handled differently based on community variations. In order to curb this, as manager I have decided to implement several programs that will promote cooperation between my patients as well amendments that will bring in suitable services to each patient. Implementation of this programs will enable my organization to continue being indispensible and financially stable despite the social challenges affecting the availability of patients in the organization.
Another external factor in the environment that will have a great impact in my company is technological environment. Implementation of more advanced methods to serve my customers is likely to improve patients’ attendance and this will boost the compan.

I enjoyed reading your post and I completely agree with your points. I would like to comment on one of your points regarding controlling the quality of care by using independent contractors. There are benefits to this initiatives, but there could be disadvantages too. The benefit of using teams of experts that you did not hire saves the organization costs associated with keeping full time employees, and patients are seen in a timely manner (In some cases). Many years ago, some departsments in a HMO that I work for had access problem such as patients having to wait 3-4 weeks when they need to see a specialist within the organization. No patient with ear infection or difficulty swallowing wants to wait for 3 three weeks to be seen.
We were sending patients outside for urgent MRIs, CT scans, as well as to different specialists even though the company has capabilities to perform some of these functions in house. Apart from the rising costs this created, the level of patients dissatisfaction went through the roof as some patients get to their appointments and were told that referrals that were to be autofaxed to the outside vendors were never received. Some patients were sent away (no referral, no service). Our Utilization Management department was bombarded with approving these external referrals. We have improved, regrouped, and expanded. State of the art facilities were built and still continue to be built, More physicians, nurses, and support staff were hired, and our patient satifaction rate has grown greatly. For example, we used to send our deaf patients to John's Hopkins Hospital for cochlear implants which cost way over hundred thousand dollars, but that's done in house now

Quality of care is a very sensitive subject for every party involved in the healthcare system. They all have different perspectives, each looking at healthcare from a different lens. Patients see quality of care in the results of their treatment and whether their treatment was effective immediately. It can also be measured by how the provider thinks, If a provider were to say that a patient would heal in 2 weeks, the patient would check for the dulling of pain around 2 weeks after the visit. Providers, on the other hand, see quality of care as the credentials that they need to get in order to renew and keep their license to practice.
Quality has its place in the healthcare system, with its positives and negatives. The positives are that it creates an air of steady improvement within competing facilities, and that it encompasses the entire scope of the patient's feelings and their care, such as the friendliness of staff to the patient, and number of services provided. The negatives are that the rating system could list quality as bad for a number of reasons that culminate in the spirit of customer service, and that constant high quality for providers means that their licenses are constantly being improved with items that fit the demanding.

Introduction
In any hospital setting, Patient Care Executives usually are charged with the responsibility of ensuring that patients receive high-quality healthcare. At Holy Cross hospital, this position is created to give the executives an opportunity of managing the patients and allow doctors to focus on treating the patients. This is intended to make the functions of the facility much smoother. Initially, a lot of work was being put on the physicians, leaving them with too little time to attend to the patients and offer the quality of care that is required.
Recently, however, physicians have been complaining that the Patient Care Executives do not perform their duties as expected. As earlier mentioned, the position was created in this facility to ensure that patients are well-taken care of and other management functions are also handled effectively. Generally, it was meant to ensure smooth management of the healthcare organization and provide a smooth system and relationship between the PCEs and the healthcare providers. Physical Environment: How I experience the work environment at any given time, how it feels to work in the organization and to perform in the organization’s culture influences my mood. Within the facility, families visiting their patients have a well-furnished waiting bay with television in which they first rest before being allowed to see their patients. Those with cars can access marked parking lots that are enough. Since the organization is Christian-based, it has a chapel in which patients’ families can gather and pray for their loved ones undergoing medical treatment.
Environmental Safety: As workers, we share values and beliefs that influence our behavior within the organization. Differences in funding, materials, and ability to recruit enough high-quality teachers for summer programs could be more difficult in lower-socioeconomic areas. In addition, the articles did not dive into other factors in the students’ lives that may be contributing to their performance such as attendance, how well-rested they are, trauma they have experiences that impacts their ability to focus during instruction, and the impact of being taught by a teacher who the students may not know or have a relationship with. Additionally, there could be a mismatch between the instructional practices and the specific needs of the students. Even though summer reading programs are only for a short time, I would challenge teachers to put energy into getting to know the students and building trust with them. This is a key foundation that is needed for learning to take place.
In challenging teachers during summer program and the regular school year to ”break out of the mold” to create better outcomes for students classified with low SES, in addition to building relationships with students, I would encourage them to build connections with their families. This may involve thinking outside the box and leaving their comfort zone. It could entail holding a parent-teacher conference off campus, closer to their home or in their community. It could also include providing resources and instructional videos to parents so they can help support their children at home. There are many parents who want to support their children academically, but they do not know how and may be uncomfortable asking the teacher for assistance. In addition, I would urge teachers to capitalize on the strengths and interests of their students to engage them in learning activities and provide them with opportunities to shine. We do not have to, and should not, be satisfied with the idea that low SES students will automatically not be able to perform. These students are capable of learning and growth just as much as any other student. I think data from test scores that demonstrate a gap between the performance of students classified as economically disadvantaged and not economically disadvantaged has led some people to hold the belief that students classified as low SES will not perform well. I think the way that school “report card” grades are published also perpetuates this belief, as it shows the test scores, but does not provide an explanation of or include any solutions for the many larger societal factors that contribute to those scores including high teacher turn over, lack of resources, child trauma, lack of sleep, lack of nutrition, crime & safety, and education level of parents.

I enjoyed reading your post and I completely agree with your points. I would like to comment on one of your points regarding controlling the quality of care by using independent contractors. There are benefits to this initiatives, but there could be disadvantages too. The benefit of using teams of experts that you did not hire saves the organization costs associated with keeping full time employees, and patients are seen in a timely manner (In some cases). Many years ago, some departsments in a HMO that I work for had access problem such as patients having to wait 3-4 weeks when they need to see a specialist within the organization. No patient with ear infection or difficulty swallowing wants to wait for 3 three weeks to be seen.
We were sending patients outside for urgent MRIs, CT scans, as well as to different specialists even though the company has capabilities to perform some of these functions in house. Apart from the rising costs this created, the level of patients dissatisfaction went through the roof as some patients get to their appointments and were told that referrals that were to be autofaxed to the outside vendors were never received. Some patients were sent away (no referral, no service). Our Utilization Management department was bombarded with approving these external referrals. We have improved, regrouped, and expanded. State of the art facilities were built and still continue to be built, More physicians, nurses, and support staff were hired, and our patient satifaction rate has grown greatly. For example, we used to send our deaf patients to John's Hopkins Hospital for cochlear implants which cost way over hundred thousand dollars, but that's done in house now

Quality of care is a very sensitive subject for every party involved in the healthcare system. They all have different perspectives, each looking at healthcare from a different lens. Patients see quality of care in the results of their treatment and whether their treatment was effective immediately. It can also be measured by how the provider thinks, If a provider were to say that a patient would heal in 2 weeks, the patient would check for the dulling of pain around 2 weeks after the visit. Providers, on the other hand, see quality of care as the credentials that they need to get in order to renew and keep their license to practice.
Quality has its place in the healthcare system, with its positives and negatives. The positives are that it creates an air of steady improvement within competing facilities, and that it encompasses the entire scope of the patient's feelings and their care, such as the friendliness of staff to the patient, and number of services provided. Analysis of Risk and Public Health Response
Mona Hanna-Attisha, MD, MPH, Jenny LaChance, MS, Richard Casey Sadler, PhD, and Allison Champney Schnepp, MD
Objectives. We analyzed differences in pediatric elevated blood lead level incidence
before and after Flint, Michigan, introduced a more corrosive water source into an aging
water system without adequate corrosion control.
Methods. We reviewed blood lead levels for children younger than 5 years before
(2013) and after (2015) water source change in Greater Flint, Michigan. We assessed the
percentage of elevated blood lead levels in both time periods, and identified geo-
graphical locations through spatial analysis.
Results. Incidence of elevated blood lead levels increased from 2.4% to 4.9% (P < .05)
after water source change, and neighborhoods with the highest water lead levels ex-
perienced a 6.6% increase. No significant change was seen outside the city. Geospatial
analysis identified disadvantaged neighborhoods as having the greatest elevated blood
lead level increases and informed response prioritization during the now-declared public
health emergency.
Conclusions. The percentage of children with elevated blood lead levels increased
after water source change, particularly in socioeconomically disadvantaged neighbor-
hoods. Water is a growing source of childhood lead exposure because of aging infra-

In April 2014, the postindustrial city ofFlint, Michigan, under state-appointed
emergency management, changed its water
supply from Detroit-supplied Lake Huron
water to the Flint River as a temporary
measure, awaiting a new pipeline to Lake
Huron in 2016. Intended to save money, the
change in source water severed a half-
century relationship with the Detroit Water
and Sewage Department. Shortly after the
switch to Flint River water, residents voiced
concerns regarding water color, taste, and
odor, and various health complaints in-
cluding skin rashes.1 Bacteria, including
Escherichia coli, were detected in the distri-
bution system, resulting in Safe Drinking
Water Act violations.2 Additional disinfec-
tion to control bacteria spurred formation of
disinfection byproducts including total tri-
halomethanes, resulting in Safe Drinking
Water Act violations for trihalomethane
levels.2
Water from the Detroit Water and
Sewage Department had very low corrosivity
for lead as indicated by low chloride, low
chloride-to-sulfate mass ratio, and presence
of an orthophosphate corrosion inhibitor.3,4
By contrast, Flint River water had high
chloride, high chloride-to-sulfate mass ratio,
and no corrosion inhibitor.5 Switching
from Detroit’s Lake Huron to Flint River
water created a perfect storm for lead leach-

Objectives
This chapter introduces the analysis of the time response of different
control systems under different scenarios. Only first and second order
systems will be considered in details using analytical and numerical
methods. Extension to higher order systems will be developed. Both
transient and steady state responses will be evaluated. Stability analysis

effect of both proportional and derivative control actions on the

Jon Smith spread his arms widely as he surveyed his garden centre.
‘Of course the whole market for leisure products and services, especially garden-related products, has been expanding over the last few years. Even so, we have been particularly successful. Partly this is because we are conveniently located, but it is also because we have developed a reputation for excellent service. Customers like coming to us for advice. We have also been successful in attracting some of the ‘personality gardeners’ from television to make special appearances. My main ambition now is to fully develop all of our twelve hectares to make the centre a place people will want to visit in its own right. I envisage the centre developing into almost a mini gardening theme park with special gardens, beautiful grounds and special events.’
Greenland is a large village situated in the Cotswolds, a popular tourist area of the UK. It has an interesting range of shops and restaurants, mainly catering for the tourist trade. About half a mile outside the village is the Greenland Garden Centre. The garden centre is served by a good network of main roads but is inaccessible by public transport.
Growth over the last five years has been dramatic and the garden centre now sells many other goods as well as gardening requisites. It also has a restaurant. It is open seven days a week, only closing on Christmas Day. Its opening hours are Monday– Saturday 9 a.m. to 6 p.m. and Sunday 10 a.m. to 5 p.m. all year round.
Outside the centre
The centre has a large car park which can accommodate about 350 cars. Outside the entrance a map indicates the various areas in the garden centre. Most customers walk round the grounds before making their purchases. The length of time people spend in the centre varies but, according to a recent study, averages 53 minutes during the week and 73 minutes at weekends.
The same study shows the extent to which the number of customers arriving at the garden centre varies depending on the time of year, day of the week, and time of day. There are two peaks in customer numbers, one during the late spring/early summer period and another in the build up to Christmas, as Greenland puts on particularly good Christmas displays.

The range of goods has increased dramatically over the past few years and now includes items such as:

The conv Command
conv(x,h) performs a 1-D convolution of vectors 𝑥 and ℎ. The resulting vector 𝑦
has length length(𝑦) = length(𝑥) + length(ℎ) − 1. Imagine vector 𝑥 as being
stationary and the flipped version of ℎ is slid from left to right. Note that conv(x,h) =

Deconvolution
The command [q,r] = deconv(v,u), deconvolves vector u out of vector v, using long
division. The quotient is returned in vector q and the remainder in vector r such that
v = conv(u,q)+r. If u and v are vectors of polynomial coefficients, convolving them is
equivalent to multiplying the two polynomials, and deconvolution is polynomial
division. The result of dividing v by u is quotient q and remainder r. An examples is

Introduction
The organization that a health care facility can distinguish will
determine its potential success and the longevity of that success
because preparation, patience and organization are vital. Health
care facilities are constantly changing, advancing and
revamping. With this being a known unknown there is a major
need for organization throughout the entire infrastructure of the
organization. Organization is highly important because it
allows for facilities to function and run smoothly with little to
no confusion within each department acting as a well-oiled
machine (Cruz, 2013 p.472). It is very difficult for a facility or
organization to protect and flourish if there is no structure
present. After witnessing first-hand the lack of proper structure
and management take place, this caused a great deal of
confusion from the upper management as they could not
decipher the numerous departments’ primary responsibilities
and this took a huge toll on their success. This particular lack
of structure can and will decide an organizations fate if not
fixed immediately. Healthcare facilities management must be in
a sense bulletproof because of the many adversities which can
and will arise as the healthcare realm changes. The healthcare
realm changes and overlaps its previous methods for rules and
2. regulations. This comes about due to the constant need for
revitalization within the healthcare realm. If a facility lacks
organization there is no structure and no sense of direction it
will most likely fail. Of course there are spurts of success and
lagging success for every healthcare facility today. But the
elite healthcare facilities always find a way to revamp and
restructure in order to flourish. The most important entity any
facility or organization can acquire is the loyalty of its
consumers. Loyalty has been proven to provide not only
success but longevity of success for healthcare facilities and if
health providers implement this type of atmosphere success is
prominent.
Hospital Organizational Structure
In today’s’ world there is a major need for organizational
structure, rules and regulations that not only govern the conduct
within an organization but also protects customers’ rights and
interests. The structuring of a hospital needs to be configured
in a strategic way that will benefit all of its occupants
regardless of what their economic and ethnic background might
be. Without proper structure and legislation it’s highly difficult
for a hospital to thrive within the healthcare realm. One will be
able to recognize by the end of this reading a well-organized
structured system for any hospital organization to thrive for
many years to come and that is due to the proper planning,
methods and steps that are outlined to construct them.
There are four major departments which are the largest and most
noticeable departments in a hospital organization. The most
extensive department is human resources management and it
consist of nearly over 100 highly trained professionals with
organization, management, and efficiency skills within human
resource management. Secondly the finance department which
includes insurance and claims management is also one of the
most viable departments that is required to maintain a reliable
and well maintained hospital organization. The insurance claim
department consists of over seventy-five highly trained
3. professionals in claims and financial recovery. The majority of
the finance department which oversees the hospitals budget for
every department and allocates moneys toward the success of
the hospital also including advertisement, outreach, and new
start up projects, is the finance system management department
and they consist of over 100 auditors, accountants, and finance
professionals. This department is to manage, reconcile and
distribute the financial funds to and from the hospital itself
while also distributing the salaries employees, vendors and
partners. The third organization, which is the heartbeat of the
hospital organization is the professional and paraprofessional
healthcare providers. Without physicians, nurses, technicians,
maintenance, receptionist, and the entire staffing from top to
bottom, there is no organization. These front line workers are
the essential resource needed for the organization to run and
have continued success. The fourth largest entity is the
inventory and equipment management department and this
section houses over sixty-five inventory management and office
management trained professionals. Each department has
described skills and duties that also overlap between each other
to ensure safety and fluency throughout the facility, and there is
extensive training and seminars provided. For each department
these duties include analysis, experiments, and trials to ensure
that our targeted markets are satisfied with our performance,
quality of care, and quality of products.
Mission and Value Statements
In any hospital the most important asset that can be acquired are
the patients and customers that are served on a daily basis and
as an organization in the field of healthcare we are obligated to
supply the highest quality of care. A healthcare organization is
also obligated to provide the highest quality of products and
services that highly trained professionals can create to ensure
customers overall satisfaction, health and happiness. Providing
the highest quality of care, products and services leads to
success within the ever changing healthcare realm and that is
the overall goal to serve and satisfy the consumers and patrons
4. of this organization. The mission statement is as follows “To
provide the highest quality of care, products and services known
to man and everything else will fall into place”. I chose to
establish this bold and high risk mission statement because our
dream is much bigger and brighter than our competitors. In
order to ensure that the organization is upholding its’ end of the
bargain there are extensive and continued trainings in quality
care, patient service and in compassion. These three
characteristics will determine a facilities success because if we
lack in any of those three areas it will decide the longevity of
success for us all. The development for my particular mission
statement began with a small idea that came to me in a dream
one night before the grand opening. The mission statement
agreed upon by the governing board before, lacked definitive
style and taste which read “A hospital you can consider home,
as we are all family”. This generic statement was missing spark
and compassion however, in a round table discussion with
brilliant minds, what surfaced was the more direct mission
statement to our board of trustees it was a hit. The board
agreed that the new mission statement will attract and keep our
facility above the competitors while actually providing high
quality care and treatment to our customers. Once it was
decided we immediately changed the original statement and
received high praise from the patrons about the new mission
statement. This I can say was a huge success which started off
as a simple dream and now is a complete and destined reality
for our customers benefit.
Feasibility Plan for Hiring
Feasibility is always considered one of the hardest entities to
implement because everyone wants to acquire the best of the
best but may not be able to afford those attributes. That is
where the feasible part comes in which most nearly means to
work and strive within your means until your facility and
organization can obtain the high quality and quantity entities. I
have come up with a feasible plan for the hiring of nurses,
physicians and allied professionals. Since we have been blessed
5. with the opportunity to be sponsored by our neighboring
communities and global sponsors who believe in our mission
statement we can acquire these entities. The hiring process will
be extensive and managed directly so that we are effectively
screening, tasking and analyzing a potential nurse’s abilities
and quality of customer service. The nurse will be evaluated by
his or her prior experience and skill set in which we will also
contact their references and educational institute for a deeper
access to their personalities. Once chosen there will be a
probationary period that will enable us as an organization to
evaluate and assess the new hires abilities, compassion and
quality of customer service. The hiring physician’s process will
also be very extensive because nurses and physicians directly
affect our patient’s daily lives whether it’s with diagnostics,
surgery or physical therapy. Furthermore there will be the
hiring of healthcare professionals to ensure that we uphold our
innovative mission statement. There hiring process will also be
very extensive as these healthcare professionals directly interact
with our patients and customers and must possess high quality
attributes. As far as feasibility goes we do not have a certain
financial limit on whom and what we can afford but we are
conscious of our spending and our financial department
monitors that. When hiring these nurses, physicians and
healthcare professionals we will ensure that we hire the most
experienced and also the most compassioned medical
professional we can find. This will give our facility the edge
over our competitors because we have acquired the top nurses,
physicians and healthcare professionals in the country and are
all fully capable of providing quality care.
Use of Technology for Patient Services
Technology is the key to evolution in todays’ world and it will
continue to provide gateways of knowledge and success for
healthcare professionals and medical professionals to utilize.
“The pressure to be innovative, technologically advanced in
order to improve the quality of services has resulted in a
significant increase in spending whilst at the same time
6. governments are faced with budget constraints” (Guah, 2011
p.12) Improving technology for the patient services will only
prove as a reliable source of the known unknown information
that many patients and consumers will benefit from having this
access. The information in the past that was passed along from
physicians to nurses and later to the patient were substantial in
that era of time because of the limited technology that was
accessible. If there is no evolution of information technology
there would be more frequent untimely deaths and illnesses
because of the lack of knowing how a patients’ health is
progressing as they age. Lacking in information technology
proves to be a major problem as we look at hospitals access to
limited technology in the past to now in the 21st century where
highly advanced technology is present. (Pogzar 2012) The need
for information technology advances has never been more in
demand especially now that more and more people become well
aware and interested in their health and families health.
Honestly if there was no information technology advancement
within the world let alone the healthcare realm then there would
be higher spikes in illnesses. I also believe that the untimely
death toll will become much larger than normal because of that
very fact of lack of technology present. This technology is
utilized to perform screenings, tests and analyses of patients’
current health, and the patients’ health as they age and progress
in life. There is undoubtedly a high importance in the demand
and need for information technology to advance and progress
lite years faster than we as humans advance and progress to stay
ahead. This is to ensure that any probable epidemics or
scenarios that may play out can be accessed and solved before a
major outbreak or recurring scenario becomes present. These
are the reasons why I believe the use of advanced information
technology is highly needed and important.
Conclusion
In conclusion organizational management of a healthcare
facility is one of the most important entities that a facility can
acquire because it directly affects the customers. Also having
7. no sense of organization or management will eventually take a
toll on the potential success that a facility can have because of
the lagging organization and management. Customers, patients
and potential investors can sense when there is little or no
organization and this will turn those potential customers away
from that facility and cause outshopping to occur. It is highly
important and valuable to appeal to the most important entity an
organization can acquire which is the very customers and
patients it serves on a daily bases. Organization helps to
maintain important files and documents while also providing a
safety net in case something goes wrong as far as treatment is
concerned the well-organized facility will be able to pinpoint
this problem and immediately act on it. I believe this is true
because without customers or investors there is no need for a
hospital or organization to exist because there is no one to serve
or provide treatment. These are the reasons why I believe that
organization and management of a healthcare facility are the
most important qualities to possess.
